Stress cardiomyopathy (Tako-Tsubo) following radiofrequency ablation in the right ventricular outflow tract

A 44-year-old female undergoing radiofrequency ablation in the right ventricular outflow tract for symptomatic, frequent premature ventricular contractions developed stress cardiomyopathy (Tako-Tsubo). Stress cardiomyopathy was probably due to hyperadrenergic state induced by the procedure itself, dobutamine infusion, sympathetic nerve stimulation at the ablation site, and parasympathetic withdrawal with atropine administration.

Right ventricular (RV) function has great impact on the survival of heart transplantation recipients; therefore, careful evaluation is of high clinical importance. However, there is no standard conventional echocardiographic parameter to assess RV systolic function. Herein, we evaluated the correlation between echocardiographic parameters of RV systolic function and ejection fraction assessed by cardiac magnetic resonance imaging (MRI RVEF) in heart transplantation recipients.

Purpose Right ventricular functions have affects on survival of heart transplantation patients. There is no standart echocardiographic evaluation methods for RV systolic functions. In this study, the correlation between the echocardiographic parameters for evaluating the RV systolic function and right ventricular ejection fraction (RVEF) calculated by cardiac magnetic resonance imaging (MRI- MR RVEF), are analyzed. Methods and Materials 43 Patients with at least 6 months over heart transplantation were included in this study. First echocardiography (2D, tissue Doppler and strain imaging) and cardiac MRI and afterwards endomyocardial biopsy and right heart catheterization were performed in six hours period. Results Only the echocardiographic parameter RV FAC was associated with MRI RVEF (r = 0.747, p 50% and MR RVEF ≤ 50%, RVFAC 48,5% value had 90,5% sensitivity and 90,5% specificity.(AUC:0,96) In the case grup defined as “normal” (n = 30). MR RVEF= 44.5% was considered as the lower referance limit and MR RVEF Conclusions RVFAC is the only parameter correlated with MR RVEF, should be used for heart transplantation patients rutine follow up.
