RATIONALE, AIMS AND OBJECTIVES In 2012, health care spending in Italy reached €114.5 billion, accounting for 7.2% of the Gross Domestic Product (GDP) and 14.2% of total public spending. Therefore, reducing waste in health facilities could generate substantial cost savings. The objective of this study is to show that Lean Six Sigma represents an appropriate methodology for the development of a clinical pathway which allows to improve quality and to reduce costs in prosthetic hip replacement surgery. METHODS The methodology used for the development of a new clinical pathway was Lean Six Sigma. Problem solving in Lean Six Sigma is the DMAIC (Define, Measure, Analyse, Improve, Control) roadmap, characterized by five operational phases which make possible to reach fixed goals through a rigorous process of defining, measuring, analysing, improving and controlling business problems. RESULTS The following project indicated several variables influencing the inappropriate prolongation of the length of stay for inpatient treatment and corrective actions were performed to improve the effectiveness and efficiency of the process of care. The average length of stay was reduced from 18.9 to 10.6 days (-44%). CONCLUSION This article shows there is no trade-off between quality and costs: Lean Six Sigma improves quality and, at the same time, reduces costs.

Monitoring of foetal heart rate and its variability (FHRV) covers an important role in assessing health of foetus. Many analysis methods have been used to get quantitative measures of FHRV. FHRV has been studied in time and in frequency domain and interesting clinical results have been obtained. Nevertheless, a standardized definition of FHRV and a precise methodology to be used for its evaluation are lacking. We carried out a literature overview about both frequency domain analysis (FDA) and time domain analysis (TDA). Then, by using simulated FHR signals, we defined the methodology for FDA. Further, employing more than 400 real FHR signals, we analysed some of the most common indexes, Short Term Variability for TDA and power content of the spectrum bands and sympathovagal balance for FDA, and evaluated their ranges of values, which in many cases are a novelty. Finally, we verified the relationship between these indexes and two important parameters: week of gestation, indicator of foetal growth, and foetal state, classified as active or at rest. Our results indicate that, according to literature, it is necessary to standardize the procedure for FHRV evaluation and to consider week of gestation and foetal state before FHR analysis.

Fetal heart rate variability is surely one of the most important parameters to monitor fetal wellbeing. Linear studies, widely employed to study fetal heart variability and its correlations with the development of the autonomous nervous system, have shown some limitations in highlight dynamics potentially relevant. During the last decades, therefore, nonlinear analysis methods have gained a growing interest to analyze the chaotic nature of cardiac activity. Techniques investigating nonlinear dynamics have been already successfully employed in adults, to analyze different physiological and pathological states. Concerning fetal monitoring, instead, a smaller number of papers is available in the literature; even if symbolic dynamics was recently employed to quantify fetal heart rate regularity, demonstrating that the use of this technique may lead to a better and more differentiated understanding of normal fetal physiological development. In this work, we applied the symbolic dynamics to analyze fetal heart rate variability in healthy fetuses at the end of a physiological pregnancy. Our results confirmed the potentiality of the technique to highlight differences between signals characterized by more or less variability.

RATIONALE Nowadays, the monitoring and prevention of healthcare-associated infections (HAIs) is a priority for the healthcare sector. AIMS AND OBJECTIVES In this article, we report on the application of the Lean Six Sigma (LSS) methodology to reduce the number of patients affected by sentinel bacterial infections who are at risk of HAI. METHODS The LSS methodology was applied in the general surgery department by using a multidisciplinary team of both physicians and academics. Data on more than 20 000 patients who underwent a wide range of surgical procedures between January 2011 and December 2014 were collected to conduct the study using the departmental information system. The most prevalent sentinel bacteria were determined among the infected patients. The preintervention (January 2011 to December 2012) and postintervention (January 2013 to December 2014) phases were compared to analyze the effects of the methodology implemented. The methodology allowed the identification of variables that influenced the risk of HAIs and the implementation of corrective actions to improve the care process, thereby reducing the percentage of infected patients. RESULTS The improved process resulted in a 20% reduction in the average number of hospitalization days between preintervention and control phases, and a decrease in the mean (SD) number of days of hospitalization amounted to 36 (15.68), with a data distribution around 3 σ. The LSS is a helpful strategy that ensures a significant decrease in the number of HAIs in patients undergoing surgical interventions. The implementation of this intervention in the general surgery departments resulted in a significant reduction in both the number of hospitalization days and the number of patients affected by HAIs. CONCLUSIONS This approach, together with other tools for reducing the risk of infection (surveillance, epidemiological guidelines, and training of healthcare personnel), could be applied to redesign and improve a wide range of healthcare processes.

The goal of this paper is to define the platform specifications dealing with medical information sharing both from research viewpoint and in terms of local health care. The purpose of this research work is based on doctor - patient relationships: VDS - Virtual Medical Doctor System. At this stage the platform is only used for scientific purposes. In particular, we assume the integrated design of the platform based on different levels (layers), which may be interconnected through the information flows (or links). The first level (the lower) involves the construction of a VDS platform. Based on this system it is possible to foresee a number of extensions such as social network for scientific research design, and risk analysis tool.

Parameters derived by techniques investigating nonlinear can be included in computerised systems of cardiotocographic monitoring. In this work, we described an application of symbolic dynamics to analyze foetal heart rate variability in healthy foetuses and a concise index, introduced for its classification in antepartum CTG monitoring. The introduced index demonstrated to be capable to highlight differences in heart rate variability and resulted correlated with the Apgar score at birth, in particular, higher variability indexes values are associated to early greater vitality at birth. These preliminary results confirm that SD can be a helpful tool in CTG monitoring, supporting medical decisions in order to assure the maximum well-being of newborns.

Healthcare is moving towards increased assistance needs with limited resources, both in economics terms, in personnel or space terms, leading to the usage of specific analysis for the acquisition, evaluation and assessment of medical technologies. The systematic evaluation of properties, effects or other impacts of a medical (or health) technology with a broad multidisciplinary approach is named Health Technology Assessment (HTA).This work presents an approach of a HTA protocol for the classification of hospitals or health facilities equipments, realized by combining the classic HTA concepts with hierarchic clustering techniques in a multidisciplinary analysis of requirements, cost, impact of logistics, technology associated risks.

Abstract Rationale, aims, and objectives Lean Six Sigma (LSS) has been recognized as an effective management tool for improving healthcare performance. Here, LSS was adopted to reduce the risk of healthcare‐associated infections (HAIs), a critical quality parameter in the healthcare sector. Methods Lean Six Sigma was applied to the areas of clinical medicine (including general medicine, pulmonology, oncology, nephrology, cardiology, neurology, gastroenterology, rheumatology, and diabetology), and data regarding HAIs were collected for 28,000 patients hospitalized between January 2011 and December 2016. Following the LSS define, measure, analyse, improve, and control cycle, the factors influencing the risk of HAI were identified by using typical LSS tools (statistical analyses, brainstorming sessions, and cause‐effect diagrams). Finally, corrective measures to prevent HAIs were implemented and monitored for 1 year after implementation. Results Lean Six Sigma proved to be a useful tool for identifying variables affecting the risk of HAIs and implementing corrective actions to improve the performance of the care process. A reduction in the number of patients colonized by sentinel bacteria was achieved after the improvement phase. Conclusions The implementation of an LSS approach could significantly decrease the percentage of patients with HAIs.

In total knee arthroplasty (TKA) and total hip replacement (THR) the restoration of the normal joint function represents a fundamental feature. A prosthetic joint must be able to provide motions and to transmit functional loads. As reported in the literature, the stress distribution may be altered in bones after the implantation of a total joint prosthesis. Some scientific works have also correlated uncemented TKA to a progressive decrease of bone density below the tibial component. Antibiotic-loaded bone cements are commonly employed in conjunction with systemic antibiotics to treat infections. Furthermore, nanoparticles with antimicrobial activity have been widely analysed. Accordingly, the current research was focused on a preliminary analysis of the mechanical and antibacterial activity of a PMMA-based bone cement loaded with gold nanoparticles. The obtained results demonstrated that nanocomposite cements with a specific concentration of gold nanoparticles improved the punching performance and antibacterial activity. However, critical aspects were found in the optimization of the nanocomposite bone cement.

Risk management in this chapter is defined as the process of identifying, through the study of all possible sources of errors and problems, the required preventive and corrective actions to reduce risk and whose consequences that compromises the capacity of an organization to reach its own objectives (Del Vecchio and Cosmi, 2003). It has been extensively used in economics, engineering and recently has also been adopted in the fields of public and private health (Gorrod, 2004; Alexander and Sheedy, 2005).
