[Studies on serum lipids and blood coagulation-fibrinolytic system in aged patients with old cerebral thrombosis].

動脈硬化の発症, 進展や血栓症の要因として考えられている血清脂質代謝異常と血液凝固線溶異常の, 相互関係は多くの研究者により検討されている. しかし, 老年慢性期脳梗塞例, いわゆる寝たきり状態例についての報告は少ない. そこで著者らは, これら寝たきり老人 (平均76.4歳, 発作より平均1.4年経過したADL著しく不良例) 38例を対象として, 血清脂質 (TC, β1p, TRG, HDL C, Apo AI, Apo AII) および血液凝固線溶関連因子(Fbg, α1AT, α2M, ATIII, p1g, FDP, α2PI, FN, Factor XIII) を測定し次の結果を得た.1) 血清Apo AI, AII は患者群で有意に低値を示した.2) 血液Fbgは患者群で有意に高値, α1AT, α2M, P1g, ATIII, FDPは有意差なく, α2PI, FN, Factor XIIIは患者群で有意に低値を示した.3) 血清Apo AIとα2PIは患者群で有意の正相関を示した.4) 血清Apo AIとα2PIは, 対照群で有意の負の相関を示した.5) β1pとFNは患者群で有意の正相関を示した.6) FNとα2PIは患者群で有意の正相関を示した.以上の成績より, 老年慢性期脳梗塞例では動脈硬化の negative risk factor であるといわれているHDL Cの質的変化が生じており, また凝固線溶面の検討より, これらの症例では, 血栓形成促進傾向にあることが考えられ, 長期臥床状態では, 再発作への危険性が大である状態にあると考えられた.
