OBJECTIVE To review the indications for total thyroidectomy as the preferred option in certain benign diseases of the thyroid in order to prevent recurrence or future reoperations. METHODS A retrospective study of patients with benign thyroid diseases, carried out from January 1997 to December 2009, analyzing the data relating to age, surgical treatment and postoperative evolution. RESULTS The mean age was 51.8 years (21/77), with the highest incidence in the fifth and sixth decades of life, with 34 (51.5%) patients. The most common preoperative diagnosis was nontoxic multinodular goiter, of which seven were also intrathoracic, followed by autoimmune thyroiditis; recurrent goiter occurred in 11 cases. Multinodular goiter was found in 37 (56.1%) patients, autoimmune thyroiditis in 22 (33.3%), follicular adenoma isolated in five (7.6%), Hurthle cell adenoma in two (3.0%). Sixteen patients (24.2%) had more than one histopathological diagnosis. Permanent injury of the recurrent nerve was observed in one patient (1.5%). There was no case of permanent hypoparathyroidism. There was no operative mortality. CONCLUSION Total thyroidectomy is an operation that can be safely performed, with low incidence of permanent complications, which allows one to broaden its indications in various benign thyroid diseases, thus avoiding future recurrences and reoperations.

BACKGROND: The exact incidence of endometriosis in the general population is unknown. Confirmation of this disease is only possible by histopathological analysis of a fragment obtained by some invasive procedure, because there is so far, no clinical secure marker. AIM: To report the experience with the clinical manifestations and surgical treatment in patients with abdominal wall endometrioma. METHODS: Retrospective analysis of patients operated for abdominal wall endometrioma with emphasis on data relating to age, symptoms, previous cesarean, relation of symptoms with the menstrual cycle, physical examinations and additional surgical treatment, postoperative course and histopathological results of specimens. RESULTS: Forteen patients were operated during the study period, aged between 28 and 40 years. The presence of local mass and pain that worsened during menstruation were the main complaints. Ultrasound and CT examinations were important to precisely localize the disease. Surgical excision was a wide excision of the tumor and affected tissues. The patient progressed satisfactorily and histopathology confirmed the diagnosis of abdominal wall endometrioma in all cases. CONCLUSION: Exists a clear relationship between cesarean operation and abdominal wall endometrioma; ultrasound studies and CT scans help to plan the surgical resection allowing resection of tumor and all affected adjacent tissues.

The authors present a case report of a pancreatic pseudocyst with an unusual spleen involvement. The aspects of this rare complication are discussed, as well as the probable etiologic factors. The outcome was satisfactory and the surgical treatment consisted of the resection of its thick capsule, since the local anatomic conditions would not permit a splenectomy with distal pancreatectomy, considered to be the ideal surgery.

Fecal incontinence is a disabling condition with relevant social costs. Many therapies are described. Objective: To evaluate the response to biofeedback and epidemiological profile, describing the used technique. Methods: A retrospective study in 3 years (June 2005 - June 2008). Results: Thirty patients, 26 women and 4 men, with an average age of 66. The number of normal pregnancies and births varied from none to six and hysterectomy was present in nine. Hypotonia in manometry was present in all patients. Eighteen patients were satisfied, ten were partially met, none was completely dissatisfied, and two have abandoned the therapy. Conclusion: The clinical therapy to biofeedback can be effective for incontinence, but the comprehension by patient appears to be the most important. Diabetes mellitus, anorectal surgery and hysterectomy were related to complaints of incontinence.

The authors present two cases of Primary Hiperaldosteronism, caused by functioning adenoma of the adrenals cortex. The two females patients presented classic symptoms of the Conns disease, especially hypocalemia and hypertension. Both were operated and unilateral adrenalectomy was done, with excelent outcome in one and satisfactory in the other one. It is discussed many aspects related to the incidence, the problems with the diagnosis, indication and surgery treatment.

Os autores relatam o caso de um paciente de 34 anos, submetido a laparotomia de urgencia com quadro de abdomen agudo ocasionado por apendicite epiploica. Foi dado enfase a manifestacao intensa dos sinais e sintomas, assim como sua apresentacao pouco usual no ato cirurgico, uma vez que comprometia o sigmoide sugerindo tratar-se de doenca de Crohn.

The cancer treatement has been subjected to substantial changes, mainly concerning clinical specialities. The advances on tumours study, especially related to genetics and molecular biology, greatly increased our understanding about many aspects of carcinogenesis, neoplasic growth and metastasizing process, untill now obscure. In this context, surgery seems to be attained its limits in trying to erradicate completely the disease, and although the great resections made, this aim has not been succeeded in many cases. New treatments are emerging each year and between the most promising we can highlight tumour angiogenesis chemical blocking agents, with highly promising experimental results. At the same time of the beginning of clinical researchs about these drugs, the authors present a review work, with the objective of presenting a general survey of the knowledge achieved about these recently discovered drugs in tumours control.
