Complete long-term survival data from a trial of adjuvant chemotherapy vs control after radical cystectomy for locally advanced bladder cancer

To report the long‐term follow‐up of patients with locally advanced bladder cancer treated with either adjuvant combined chemotherapy with methotrexate, vinblastine, doxorubicin/epirubicin, and cisplatin (MVAC/MVEC) or no additional treatment after radical cystectomy, to examine various survival endpoints and factors associated with long‐term survival.

Urological De Novo Malignancy After Kidney Transplantation: A Case for the Urologist

PURPOSE The development of de novo malignancies is a well-known complication after renal transplantation. With increasing donor and recipient age the risk of post-transplant malignancy including genitourinary cancers is increasing. Thus, urologists have an increasing likelihood of treating these cases. We report our experience with the management of urological de novo malignancies after renal transplantation. MATERIALS AND METHODS Urological de novo malignancies developed in 29 of 802 patients after renal transplantation between 1988 and 2009. Data were analyzed for tumor incidence, treatment, followup and possible factors contributing to tumor development. RESULTS Patients had renal cell carcinoma (12 at a median of 46.5 months after renal transplantation), transitional cell carcinoma (6 bladder cases at 35 months, 2 renal pelvis cases at 37.5 months), carcinoma of the prostate (7 at 69 months) and seminoma (2 at 41.5 months). No treatment related graft losses occurred. Of 3 cases of renal cell carcinoma developing in the graft 2 were treated with nephron sparing surgery. CONCLUSIONS Urological post-transplant malignancies are an increasing problem for urologists. Regular surveillance after renal transplantation is mandatory to detect early occurrence of de novo malignancies and standard urological treatment principles can be applied. Nonfunctioning native kidneys with suspicious lesions should be removed early. Radical pelvic surgery after renal transplantation and nephron sparing procedures in the graft can be a challenge even for the experienced urologist, and require surgical versatility.

Intussuscepted ileal flap valve for revisional surgery

If the construction of a continent outlet for continent cutaneous urinary diversion (CCUD) is required in revisional surgery, the ileal intussusception flap valve is a straightforward and reliable solution to the problem. Construction of a continent outlet in the presence of an established intestinal urinary pouch may be required for conversion of an orthotopic bladder substitution into a CCUD or for repairing a failed continence mechanism in a CCUD. However, the surgical technique may also be used to convert any other form of urinary diversion (e.g. conduit diversion, ureterosigmoidostomy) into a CCUD.

Gleichzeitig handelt es sich dabei um die seltene histologische Variante eines plexiformen Schwannoms. Diese Variante des Schwannoms imitiert wegen ihres multinodulären Wachstumsmusters das plexiforme Neurofibrom und wird ebenfalls zu den gutartigen Tumoren der Nervenscheiden gezählt.AbstractIn this report we describe the case of a testicular schwannoma in a human. Histologically, the tumor presented as a plexiform schwannoma. This rare benign peripheral nerve sheath tumor is an uncommon nodular variant of schwannoma, which mimics plexiform neurofibroma by its multinodular growth.
