The criteria listed in the fourth edition of the Diagnostic and Statistical Manual (DSM-IV) from the American Psychiatric Association are the most used ones for the diagnosis of Attention-Deficit Hyperactivity Disorder (ADHD) being based on field studies with children and adolescents. The Adult Self-Report Scale (ASRS, version 1.1) was developed to adapt those symptoms to an adult life context. The present study consisted of a transcultural adaptation of the original instrument in English into a final version to be used in Brazil. Results indicated a satisfactory equivalence between versions, with some modifications being done after debriefing, supporting the importance of this step in studies like this.

Diagnosticando o TDAH em adultos na prßtica clínica

Although attention deficit hyperactivity disorder (ADHD) has been officially recognized as a valid disorder into adulthood, this diagnosis is still an issue of debates between specialists. Most of the existing knowledge regarding this condition is due to the excess of findings of studies with children. As the field studies that determined DSM-IV diagnostic criteria comprised only children and adolescents, more studies with adult populations are required to validate the criteria for this population. The age of onset criteria, the cut-off of six symptoms and the requirement of impairment in at least two different settings are some of the difficulties faced by clinicians to make the diagnosis in clinical practice. Moreover, adults with ADHD often show difficulties to recall the symptoms in childhood. The symptoms described by DSM-IV are also criticized. To minimize the aforementioned problems, the use of instruments adapted to adults (as ASRS-18), reports of other informants (like partner and parents), the broadened of the age of onset criterion and a broaden investigation of the different settings that might be impaired by the symptoms are recommended.

Urticária aguda como manifestação de infecções virais na infância

Objective: To report serial cases of acute urticarial associated with viral infections. Material and Methods: Serial report cases of children diagnosed with acute urticaria associated with viral infections and literature review. Results: Seven patients were evaluated, three of them female, with an average age of 3 years. Three patients had urticarial lesions, without other associated symptoms. The remaining patients presented a previous characteristic of upper airways. All were evaluated in emergency service, medicated with antihistamines and referenced for evaluation by specialist in allergy and immunology. The urticaria remission period ranged from 5 to 15 days. In the diagnostic investigation, two children presented reactive IgM for Parvovirus B19, three presented reactive IgM for Epstein Barr virus (EBV), one presented reactive IgM for EBV and for Herpes Simplex virus I and II, and one had reactive IgM for Herpes Simplex I and II. Conclusion: Acute urticaria is a commom childhood disease with detailed clinical history and physical examination essential for the etiological diagnosis. The pediatrician should be aware of the main triggering factors, including viral infections.
