Three-dimensional removable partial denture in the treatment of mandibular free-end edentulous cases

Rigid supporting of the denture should be required with the stability of the removable partial den ture under the protection of abutment teeth and residual ridges, especially in distal extension saddle removable partial denture. Three-dimensional removable partial dentures were applied to mandibu lar free-end edentulous cases. As a result, well-balanced functions are effectively performed. And the stabilization of the removable partial denture is acquired.

DNA Probe Analyses for the Detection of Periodontal Pathogens : Prognostic Markers for Clinical Response Following Initial Treatment.

DNAプローブを用いて, 初期治療前後の歯肉縁下細菌 (A. actinomycetemcomitans, P. gingivalis, P. intermedia, F. nucleatum, T. denticola, C. rectus) を検索し, 初期治療後の歯肉縁下細菌が術後の臨床変化を予知しうるかどうかを検討した。成人型歯周炎と診断した患者6名から25部位を選び被験部位とした。平均PPDは6.12mmであった。歯肉縁下細菌叢は, 実験開始時とスケーリング・ルートプレーニング後4週と8週に採取した。術後14週の臨床診査の結果から, 被験部位を治療反応良好群と治療反応不良群に群分けし, 統計分析を行った。その結果, 治療反応良好群では, すべての診査時にCALの有意な増加が認められたが, 治療反応不良群では認められなかった。また, 両群間の全細菌の陽性率の比較では, 8週のT. denticolaにのみ有意差が認められた (P=0.02) 。これらの結果から, lowレベル (≦6×103cells<6×104) 以上を陽性部位として, 術後8週にT. denticolaの有無を検索すれば, 術後14週の臨床変化を予知しうる検査項目として有効であることが明らかになり, 予後診断の指標としての有用性が示唆された。

咬頭嵌合位での早期接触が歯槽骨吸収の原因の一つであると考えられる16症例 (27歳~62歳の男女16名) について, 歯槽骨吸収形態と咬合接触関係との関連性について検索することを目的に, 画像解析装置を用いて歯槽骨吸収形態と咬頭嵌合位での早期接触部位, 咬合接触部位および咬合接触面積を観察した。その結果, (1) 早期接触部位がI型吸収では歯槽骨吸収側に, II型吸収とIII型吸収では頬側咬頭外斜面と頬側咬頭内科面に多く認められた。 (2) Occlusal Contact Area (OCA) 領域とLuminosity Grade 1 (LG 1) 領域が1型吸収では歯槽骨吸収側に, II型吸収では頬側咬頭外斜面, 内斜面および遠心辺縁隆線に, III型吸収では頬側咬頭外斜面, 頬側咬頭内斜面および舌側咬頭内斜面に分布する傾向頬側咬頭内斜面, 舌側咬頭が認められた。 (3) O CA領域とLG 1領域の咬頭外斜面と内斜面部への分布は, I型吸収とII型吸収では内科面に比較して外斜面に広い分布に認められた。III型吸収では内斜面部に広い分布が認められた。

[Maintenance and the clinical evaluation of periodontal patients in Konus-Telescope denture].

In order to assess by periodontal evaluation the changes that might occur with time in the abutment teeth and periodontal tissues when Konus-Telescope dentures are used as that final treatment of periodontal disease, the dentures (15 units) were placed in 13 patients with missing tooth and periodontal disease and findings at the time of denture placement and 30 months after the placement were compared. The number of cases that exhibited significant changes in hygiene level, tissue inflammation and periodontal pocket depth of the abutment teeth after 30 months was very small, while as many as 85.2% of the abutment teeth showed decrease in tooth mobility. Increase in tooth mobility was not detected in any of the cases. In addition, X-ray examination revealed tendencies toward improvement of the periodontal ligament and remission of alveolar bone resorption in many of the cases. These results suggest that Konus-Telescope denture is highly offers protection of the residual periodontal tissues through its secondary splint action.

外傷性咬合と歯槽骨吸収形態との関連性について検討したいために, 画像解析装置を用い, 外傷性咬合の検討は, 外傷性咬合時の咬合力の大きさと咬合力の加わる方向を指標にした。また被験歯は, 早期接独があってわれわれが規定した一定の条件を満たした下顎の臼歯21歯を対象にした。観察の結果は,1. SHonic-GAシステムを用い規格デンタルX線写真を画像処理し, まず臼歯の歯槽骨吸収形態を小臼歯は4種に大臼歯は5種に分類した。2. 小臼歯では, 歯槽骨の吸収域に一致して, 早期接触, OCAおよびLG1° が認められた。3. 大臼歯のI型とII型では, 歯槽骨の吸収域に一致して, OCAとLC1° が認められることが多く, 歯槽骨吸収形態との関連性が示唆されたが, III型とIV型では認められなかった。4. 早期接触部と, 各歯の最大OCAおよび最大LG1° の部位が一致して認められた。
