Este texto aborda o estudo da fibromialgia, sindrome cujas dores cronicas sem causalidade orgânica constatavel sao fonte de sofrimento para pacientes e desafio para os clinicos. Situada na fronteira entre a reumatologia e a patologia psicossomatica, com seu cortejo de transtornos, conduz a uma degradacao da qualidade de vida no plano profissional, social e/ou familiar. Dados epidemiologicos apontam uma maior incidencia dessa entidade clinica controversa em mulheres jovens. A falta de evidencias na materialidade do corpo e a presenca de fatores psicopatologicos problematizam seu diagnostico e tratamento. Face a diversidade e dos fatores envolvidos nessa sindrome, a indicacao de uma abordagem multidisciplinar para seu tratamento vem se firmando como consenso. Por outro lado, o carater subjetivo da experiencia de dor contribui para a reticencia das autoridades medicas e governamentais em afirmar plenamente o estatuto de doenca da sindrome. Nesse contexto, ao mesmo tempo em que buscam uma cura para suas dores, os pacientes clamam pela legitimacao do reconhecimento da fibromialgia como doenca. Nossa proposta e considerar a eventual funcao da fibromialgia na estruturacao psiquica como solucao subjetiva. Para tanto, o referencial teorico-clinico da psicanalise de orientacao lacaniana fornece elementos para reflexoes sobre a dor no corpo e seu lugar na economia psiquica. Dados da clinica sugerem algumas especificidades dos usos da dor em funcao da estruturacao psiquica de cada sujeito. A partir do estado atual das pesquisas sobre o tema e considerando a escassez de estudos no campo da psicanalise, interessa-nos contribuir para uma abordagem da fibromialgia que sustente o particular da enunciacao no relato da experiencia de dor. Sem pretender reduzir a fibromialgia a qualquer quadro psicopatologico, como a histeria ou a depressao, interessa-nos sublinhar a relevância da posicao subjetiva daquele que sofre em seu corpo para o diagnostico e tratamento da fibromialgia.

This paper presents a reflection on the etiology and treatment of chronic pain from a psychoanalytic perspective. We discuss the reduction of chronic pain to a contemporary manifestation of hysteria. Considering a symptom as a specific form of jouissance and a way of inscribing the individual in a social bond, the text emphasizes the function of pain based on the specific aspects of each case.

It is argued that for those who are not the successful beneficiaries of the system, schooling creates the conditions for lifelong suffering, in both psychological and social terms, without serious recourse to a vital and effective public sphere of debate, decision and action where their voices may be heard and taken into account. The authors develop this argument in terms of three foci. (1) the example of the reduction of professional work to assessable performances in the continuing professional development (CPD) of medical professionals; (2) the experience of self-harm at school; and 3) the role of the private sector in schooling. Their final argument is that the solution is not to reform the lifelong schooling of people but to emancipate through education. They argue that bringing about young peoples engagement with adults in the work of education as a democratic activity requires the kind of openness and tolerance, and also the suspension of authority of those in charge, that is antithetical to the forms of organisation found throughout the private, public and military sectors. Their conclusion, therefore, is not optimistic.
