Experimental and Clinical Study on ATP-MgCl2 Administration for Postischemic Acute Renal Failure

The present study was undertaken to investigate the effect of ATP-MgCl2 on the recovery of renal function following renal ischemia. Bilateral renal ischemia was produced for 90 minutes in dogs. Immediately after the release of ischemia, ATP-MgCl2 (50 mumoles/kg) was given intravenously. Serum creatinine and FeNa were measured following the release of ischemia. Renal cellular energy charge, glomerular endothelial thickness and per cent circularity of interstitial cells were measured. Creatinine and FeNa were significantly lower in ATP-MgCl2 treated dogs compared to those in saline treated controls. Changes in energy charge, glomerular endothelial thickness and per cent circularity indicated ischemically induced renal cellular edema was reversed with ATP-MgCl2 through the improvement of energy metabolism. Taking those experimental data into consideration, ATP-MgCl2 was given to 16 acute renal failure patients and 13 patients survived. ATP-MgCl2 administration is effective for the treatment of acute renal failure.

Assortment of Posthepatectomy Hyperbilirubinemia by the Occurring time with Reference to Indication of Plasma Exchange

This study was undertaken to assort the group of posthepatectomy hyperbilirubinemia by occurring time for helping elucidate its indication of plasma exchange (PE). PE was carried out for 14 cases among 198 hepatectomized patients. The 14 cases were classified for 3 groups due to the period between operation and PE. 1: Early type (less than 2 weeks), 2:Intermediate type (2 less than-less than 8 weeks), 3: Late type (greater than 8 weeks). The results were as follows. Number (liver cirrhosis (LC)) [table: see text]. The effect was expected for the early groups since PE would remove overload against the regenerating residual liver. However, there were no survival cases among the early group or LC group. These results suggest that PE may be effective for the slowly increasing hyperbilirubinemia in not cirrhotic patients and that the state of hyperbilirubinemia is the terminal stage for LC patients even under the particular situation such as posthepatectomy.

Renal osteodystrophy in renal transplant recipients and hemodialysis patients.

腎移植患者30例 (年齢35.6±7.5歳, 透析期間2.8±1.6年, 腎移植期間5.9±4.2年) と血液透析患者79例 (年齢46.7±10.5歳, 透析期間8.8±4.7年) とを, 症状や骨代謝指標について比較検討した. 骨関節症状は, 透析患者で62%であったが, 移植患者では6.6%と低率であった. 移植患者の血清クレアチニン値は1.6±0.6mg/dlで, 骨代謝は透析群に比し移植群で血清Caが高く, 無機リン, β2-microglobulin (β2-MG), アルミニウム, C-PTH, N-PTH, アルカリフォスファターゼは低かった. 移植群のオステオカルシン値は15.3±20.1ng/mlで, 透析群の約1/4であったが, 正常値上限の約3倍であった. MD法のMCIスコア, ΣGS/Dスコア, 総スコアでは, 透析群と移植群に差はなかった. 手根骨のcystic radiolucencyは移植群の24%に認められた.移植腎機能の改善とともに骨代謝も改善するが, オステオカルシンが高値であり骨代謝回転は正常化しておらず, MD法各スコアも高く骨病変の改善は認められなかった. 腎移植による骨代謝改善は数年を要するものと思われた.

Effects and limitations of plasma exchange in acute hepatic failure. Comparison with hemoadsorption and evaluation of similar cases of impaired regeneration syndrome.

急性肝不全 (AHF) に対する血漿交換 (PE) の効果と限界について検討した. 1975年9月から1987年2月までに当科でAHFで血液浄化法を施行した44例をPE中心治療群26例と血液吸着 (HA) 治療群18例に分け検討した.救命率はそれぞれ27%, 22%と両群に差がなく, 剖検時肝重量にも差を認めなかった. PE施行平均回数は, 死亡例で救命例の2倍であった. PE前後でビリルビン, 胆汁酸は除去され, アンチトロンビンIII, C4は補充されるが正常値まで改善しなかった. PEはAHF治療に対し延命効果があるが, 除去, 補充療法としては不十分であった. PE施行例の中に長期間覚醒後肝不全で死亡した4例を経験した. その剖検時肝病理組織像はimpaired regeneration syndrome (IRS) に類似していた. AHFのIRS類似例の肝再生不良像に注目し, 肝再生を促す人工補助肝の開発が必要と思われた.

Evaluation and surgical management of emergency operation, including that for diabetic nephropathy, in hemodialysis patients.

1966年9月より1989年6月まで当施設で慢性維持透析患者の併存病変に対して施行した緊急手術72例, 待期手術164例について比較検討した. 手術死亡率は, 緊急手術で22%, 待期手術で0.6%であり, 術後合併症も緊急手術で多かった. 最近, 糖尿病性腎症例の緊急手術例を3例経験したが, 全例に術前合併症を認め, 術後合併症も心不全や感染症を全例に認めた. 糖尿病性腎症緊急手術例では, 特に水分管理と院内感染予防が重要である. 低分子ヘパリンを用いた術後初回透析で凝固時間の延長と後出血を認めず十分に施行できた. 術後透析および術直前透析では, 低分子ヘパリンやnafamostat mesilateを用いた透析か, EVA膜少ヘパリン透析が有用である. 汎発性腹膜炎等では手術にて感染および壊死巣を除去し, その後透析を行い, 上部消化管穿孔等では, 前日透析してあれば即手術を, 前々日の透析であれば透析後に手術をするのがよいと考えている.

A report of successful pregnancy and delivery in a hemodialysis patient, and normal developement in her child

帝王切開後4年5か月経過した現在, 母子ともに健在な血液透析患者の妊娠出産例を経験したので報告する.症例は33歳女性, 原疾患は慢性糸球体腎炎で透析歴は5年であった. 腹部膨隆を自覚し産科を受診したところ妊娠14週と診断された. 33週で帝王切開にて出産し, 新生児は体重1584gでApgar scoreは7であり, 乳児発育に異常は認められなかった.本邦における慢性腎不全患者の出産例は1986年までに15例の報告があり, 本症例は第8例目であった. 合併症として早産, 弛緩出血, 肝障害および羊水過多が多く, 早産防止には羊水過多の管理が重要である.
