Postoperative Reflux Esophagitis with the Use of 24-Hour Esophageal pH Monitoring

In a follow-up on 63 patients who had had a subtotal gastrectomy for gastric cancer, the incidence of reflux symptoms was studied. The Billroth I group had a high incidence of 36.2%, though reflux esophagitis is most commonly seen after distal gastrectomy with Polya’s operation [1]. The purpose of the study was to clarify the pathogenesis of reflux esophagitis after subtotal gastrectomy with Billroth I anastomosis and examine the usefulness of the reconstruction with jejunal interposition between the gastric remnant and duodenum as a method of prevention.

Changes of several immunological parameters in before and after gastrectomy and immunotherapy in patients with gastric cancer.

胃癌切除症例55例に対し, リンパ球サブセットを含む各種免疫パラメーターを測定し, 進行度別, 術前・中・後の推移, PSK投与の有無により比較検討した. 進行度別ではPPD, SuPS, IAP, リンパ球数は進行度を反映したが, リンパ球サブセットには一定の傾向が認められなかった. 術前・中・後の推移では, 術中にOKT4, 4/8比の低下が認められ, 手術侵襲により, ごく早期からリンパ球サブセットの変動が認められた. PSK投与群では, 非投与群に比べ術後3ヵ月ころよりOKT4, 4/8比が上昇し, 術後の免疫能の改善にPSKが促進的に作用しているものと思われた. また, T細胞サブセットの解析は免疫療法の効果判定の1つの指標として有用であると思われた.

Pathogenesis of reflux esophagitis and patterns of gastroesophageal reflux in asymptomatic human subjects and hiatal hernia patients.

健常人7名と逆流性食道炎の有無で分けた滑脱型食道裂孔ヘルニア17例を対象に24時間食道pH測定を行い, gastroesophageal reflux (GER) パターンと食道炎の病因について検討した. 健常人のGERは食後1時間に最も多く, 時間当りの逆流回数, 逆流時間共日中が夜間を上回る値であった. 食道炎のある裂孔ヘルニア症例は健常人と比べ, 時間当りの逆流回数, 逆流時間の双方において日中, 夜間共有意に上回る値を示し, また夜間の1回のGER時間は10.5分と最長であった. 以上より, 正常のGERパターンは嚥下によるLES弛緩と酸自浄能のバランスからなる恒常性を持つことがうかがわれた. また, 食道炎発生には酸自浄能の低下がより大きく関与することが示唆された.
