SummaryA case of alpha-feto protein (AFP) positive hepatic cell carcinoma has been presented in which the appearance of a mono-clonal gammopathy can be explained by persistent infection with HB virus (HBV). The patient, a fifty year old male, developed acute jaundice with hepatitis in July, 1952, and died of an hepatic cell carcinoma in December, 1973. The interest of this case lies in the fact that during the seven months prior to be patient’s demise, a mono-clonal gammopathy of IgG (L type), together with positive titeres for HBs Ag (adr) and AFP, was demonstrated.

肝細胞癌(肝癌)138例に対する抗癌剤加Lipiodol (LP)-TAE(LP-TAE)の成績を中心に,計212例での各種治療法との生存率(SR)を検討し,また肝癌結節内LP停滞状態の解析も行い,LP-TAE療法を臨床評価した.その結果,LP-TAEのSRは,肝切除にやや劣るものの,非切除ないし切除不能例では,最も良い予後を示したが,2年以降のSRが不良であった.一方,LP動注後CT(LP-CT)は結節検出能が高く,進展度診断と,結節内LP停滞状態による効果判定に有用で,LP停滞完全例の予後は極めて良いが,不完全例では,LP-TAE効果が少なく予後不良で,早期に他の治療法を選択するべきと考えられた.以上から,LP-TAE療法は肝癌集学的治療の中核となりえるが,長期予後改善が今後の課題と考えられた.

Localization of HBsAg and HBcAg in the liver tissue of patients with hepatocellular carcinoma

HBウイルス(HBV)のHBs抗原およびHBc抗原の肝細胞癌組織内局在を,蛍光抗体法およびperoxidase抗体法を用いて検索した.対象材料は血中HBs抗原陽性6例,HBs抗体陽性2例および両者ともに陰性2例の合計10例の手術もしくは剖検によって得た肝組織である. その結果,血中HBs抗原陽性6例では,非癌部肝細胞の細胞質内にHBs抗原を,同じく核内にHBc抗原を全例に検出したが,肝癌細胞内にはHBs抗原,HBc抗原ともに検出することができなかった.一方,その他の4例では,非癌部および癌部肝細胞に,全例,HBs抗原ないしHBc抗原を認めなかった. 従って,従来,疫学調査などで報告されているHBVと肝細胞癌との陽性の関連を,ウイルス抗原の面から細胞レベルでは証明できず,今後さらにこの分野からの研究が望まれた.
