An important role of silent aspiration of gastric contents as a cause of pulmonary complications following surgery for esophageal cancer

The progress of postoperative chest X-ray films was analyzed in patients who had undergone radical surgery for esophageal cancer at the second Department of Surgery, Osaka University Medical School from 1969 to 1981. We subsequently reached a conclusion that abnormal shadows were caused by the intratracheal aspiration of oropharyngeal secretions or gastric contents. In particular, the aspiration of gastric contents was considered to be a factor in the occurrence and aggravation of pneumonia. In order to prevent aspiration of gastric contents, a drainage tube was inserted into the stomach as a reconstructive organ and endotracheal intubation was maintained for more than 24 hours post-operatively in order to suction the tracheobronchial secretion. Consequently, the frequency of occurrence and the aggravation of pneumonia decreased and the operative mortality rate improved significantly.

Pulmonary Complications Following Surgery for Esophageal Cancer

Pulmonary complications after surgery for esophageal cancer are the most frequent and have a great influence on the surgical results. We reported at the 2nd World Meeting of the International Society for Diseases of the Esophagus (ISDE) that pneumonia was the most frequent of the postoperative pulmonary complications and that aspiration of gastric content is the most important factor causing pneumonia after radical surgery for esophageal cancer [1]. Therefore, in order to prevent aspiration (silent or massive) of gastric content or saliva, gastric tube drainage, endotracheal intubation, and mechanical ventilation were performed on postoperative patients [2]. In this report, postoperative pulmonary complications were analyzed referring to chest X-ray and the clinical course, and the states of these patients were compared before and after the application of these preventive methods.

乳房自己検診法 (BSE) と, 危険因子や乳癌を含めた乳腺疾患についての教育は, 乳癌検診において医師の診察と同様に大切なことである。十分なレベルのBSEや教育を受診者に修得させるには, 検診する側の検診に対する関心や知識の向上も必要であるが, この点に関する検討は今まであまりされていない。そこで今回, 南大阪乳癌検診・診断研究会で保健センターおよび一般医療機関勤務者とその他の一般女性を対象とし, BSE認知, BSE施行, 検診受診歴, 乳腺疾患に関する知識などについてアンケート調査を行い, 検討した。平成10年6月30日での回収数202人中, 保健婦および看護婦114人であった。この中で直接検診に携わっているもの (直接群) は34人, 直接には携わっていないもの (間接群) 26人であった。検診の受診者に対してBSEの指導・教育を行わなければならない保健センター勤務の直接群でさえ, BSE認知が100%でなく, またBSEを実施しているものは61%にすぎなかった。さらにBSEを毎月実施している者は42%しかなかった。また検診直接関与者でも乳腺疾患の症状についての知識は69%, 検査法についての知識62%, 手術を含めた治療については40%しか持っていなかった。以上の結果より, 十分なBSEや教育を受診者に修得させるには, まず検診する側のBSEや危険因子, さらに乳腺疾患に対する関心や知識をレベルアップする必要があると考えられた。

Solitary Tuberculoma of the Liver Concomitantly Found in a Pancreatic Cancer Patient.

孤立性肝結核腫は比較的まれな疾患であり, 肉芽腫を主体とするものは, 自験例を加えても本邦報告例は25例にすぎない. 著者らが最近経験した1切除例を本邦報告例の検討とあわせて報告する. 症例は77歳の男性で, S状結腸癌術後の経過観察中にCA19-9値の上昇が見られた. 腹部CT, エコー, ERCPおよび血管造影所見より, 膵体部癌とS6肝転移が疑われた. 膵体尾部切除およびS6肝部分切除を施行したが, 肝腫瘤部は, 病理組織学的には乾酪壊死巣と間質へのリンパ球浸潤および類上皮細胞が認められ, 肝結核腫と診断された. 孤立性肝結核腫はまれであるものの, 肝腫瘍の鑑別診断に際し留意すべき疾患であると考えられる.

RADIOLOGICAL STUDY ON PULMONARY COMPLICATIONS AFTER THE OPERATION OF ESOPHAGEAL CANCER

食道癌手術症例の胸部x線像の経過を検討したところ, 初発異常陰影としては細い気管支の閉塞による無気肺と, それに炎症を伴った浸潤影が大部分を占めた. この陰影の多くは術直後, 術後1日目に左下肺からはじまっており, 原因として気道内分泌物貯留だけでなく, 口腔内分泌物や消化液の流入, 誤嚥が関与しているものと考えられた. また, 術式により初期陰影の発生頻度と増悪率に差があることから, とくに消化液の誤嚥は浸潤影の発生因子となり, 増悪因子となるものと考えられた. この防止策として, 挙上胃管内に減圧チューブの挿入および術後気管内挿管による管理などが有効と考えられる.
