Quality of different gutta-percha techniques when filling experimental internal resorptive cavities: a micro-computed tomography study.

The purpose of this ex vivo study was to determine the quality of root fillings in experimental internal resorptive cavities filled using cold lateral condensation (CLC), vertical condensation and Thermafil techniques. Forty-five extracted maxillary canine teeth were selected. After root canal instrumentation, the roots were sectioned horizontally and experimental internal resorption cavities were prepared. The root sections were re-approximated with a luting agent and randomly assigned to three groups (nu2009=u200915) to be filled using CLC, vertical condensation and Thermafil, respectively. Assessment of obturation quality in the internal resorptive cavity was performed using a desktop X-ray micro focus computed tomography scanner, and the percentage of gutta-percha (GP), sealer and voiding was calculated for each specimen. Data were statistically analysed using non-parametric tests, with Pu2009<u20090.05 denoting a statistically significant difference. Vertical condensation filled 96.25u2009±u20098.31% of the total artificial cavity, and was superior to CLC (63.20u2009±u200916.87%) and Thermafil (59.26u2009±u200918.47%). The mean percentage of voids was 3.75u2009±u20098.31% in the vertical condensation group, 37.09u2009±u200917.13% in the CLC group and 41.06u2009±u200918.60% in the Thermafil group. We conclude that obturation of experimental resorption cavities is significantly better when using vertical condensation than when using other GP-based techniques.

Nitric Oxide Levels in Serum of Patients with Symptomatic Irreversible Pulpitis

Nitric oxide (NO) affects both pain and inflammation in human tissues. Pharmacotherapy that decreases NO concentrations may have utility in treating inflammatory painful conditions. To determine the types of disorders in which such an approach should be studied, changes in NO serum levels before and after the painful inflammatory condition resolves would be helpful. This study compared the pre-treatment and post-treatment serum nitric oxide (NO) concentrations in irreversible pulpitis (inflammatory toothache). Thirty-two patients (16 males, 16 females) with irreversible pulpitis were included in this study. Before treatment, patients had severe symptoms of inflammation, but at the end of treatment no symptoms of inflammation were observed. NO concentrations were measured in serum of patients with irreversible pulpitis, before and after treatment. Differences in serum NO concentrations were not statistically significantly different before and after treatment.

KALSİYUM HİDROKSİT VE KLORHEKSİDİN JEL/KALSİYUM HİDROKSİT KANAL İÇİ MEDİKAMANLARININ FARKLI İRRİGASYON AKTİVASYON YÖNTEMLERİ İLE KÖK KANALINDAN UZAKLAŞTIRMA ETKİNLİĞİNİN DEĞERLENDİRİLMESİ

Amac : Bu calismanin amaci; farkli irrigasyon solus- yonlarinin, aktivasyon yontemleri ile aktive edildikten sonra, kok kanal tedavisi sirasinda kullanilan kanal ici medikamanlarini, kok kanallarindan uzaklastirma etkinliklerinin degerlendirilmesidir. Gerec ve Yontem: Dislere uygulanan kanal ici medi- kamanlardan kalsiyum hidroksit ve klorheksidin jel/kal- siyum hidroksit, 3 farkli irrigasyon solusyonu (NaOCl, Qmix ve Maleik asit), 4 farkli aktivasyon teknigi (igne/ siringa irrigasyonu, Endoaktivator, Endovac ve pasif ultrasonik irrigasyon) ile aktive edilerek kok kanalla- rindan uzaklastirilmaya calisildi. Kok kanallarinda kalan medikaman miktari steromikroskop ile incelendi. Elde edilen veriler Kruskal Wallis ve Bonferoni duzeltmeli Mann-Whitney U testleri ile istatistiksel olarak analiz edildi (p = 0.05). Bulgular: Irrigasyon solusyonlarindan maleik asit ve irrigasyon aktivasyon yontemlerinden pasif ultrasonik irrigasyon aktivasyon yontemi, hem kalsiyum hidroksiti hem de klorheksidin jel/kalsiyum hidroksit kombinas- yonunu uzaklastirmada diger yontemlere gore istatik- sel olarak anlamli derecede daha etkili bulundu (p 0.05). Conclusions: Irrigation activation methods are required for efficient removal of intracanal medicaments from root canal system. Maleic acid irrigation and passive ultrasonic irrigation activation was found as the most efficient method, however none of the tested methods was able to render canal systems free of medicaments. Oral lichenoid contact lesion to amalgam restoration: a case report

Amalgam has been used for nearly 150 years in dentistry and it is still being used because of advantages such as low cost, ease of handling, physical characteristics like diversity in applications and low frequency of local and systemic biological side effects. On the other hand the disadvantages of dental amalgam comprise poor aesthetics, local degradation, occasional allergic responses to some of its components or degradation products, and the toxicity of mercury. Amalgam and/or its components may cause a type IV hypersensitivity reactions on the oral mucosa. Most patients with oral lichen planus (OLP) have no evidence of any association with dental restorative materials. However, contact or proximity to restorations involving amalgam or other materials causes some lichenoid reactions that is to say, lesions that clinically and histologically resemble lichen planus (LP), but have an identifiable etiology. These reactions are presumably due to allergic or toxic reactions to compounds released or generated. The present case report presents a patient with a unilateral oral lichenoid reactions (OLRs) to an amalgam restoration in the right mandibular molar region.

Evaluation of the combination of amoxicillin trihydrate, diclofenac potassium and ornidazol for the management of endodontic pain

Abstract Aims: The purpose of this study is to evaluate the efficacy of drug combination on the flare up that may occur after pulpectomy in patients treated for painful irreversible pulpitis. Material and methods: Thirty emergency patients were included in the study. All the patients who participated in the study suffered from irreversible pulpitis with spontaneous moderate-to-severe pain. The patients were randomly divided into two groups of 15 patients, all of whom were subjected to endodontic treatment. They were randomly classified as Group A, to whom medicine combination was administered, and Group B to whom medicines were not administered. The ingredients of the combination were 50 mg of diclofenac potassium, 500 mg of amoxicillin trihydrate and 250 mg ornidazol. Pain in the groups was statistically compared according to Mann–Whitney and Wilcoxon Signed Ranks test before and after the endodontic treatment. Lipid peroxidation and antioxidant enzymes in serum of patients with painful acute periradicular abscess

Abstract Aims: The purpose of this study was to compare the pre-treatment and post-treatment serum antioxidants levels in acute periradicular abscess (APA), with pain. Method: Thirty-two patients (18 males, 14 females) with acute and painful endodontic abscesses were included in this study. Before treatment, patients had severe symptoms of inflammation, but at the end of treatment no symptoms of inflammation were observed. Glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD) activities, and malondialdehyde (MDA) concentrations, were measured in serum of patients with APA, before and after treatment. Results: There was no statistically significant effect on levels of SOD activity and MDA concentrations. The levels of GSH-Px activity increased significantly (p < 0.05) after endodontic treatment.
