Effectiveness of Enteral Nutritional Therapy in the Healing Process of Pressure Ulcers: A Systematic Review*

OBJECTIVE To evaluate the effectiveness of enteral nutritional therapy (ENT) in the healing process of pressure ulcers (PU) in adults and the elderly. METHOD A systematic review whose studies were identified through the databases of Cochrane, MEDLINE/PubMed, SciELO, LILACS, EMBASE, CINAHL, Web of Science, and manual searches. It included randomized clinical trials (RCTs) without delimiting the period or language of publication, which addressed adults and elderly patients with pressure ulcers in a comparative treatment of enteral nutritional therapy and placebo or between enteral nutritional therapy with different compositions and dosages. RESULTS We included ten studies that considered different interventions. It resulted in more pressure ulcers healed in the groups that received the intervention. The included studies were heterogeneous with regard to patients, the type of intervention, the sample and the follow-up period, all of which made meta-analysis impossible. CONCLUSION Although the enteral nutritional therapy demonstrates a promotion of pressure ulcer healing, sufficient evidence to confirm the hypothesis was not found.OBJETIVO Avaliar a efetividade da terapia nutricional enteral no processo de cicatrizacao das ulceras por pressao em adultos e idosos. METODO Revisao sistematica cujos estudos foram identificados por meio das bases de dados Cochrane, MEDLINE/PubMed, SciELO, LILACS, EMBASE, CINAHL, Web of Science, e por busca manual. Incluiram-se ensaios clinicos randomizados, sem delimitacao de periodo e idioma da publicacao, que abordaram pacientes adultos e idosos portadores de ulceras por pressao, em tratamento comparativo entre terapia nutricional enteral e placebo ou entre terapia nutricional enteral com diferentes composicoes e dosagens. RESULTADOS Foram incluidos dez estudos, que contemplaram diferentes intervencoes. Apontaram maior numero de ulceras por pressao cicatrizadas nos grupos que receberam a intervencao. Os estudos incluidos foram heterogeneos em relacao aos pacientes, ao tipo de intervencao, a amostra e ao tempo de seguimento, aspectos que inviabilizaram a metanalise. CONCLUSAO Embora a terapia nutricional enteral demonstre favorecer a cicatrizacao de ulcera por pressao, nao foram encontradas evidencias suficientes para confirmar essa hipotese.

Therapeutic communication underlying interpersonal relationship between elementary school adolescent and teacher

Descriptive research, qualitative in nature, carried out with 11 teachers from an elementary school in Curitiba/Brazil. Objective: to bring about effective interpersonal relationship between teaching staff and teenage students. Data collection was performed by means of group discussion strategy. Theoretical landmark was Therapeutic Communication by Maguida Stefanelli. Results were presented under the following theme categories: “Communication in the interpersonal relationship between teacher and teenage student”; “The teacher acknowledges the therapeutic function in communication”; “The specificities in teenagers’ communication”; they pointed out the relevance to health and education professionals in learning therapeutic communication, specially for those who work with teenagers, in order to make pedagogic relationship and actions more effective, less conflicting and trustworthy. Participants became aware of how important it is to deepen their knowledge on the studied theme for a constructive teaching practice among teenagers.

Cuidados com Lesão de Pele: Ações da Enfermagem

Estudo descritivo exploratorio que objetivou identificar atividades de Enfermagem relacionadas as lesoes de pele em um hospital de ensino de Curitiba, no estado do Parana, Brasil. Foram entrevistados 25 enfermeiros responsaveis pelas unidades de internacao, em janeiro de 2013. Os dados foram analisados por meio de estatistica descritiva e agrupados pelas tematicas: prevencao, tratamento, gestao e educacao permanente. Os resultados apontam que a mudanca de decubito do paciente e a medida de prevencao mais utilizada em ulceras por pressao, no tratamento e a realizacao de curativos. A instituicao tem uma gestao de qualidade e de risco, porem a maioria dos enfermeiros desconhece qualquer indicador relacionado ao cuidado com a pele e nao notifica as lesoes. Um total de 88% declarou que nao existe politica educativa sobre o cuidado com a pele. As lesoes sao fenomenos complexos e controla-las exige um conjunto de medidas no cuidado baseado em evidencias cientificas por parte do profissional, politicas de gestao e educacao da instituicao.
