Effectiveness of the electronic cigarette: An eight-week Flemish study with six-month follow-up on smoking reduction, craving and experienced benefits and complaints.

Background: Smoking reduction remains a pivotal issue in public health policy, but quit rates obtained with traditional quit-smoking therapies remain disappointingly low. Tobacco Harm Reduction (THR), aiming at less harmful ways of consuming nicotine, may provide a more effective alternative. One promising candidate for THR are electronic cigarettes (e-cigs). The aim of this study was to investigate the efficacy of second-generation e-cigs both in terms of acute craving-reduction in the lab and in terms of smoking reduction and experienced benefits/complaints in an eight-month Randomized Controlled Trial (RCT). Design: RCT with three arms. Methods: Participants (N = 48) unwilling to quit smoking were randomized into two e-cig groups and one control group. During three lab sessions (over two months) participants, who had been abstinent for four hours, vaped/smoked for five minutes, after which we monitored the effect on craving and withdrawal symptoms. eCO and saliva cotinine levels were also measured. In between lab sessions, participants in the e-cig groups could use e-cigs or smoke ad libitum, whereas the control group could only smoke. After the lab sessions, the control group also received an e-cig. The RCT included several questionnaires, which repeatedly monitored the effect of ad libitum e-cig use on the use of tobacco cigarettes and the experienced benefits/complaints up to six months after the last lab session. Results: From the first lab session on, e-cig use after four hours of abstinence resulted in a reduction in cigarette craving which was of the same magnitude as when a cigarette was smoked, while eCO was unaffected. After two months, we observed that 34% of the e-cig groups had stopped smoking tobacco cigarettes, versus 0% of the control group (difference p < 0.01). After five months, the e-cig groups demonstrated a total quit-rate of 37%, whereas the control group showed a quit rate of 38% three months after initiating e-cig use. At the end of the eight-month study, 19% of the e-cig groups and 25% of the control group were totally abstinent from smoking, while an overall reduction of 60% in the number of cigarettes smoked per day was observed (compared to intake). eCO levels decreased, whereas cotinine levels were the same in all groups at each moment of measurement. Reported benefits far outweighed the reported complaints. Conclusion: In a series of controlled lab sessions with e-cig naïve tobacco smokers, second generation e-cigs were shown to be immediately and highly effective in reducing abstinence induced cigarette craving and withdrawal symptoms, while not resulting in increases in eCO. Remarkable (>50 pc) eight-month reductions in, or complete abstinence from tobacco smoking was achieved with the e-cig in almost half (44%) of the participants.

Associative learning of likes and dislikes: a review of 25 years of research on human evaluative conditioning.

Evaluative conditioning refers to changes in the liking of a stimulus that are due to the fact that the stimulus has been paired with other, positive or negative stimuli. Although evaluative conditioning appears to be subjected to certain boundary conditions, significant evaluative conditioning effects have been obtained using a large variety of stimuli and procedures. Some data suggest that evaluative conditioning can occur under conditions that do not support other forms of Pavlovian conditioning, and several models have been proposed to account for these differences. In the present article, the authors summarize the available literature, draw conclusions where possible, and provide suggestions for future research.

Evaluative Conditioning in Humans: A Meta-Analysis

This article presents a meta-analysis of research on evaluative conditioning (EC), defined as a change in the liking of a stimulus (conditioned stimulus; CS) that results from pairing that stimulus with other positive or negative stimuli (unconditioned stimulus; US). Across a total of 214 studies included in the main sample, the mean EC effect was d = .52, with a 95% confidence interval of .466-.582. As estimated from a random-effects model, about 70% of the variance in effect sizes were attributable to true systematic variation rather than sampling error. Moderator analyses were conducted to partially explain this variation, both as a function of concrete aspects of the procedural implementation and as a function of the abstract aspects of the relation between CS and US. Among a range of other findings, EC effects were stronger for high than for low contingency awareness, for supraliminal than for subliminal US presentation, for postacquisition than for postextinction effects, and for self-report than for implicit measures. These findings are discussed with regard to the procedural boundary conditions of EC and theoretical accounts about the mental processes underlying EC.

When somatic information threatens, catastrophic thinking enhances attentional interference

&NA; Styles of catastrophic thinking about pain have been related to an inability to divert attention away from pain. We investigated whether pain catastrophizers displayed high attentional interference during a threatening low‐intensity electrocutaneous stimulus (ES). In Experiment 1, 44 undergraduates performed a tone discrimination task whilst experiencing several times an ES on the left or right arms. Tones were also presented 250 ms and 750 ms after ES onset. Participants were threatened that a high‐intensity painful stimulus would occur at one site. As predicted, pain catastrophizers displayed pronounced task interference immediately after threat stimulus onset. In Experiment 2, threat was induced in 36 undergraduates by informing them that an ES excites pain fibres. Again, catastrophizers had marked interference immediately after onset. The results are discussed in terms of how catastrophizing amplifies somatosensory information and primes fear mechanisms.

Contingency awareness in evaluative conditioning: A case for unaware affective-evaluative learning

Abstract According to Martin and Levey (1987) evaluative conditioning is different from signal learning, i.e. the acquisition of knowledge about predictive relations between environmental events. The hypothesis was tested that evaluative conditioning, unlike signal learning, does not require awareness of the CS-US contingency. In three pilot experiments it was demonstrated that pairing neutral stimuli with either liked or disliked stimuli is sufficient to change neutral stimuli into a positive or negative direction. As indicated by postconditioning recognition questionnaires, this evaluative shift did not require and was not even influenced by contingency awareness. These findings were replicated and corroborated in an experiment, using a concurrent awareness assessment procedure and more fine-grained evaluative response measurements. The relevance of this conditioning without contingency awareness is discussed in the context of recent information processing models of Pavlovian conditioning.

On the generality of the affective Simon effect

In affective Simon studies, participants are to select between a positive and negative response on the basis of a nonaffective stimulus feature (i.e., relevant stimulus feature) while ignoring the valence of the presented stimuli (i.e., irrelevant stimulus feature). De Houwer and Eelen (1998) showed that the time to select the correct response is influenced by the match between the valence of the response and the (irrelevant) valence of the stimulus. In the affective Simon studies that have been reported until now, only words were used as stimuli and the relevant stimulus feature was always the grammatical category of the words. We report four experiments in which we examined the generality of the affective Simon effect. Significant affective Simon effects were found when the semantic category, grammatical category, and letter-case of words was relevant, when the semantic category of photographed objects was relevant, and when participants were asked to give nonverbal approach or avoidance responses on the basis of the grammatical category of words. Results also showed that the magnitude of the affective Simon effect depended on the nature of the relevant feature.

Once in contact always in contact: Evaluative conditioning is resistant to extinction

Abstract The present study aimed at obtaining some further support for the hypothesis of a distinction between two basically different kinds of learning in a Pavlovian conditioning preparation: signal-learning and affective-evaluative learning ( Baeyens et al. , 1988a,b ; Levey and Martin, 1987 ). In this respect, we conducted an experiment to verify the Levey and Martin (1983, 1987) hypothesis that, unlike signal-learning, evaluative conditioning should be resistant to extinction. Mere contingent presentation of neutral with (dis)liked stimuli was sufficient to change the affective—evaluative tone of the originally neutral stimuli in a (negative) positive direction ( p not have any influence on the acquired evaluative value of the originally neutral stimuli ( p p et al. , 1986).

Human evaluative conditioning: Acquisition trials, presentation schedule, evaluative style and contingency awareness

Two different processes may be operative in human Pavlovian conditioning: signal learning and evaluative learning. Whereas most studies on evaluative conditioning focused on a mere demonstration of the phenomenon or on a theoretical analysis of the underlying processes, some basic parameters of evaluative learning are still unexplored. Hence, using the standard neutral picture--(dis)liked picture pairing paradigm (Baeyens, Eelen & Van den Bergh, 1990), in this study the effect of two parameters of evaluative conditioning was assessed on a between-subjects base, namely the Number of Acquisition Trials (2/5/10/20) and the Presentation Schedule of the stimulus pairs (blockwise or random). Additionally, the study included an exploratory analysis of the potential effects of the Evaluative Style of subjects (Feelers vs Thinkers, operationalized in terms of speed of emitting evaluations). Finally, the relationship between contingency awareness and evaluative learning was reassessed. Neutral-liked conditioning was found to be quadratically related to the number of acquisition trials (increase in effect up to 10 trials, decrease from 10 to 20 trials), whereas neutral-disliked conditioning linearly increased with increasing numbers of trials. Randomized vs blockwise presentation schedules of the stimulus pairs did differentially affect the overall pattern of conditioning, but in a way which was both unexpected and difficult to account for theoretically. Both the Evaluative Style of subjects and contingency awareness were demonstrated to be generally orthogonal to conditioned shifts in CS valence. Based on these findings, some practical suggestions are provided for the application of evaluating conditioning based therapeutical interventions to affective-behavioral disorders which are centred around inappropriate (dis)likes.

Affect 4.0: A free software package for implementing psychological and psychophysiological experiments

We describe Affect 4.0, a user-friendly software package for implementing psychological and psychophysiological experiments. Affect 4.0 can be used to present visual, acoustic, and/or tactile stimuli in highly complex (i.e., semirandomized and response-contingent) sequences. Affect 4.0 is capable of registering response latencies and analog behavioral input with millisecond accuracy. Affect 4.0 is available free of charge.

Avoidance and Confrontation of Painful, Back-Straining Movements in Chronic Back Pain Patients

Avoidance of painful activities has been proposed to be an important nsk factor for the initiation and maintenance of chronic low back suffering, whereas exposure to these activities has been suggested to be beneficial for recovery. In a cross-sectional study, the differences between chronic patients with avoidant and confrontational styles were investigated using self-report measures and a behavioral test. Participants were first classified as avoiders or confronters. In comparison with confronters, avoiders reported greater frequency and duration of pain, higher fear of pain and injury, more disability in daily living, and more attentiorr to back sensations. Finally, avoiders reported more fear of (re)injury during the behavioral test and had a worse performance than confronters. The results suggest a close link between the fear of pain/(re)injury on one hand and avoidance behavior and physical deconditioning on the oither hand.
