Risk Factors in Head and Neck Cancer

John P. Decker; Jerome C. Goldstein

THE National Cancer Institute estimates that 51,000 new cases of head and neck cancer will occur in 1982 and will result in 16,000 deaths. These cases will include 26,600 cancers of the buccal cavi...

Pseudomonas aeruginosa in chronic maxillary sinusitis

Peter J. Koltai; Barry O. Maisel; Jerome C. Goldstein

Psendomonas aeruginosa (Ps. Au.) infection of the maxillary sinus has been reported as an incidental finding on routine antrostomy; however, it has also been noted in several studies as the significant organism in the etiology of chronic sinusitis. Four case reports of culture verified Ps. Au. maxillary sinusitis are presented. The therapeutic modality used in two of the cases was a Caldwell‐Luc operation and in two, an intranasal antrostomy. In all cases, multiple irrigations through the surgically created nasoantral windows were done postoperatively, as was the instillation of gentamicin ophthalmic drops intranasally. In all four cases the infection cleared with this combined surgical and medical therapy.

Psychoacoustic and electrophysiologic effects of partial eighth nerve damage

Anthony T. Cacace; Jerome C. Goldstein; Steven M. Parnes; Saty Satya-Murti; Jonathan R. Wolpaw

The authors present psychoacoustic and electrophysiologic data concerning a patient with partial damage to the auditory nerve, presumably from a dilated arterial vessel. The lesion was described and documented during neurosurgery for vestibular nerve decompression. Pure tone thresholds, psychoacoustical tuning curves, speech reception ability for spondaic words, maximum word recognition performance for monosyllabic NU-6 word lists, and performance on the synthetic sentence identification test in the ipsilateral and contralateral competing message modalities were normal. Findings consistent with eighth nerve dysfunction, absent contralateral acoustic reflexes, absent or unrecognizable early evoked potentials occurring in the first 10 msec, brainstem auditory evoked potentials (BAEPs), and slight rollover of the performance intensity function for monosyllabic words were obtained on the involved side. In spite of the grossly abnormal BAEPs, two late, presumably cortical, auditory evoked potential components, which occur in the 60 to 250-msec range--vertex potential and T complex--were present and normal. The findings indicate that the perceptual processes needed for speech reception and recognition and for the appearance of later, presumably cortical-evoked potentials can be preserved despite partial damage to the auditory nerve.

Problems in Head and Neck Surgery

Jerome C. Goldstein

This book is intended for young trainees in the field of head and neck surgery who have some knowledge of the subject, and who are in the early stages of acquiring experience and expertise in operative surgery. It is about the prevention and rectification of problems and complications. Surgical procedures for the correction of established complications are described. Included are chapters on the thyroids, benign cervical swellings, the parotid, radical neck dissection, the nose and paranasal sinuses, the ear, the oral cavity and oropharynx, the larynx, the trachea, the hypopharynx, and the esophagus. Each chapter contains a list of helpful references. The author is consultant ear, nose, and throat surgeon to Guys Hospital and the Royal National Throat, Nose, and Ear Hospital, London, and has been trained on both sides of the Atlantic, having served as fellow and assistant professor at the State University of Iowa. Shaheen defines a surgical

The origins of the cochlear efferent bundle of Rasmussen in primates

Norman L. Strominger; Steven M. Silver; Timothy C. Truscott; Jerome C. Goldstein

The spatial distribution of neurons emitting fibers of the olivocochlear bundle was determined by injecting horseradish peroxidase into the round window in a series of new and old world monkeys. Animals were perfused 24–28 h later. Sections through the pons and medulla were reacted with tetramethyl benzidine and counterstained with thionin and/or neutral red. Perikarya containing reaction product numbered over 800 in one case. The greatest concentration of labeled cells was located in the dorsal periolivary area intermediate between the dorsal margins of the lateral and medial superior olivary nuclei. Approximately 400 cells were ipsilateral to the injection, opposed to 100 contralaterally. The dorsolateral periolivary area contained over 100 labeled cells ipsilaterally, and only about one‐third as many contralaterally. While 95 labeled cells occupied the region of the contralateral medial preolivary nucleus, about two‐thirds as many labeled cells were found in the comparable ipsilateral nucleus. Although...
