OBJECTIVE Evaluate adherence to the checklist of the Programa Cirurgias Seguras (safe surgery programme) at a teaching hospital. METHODS Evaluative study conducted at a teaching hospital in the south of Brazil in 2012. Data were collected by means of non-participant observation in 20 hip and knee replacement surgeries and an instrument that was created for research based on the checklist and used by the institution. RESULTS In the observed procedures (n=20) there was significant adhesion (p<0.05) to the instrument in relation to the verification of documentation, fasting, hair removal in the surgical site, absence of nail varnish and accessories, identification of the patient and surgical site on admission to the surgical unit, availability of blood and functionality of materials. However, there was no significant adherence to the checklist in the operating room in relation to patient identification, procedure and laterality, team introduction, surgical break and materials count. CONCLUSION The results showed that the items on the checklist were verified nonverbally and there was no significant adherence to the instrument.

Objective To characterize arthroplasty procedures, calculate the surgical infection rate and identify related risk factors. Methods This was a retrospective cohort study. Data on operations performed between 2010 and 2012 were gathered from documental sources and were analyzed with the aid of statistical software, using Fishers exact test, Students t test and the nonparametric Mann–Whitney and Wilcoxon tests. Results 421 total arthroplasty procedures performed on 346 patients were analyzed, of which 208 were on the knee and 213 on the hip. It was found that 18 patients (4.3%) were infected. Among these, 15 (83.33%) were reoperated and 2 (15.74%) died. The prevalence of infection in primary total hip arthroplasty procedures was 3%; in primary total knee arthroplasty, 6.14%; and in revision of total knee arthroplasty, 3.45%. Staphylococcus aureus was prevalent. The length of the surgical procedure showed a tendency toward being a risk factor (p = 0.067). Conclusion The prevalence of infection in cases of primary total knee arthroplasty was greater than in other cases. No statistically significant risk factors for infection were identified.

Objective: To analyze and relate the registration of information and content of checklists with the objectives of the Safe Surgery Saves Lives Program. Methods: Documentary study with 257 checklists from orthopedic surgeries performed between 2011 and 2012 in a Brazilian hospital. The 12,629 items related to surgical safety were entered into an Excel spreadsheet and were analyzed using the Statistical Package for the Social Sciences. Results: 99.8% of the checklist items were checked and the content of the records showed, through documental checks, no guarantee of safety elements relative to the correct surgical site (objective 1), blood loss (objective 4), allergic reaction (objective 5) retention of instruments/compresses (objective 7), identification of surgical specimens (objective 8) and communication (objective 9). Conclusion: The high adherence to completing the checklists allowed potential surgical risks arising from unconfirmed safety actions to be identified, requiring actions seeking to qualify the care.

OBJECTIVE To investigate compliance of national patient safety protocols in Emergency Care Units (UPA) of the Paraná State. METHOD From April until September 2016, the exploratory stage of the action research was conducted on stratified sampling with 377 patients of eight units, with use of verification instrument of basic safety actions. RESULTS The absence of systematic identification of patients, fall risk assessment and signaling and development of pressure injuries were evidenced. We observed that 52.8% of parenteral solutions in use were not identified and that, in only 29.4% of the cases, the allergic condition was investigated. In 80.6% of the emergency units there was availability of alcoholic solution to hand hygiene. CONCLUSION We concluded that the non-compliance of basic actions concerning patient safety exposes users to preventable adverse events and demands systematized actions to comply with government guidelines and promote quality of health assistance.
