Clinical risk management improves quality in health care by increasing patient safety and by trying to eliminate avoidable errors. Implementing risk management programs requires the creation of risk management units in hospitals that will be responsible of all the activities related to patient safety. This article describes the development one of the first clinical risk management units in Europe directed by medical and nursing staff who continue to work at the same time in their departments.

Abstract Reaction of optically active aldehyde complexes (+)-( RS )-[(η 5 -C 5 H 5 )Re(NO)(PPh 3 )(η 2 -OCHR)] + BF 4 − and (CH 3 CH 2 ) 4 N + CN − gives cyanohydrin alkoxide complexes (+)-(η 5 -C 5 H 5 )Re(NO)-(PPh 3 )(OCH(CN)R) (78–95%) of 89-53% de. The alkoxide ligands are then converted to optically active MTPA esters (96-85%) of 90-56% de.

The title compounds (R = Me, Et, Ph) are synthesized from ketones in high yield, structurally characterized, and shown to undergo highly stereoselective nucleophilic attack; secondary alcohols can be made with 95 to 99% enantiomeric excess (e.e.)
