Attitudes of nursing staff related to the nursing process

The aims of the study were to describe nurses’ positions on nursing process and their perception of power; and to analyse asso ciations between positions on nursing process, power perception and selected variables. One thousand six hundred and five nurses (86.9% female, mean age=44.12 years, SD=9.55) participated in the study. Mean score on the Positions on Nursing Process (PNP) tool was 112.37 (SD=22.28); and on the Power as Knowing Participation in Change Tool – Brazilian Version (PKPCT) was 281.12 (SD=38.72). Baccalaureate nurses had statistically higher scores on PNP and PKPCT than auxiliary nurses. There was positive and moderate correlation between PNP and PKPCT scores. Auxiliary nurses’ scores on PNP were associated with sex and post-graduation; auxiliary nurses’ scores on PKPCT were associated with sex. For baccalaureate nurses there was association between PKPCT and admi nistrative position. More studies should be developed in order to identify variables po tentially associated with the use of nursing process in clinical practice.

Atitudes dos profissionais de enfermagem relacionadas ao Processo de Enfermagem

Sleeping difficulties reported by patients with heart failure

The study aimed to describe the reports of heart failure patients on the factors that cause difficulties to sleep and the association of these factors with the quality of sleep. This cross-sectional study involved a non-probabilistic sample of 400 patients (mean age 57.8 years, 64.8% were men, average education of 6.1 years, 82.5% in functional class II or III) with heart failure. The main factors associated with sleeping difficulty were: nocturia, interrupted sleep at night and breathing difficulty. Sleeping difficulties in heart failure patients are diverse and there is an association between these difficulties and quality of sleep. Nursing process documentation: rationale and methods of analytical study

OBJECTIVE to describe the methods used to analyze the associations between variables of service, nursing and the nursing process documentation in institutions of the Department of Health of the State of São Paulo. METHOD multilevel analytical study with data obtained in the domains of institution, units of the institution and nursing professionals who work there, using standardized instruments. The analyses had as axis the degree of completeness of the nursing process documentation in units or institutions and their association with variables of nursing personnel, of units and of institutions. CONCLUSION This study will provide important empirical evidence on the factors involved in the nursing process documentation.

Adaptation and validation of the instrument positions on the nursing process

OBJETIVO: estimar las propiedades psicometricas de la adaptacion del instrumento Posicoes frente o diagnostico de enfermagem para evaluar las actitudes del personal de enfermeria respecto al proceso de enfermeria. METODO: estudio metodologico con muestra no probabilistica de 973 auxiliares de enfermeria y 632 enfermeros de 35 hospitales y ambulatorios vinculados a la Secretaria del Estado de la Salud de Sao Paulo. La validez del instrumento de 20 items fue verificada mediante el analisis factorial confirmatorio que identifico un factor mas general en segundo nivel, formado por los tres factores clasicos de actitudes. La confiabilidad correspondio a 0,954 para el instrumento total. RESULTADOS: el instrumento Posicoes sobre o Processo de Enfermagem posee validez y confiabilidad adecuadas. CONCLUSIONES: se recomienda verificar en estudios futuros si los items presentan comportamiento diferencial segun la categoria profesional del respondiente y verificar el comportamiento del instrumento con estudiantes

Informatics competencies essential to decision making in nursing management

OBJECTIVE To identify informatics abilities essential to decision making in nursing management. METHOD Survey study with specialist nurses in health informatics and management. An electronic questionnaire was built based on the competencies Information Literacy (five categories; 40 abilities) and Information Management (nine categories; 69 abilities) of the TIGER - Technology Informatics Guiding Education Reform - initiative, with the guiding question: Which informatics abilities are essential to decision making in management? Answers were sorted in a Likert scale, ranging from 1 to 5. Rasch analysis was conducted with the software WINSTEPS(®). Results were presented in logits, with cutoff value zero. RESULTS Thirty-two specialists participated, coming from all regions of Brazil. In the information literacy competency, 18 abilities were considered essential and in Information Management, 38; these were sorted according to their degree of essentiality. CONCLUSION It is believed that the incorporation of these abilities in teaching can support the education of nurse managers and contribute to evidence-based practice, incorporation of information and communication technologies in health and information management.Resumo OBJETIVO Identificar as habilidades de informatica essenciais a tomada de decisao no gerenciamento em enfermagem. METODO Estudo survey com enfermeiros especialistas em informatica em saude e gerenciamento. Foi construido questionario eletronico baseado nas competencias Informacional (cinco categorias; 40 habilidades) e Gestao da Informacao (nove categorias; 69 habilidades) da iniciativa TIGER - Tecnologia da Informacao Guiando a Reforma Educacional, partindo da pergunta: Quais sao as habilidades de informatica essenciais para a tomada de decisao no gerenciamento? Respostas foram indicadas em escala Likert, variando de 1 a 5. Foi realizada analise Rasch com o software WINSTEPS, os resultados foram apresentados em logits, com valor de corte zero. RESULTADOS Participaram 32 especialistas, com representacao de todas as regioes do Brasil. Na competencia Informacional 18 habilidades foram consideradas essenciais e na Gestao da Informacao 38 habilidades; estas foram ordenadas segundo seu grau de essencialidade. CONCLUSAO Acredita-se que a incorporacao dessas habilidades no ensino possa sustentar a formacao de enfermeiros gestores e contribuir com a pratica baseada em evidencias, incorporacao de tecnologias de informacao e comunicacao em saude e gestao da informacao.

Burnout and nursing work environment in public health institutions

Objective: to identify associations between the Burnout domains and the characteristics of the work environment. Method: cross-sectional study with 745 nurses from 40 public health institutions in São Paulo. Nursing Work Index–Revised (NWI-R) and Maslach Burnout Inventory were used. Similar institutions according to NWI-R were grouped by clustering and the Anova and Bonferroni tests were used in the comparative analyzes. Results: there was signifi cant and moderate correlation between emotional exhaustion and autonomy, control over the environment and organizational support; between reduced personal accomplishment, autonomy and organizational support; and between depersonalization and autonomy. The group that presented the worst conditions in the work environment differed on emotional exhaustion from the group with most favorable traits. Conclusion: emotional exhaustion was the trait of Burnout that was more consistently related to the group of institutions with more unfavorable working conditions regarding autonomy, organizational support and control over the environment. RESUMEN Objetivo: identifi car asociaciones entre los dominios de Burnout y las características del ambiente de trabajo. Método: estudio transversal entre 745 enfermeros de 40 instituciones públicas de salud de São Paulo. Se utilizaron, la Escala Adaptada del Entorno de Práctica Enfermera (Nursing Work Index–Revised, NWI-R) y el cuestionario Maslach Burnout Inventory. Se agruparon instituciones semejantes por el método de Cluster, según la NWI-R y se aplicaron las pruebas Anova y Bonferroni en los análisis comparativos. Resultados: hubo correlación signifi cativa y moderada entre agotamiento emocional y autonomía, control sobre el ambiente y soporte organizacional; baja realización personal y autonomía y soporte organizacional; despersonalización y autonomía.
