SummaryThe gastric acid output was studied in the 11 patients of hyperparathyroidism before and after parathyroidectomy. The gastric acid output before operation was almost equal to the normal control in our hospital. After the correction of serum calcium by parathyroidectomy, the gastric acid output and serum gastrin were decreased. The decreased gastric acid output was recovered as the days passed since operation and approached to the preoperative level. The acid output in hyperparathyroidism was less in the case whose activity of alkaline phosphatase was more, which suggested that the calcium deposition on gastric mucosa might damage the parietal cell as the result of long lasting hypercalcemia.

SummaryCaerulein was applied to oral cholecystography and the opacity of the common bile duct was evaluated at intervals. To 127 subjects whose gallbladder had been visualized by cholecystography was administered caerulein by intramuscular route in a dose of 0.2 μg per kg of body weight. As a result:1.The visualization of the common bile duct reached its maximum 15 minutes after administration of caerulein (52% of the subjects); in total, 62.2% of the subjects responded to this cholecystography.2.The visualization by caerulein was superior to that by the yolk both in its rate and intensity.3.The cholangiogram taken after administration of caerulein was preferably available for diagnosis of bile duct lesions Caerulein used for a pretreatment of drip infusion cholangiography (DIC) remarkably improved the picture in 3 of the 10 subjects in whom DIC alone had failed to show the bile duct Thus, the application of caerulein to cholangiography facilitated diagnosis of bile duct lesions.

壮年期の健康対策として厚生省は老人保健法いわゆる老健法を制定, 昭和58年より第1次, 昭和63年より第2次5か年計画, そして平成4年から第3次8か年計画をたてた。大腸ガンについては対前年比の伸び率をゼロにすることを目標として, 第3次計画に大腸ガン検診を初めて導入, 受診率を徐々に上げて最終年度の平成11年度の受診率30%を目標とした。今後の検診活動を更に発展させる材料とするために, 岐阜県における第3次8か年計画に基づく大腸ガン検診の成績の推移を全国平均と比較し現状を分析した。岐阜県の大腸ガン死亡率が, 男性は全国平均並であるが女性は常に全国平均を上回り, 47都道府県中ワースト1であることも本研究を志すキッカケとなった。平成11年度の受診者総数は, 平成4年度に比し約2倍に増加したがそれでも受診率は14.8%と目標の半分, 全国平均の15.3%を下回った。岐阜県全体でのガン発見率, 早期ガン率は全国平均と大差なく, 精度管理上大きな問題はないが, 岐阜県内の検便を委託された検査機関別の精度には若干のばらつきが見られた。大腸ガンによる死亡率を低下させるためには, 精検受診率を向上させねばならないが, 最も重要なことは検診受診率を上げることで, そのためには厚生労働省が有効と認めた検診に限り健康保険適応とすべきであることを主張したい。

JA岐阜厚生連看護専門学校の全校生徒101名を対象に, 無記名アンケート形式の喫煙状況調査を行った。平成13年 (2001) 7月時点の同校学生の喫煙率は17.8%(18名) である。喫煙学生のほとんどはニコチン量0.9mg/本以下のタバコ15本以内/日の喫煙量であり, Fagerstrom Tolerance Questionnaire (FTQ) 指数は1.94±1.51とニコチン依存度はそれ程高くなく, 喫煙学生の約8割が禁煙を希望・意識している実態が把握された。又, 非喫煙学生の約8割が周りの喫煙に不快感を抱く一方, 約2割の学生は機会により, 容易に喫煙を開始し得る可能性が示された。同校学生に対し, 今後十分な啓蒙が必要であり, カリキュラムの一貫としての禁煙教育の必要性が示唆された。

1994年~1997年の4年間で, 東濃地域におけるCPA (cardioplumonary arrest: 心肺停止) に対するbystander CPR (cardioplumonary resuscitation: 心肺蘇生) を調査したが集計されていない市が認められた。CPR率は11.2%であった。一方, CPR教育 (応急手当講習) の受講者は人口の11.3%であった。さらに県防災ヘリコプターの訓練及び利用状況の調査においても, 訓練さえなされていない市が見うけられた。当地域での救急医療に対する意識の低さを示すものであり, 市民, 救急隊員の意識改革にはCPR教育, 啓蒙活動に, 医療機関のさらなる協力が必要と思われた。

SummaryTo study the intestinal phase of ulcer inhibition, olive oil or hydrochloric acid was infused into small intestine in white male rats weighing 200–250 gr. The animals were fasted before study. Tetragastrin was administered continuously through the tail vein using the infusion pump. The ulcer formation by gastrin at the rate of 60% was decreased by the oil or hydrochloric acid infused into small intestine as well as by secretin administered intravenously (8 U/kg-h). The acid output and the rate of secretory volume were also inhibited by oil, HCl and secretin. Peptic output by gastrin was decreased by infusing the hydrochloric acid into small intestine, however olive oil in small intestine neither decreased nor augmented the peptic putout. Through these results the important role of small intestine in the inhibitory system of ulcerogenesis became obvious. To make clear the hyperacidity as one of the ethiological factors of peptic ulcer, the intestinal phase of gastric acid secretion should be investigated especially by the study of gastrointestinal hormone.
