Tincreasing the intensity of radiation therapy and chemotherapy in the management of cancer has increased the incidence of adverse effects, especially oral mucositis. AIM AND METHODS: a bibliographical review was conducted on the definition of oral mucositis, its clinical findings, the incidence, its etiology, the pathofisiology, associated morbidity, prevention and treatment. RESULTS: current studies define oral mucositis as a very frequent and painful inflammation with ulcers on the oral mucosa that are covered by a pseudo membrane. The incidence and severity of lesions are influenced by patient and treatment variables. Oral mucositis is a result of two major mechanisms: direct toxicity on the mucosa and myelosuppression due to the treatment. Its pathofisiology is composed of four interdependent phases: an initial inflammatory/vascular phase; an epithelial phase; an ulcerative/bacteriological phase; and a healing phase. It is considered a potential source of life-threatening infection and often is a dose-limiting factor in anticancer therapy. Some interventions have been shown to be potentially effective to prevent and treat oral mucositis. Further intensive research through well-structured clinical trials to obtain the best scientific evidence over the standard therapy of oral mucositis is necessary to attain ideal parameters for radiotherapy and chemotherapy.

Phototherapy is noninvasive, painless and has no known side effect. However, for its incorporation into clinical practice, more well-designed studies are necessary to define optimal parameters for its application. The viability of fibroblasts cultured under nutritional stress irradiated with either a red laser, an infrared laser, or a red light-emitting diode (LED) was analyzed. Irradiation parameters were: red laser (660 nm, 40 mW, 1 W/cm(2)), infrared laser (780 nm, 40 mW, 1 W/cm(2)), and red LED (637 ± 15 nm, 40 mW, 1 W/cm(2)). All applications were punctual and performed with a spot with 0.4 mm(2) of diameter for 4 or 8 s. The Kruskal-Wallis test and analysis of variance of the general linear model (p ≤ 0.05) were used for statistical analysis. After 72 h, phototherapy with low-intensity laser and LED showed no toxicity at the cellular level. It even stimulated methylthiazol tetrazolium assay (MTT) conversion and neutral red uptake of fibroblasts cultured under nutritional stress, especially in the group irradiated with infrared laser (p = 0.004 for MTT conversion and p < 0.001 for neutral red uptake). Considering the parameters and protocol of phototherapy used, it can be concluded that phototherapy stimulated the viability of fibroblasts cultured under nutritional deficit resembling those found in traumatized tissue in which cell viability is reduced.

The aim of this study was to determine, by Atomic Absorption Spectrometry (AAS), the concentration levels of 11 metals in Type V gray and structural white PC, ProRoot MTA, and MTA Bio. Samples, containing one gram of each tested cement, were prepared and transferred to a 100 mL Teflon tube with a mixture of 7.0 mL of nitric acid and 21 mL of hydrochloric acid. After the reaction, the mixture was filtered and then volumed to 50 mL of distilled water. For each metal, specific patterns were determined from universal standards. Arsenic quantification was performed by hydride generator. The analysis was performed five times and the data were statistically analyzed at 5% level of significance. Only the cadmium presented concentration levels of values lower than the quantification limit of the device. The AAS analysis showed increased levels of calcium, nickel, and zinc in structural white PC. Type V PC presented the greatest concentration levels of arsenic, chromium, copper, iron, lead, and manganese (P < 0.05). Bismuth was found in all cements, and the lowest concentration levels were observed in Portland cements, while the highest were observed in ProRoot MTA. Both PC and MTA-based cements showed evidence of metals inclusion.

Background: A cross-sectional study was conducted at the Craniofacial Rehabilitation Center in the General Hospital of the University of Cuiabα, Cuiabα city, Mato Grosso, Brazil. Materials and Methods: Poisson regression model was used to analyze the relationship between antenatal factors and the occurrence of oral clefts in 116 patients. Results: Oral clefts were more common in males (64.66%) and White race (46.02%). The mean age of the children was 21.91 months. The most common type of cleft was cleft lip and palate (CLP, 55.17%). Maternal and paternal smoking in the first trimester of pregnancy and parity were significantly associated with the occurrence of CLP. Parent′s age, educational level, and occupation did not interfere in the occurrence of oral clefts. There was also no significant association between maternal illness, medication use, alcohol consumption, and maternal exposure to chemicals in the first trimester of pregnancy and the occurrence of clefts in this population. Conclusion: The analysis of the environmental factors present during the pregnancy of children with oral clefts revealed a significant association between parity (second onward), maternal smoking, and paternal smoking and the occurrence of CL and/or palate in this population.

Natal teeth are those present in the oral cavity at the childs birth. These teeth can cause ulcers on the ventral surface of the tongue, lip, and the mothers breast characterizing the Riga-Fede Disease. The treatment depends on the tooths mobility and the risk of aspiration or swallowing; whether it is supernumerary or regular primary teeth; whether it is causing interference in breastfeeding; breast and oral soft tissue injuries; and the general state of childs health. A 1-month-old female infant was diagnosed with two natal teeth and an ulcerated lesion on the ventral surface of the tongue, leading to the clinical diagnosis of Riga-Fede Disease. The treatment performed consisted of the maintenance of the natal tooth that showed no increased mobility, adding a small increment of glass ionomer cement to its incisal edge, and orientation for hygiene with saline solution. Due to the increased mobility of the other natal tooth, surgical removal was performed. There was regular monitoring of the patient and complete wound healing was observed after 15 days. The proposed treatment was successful and the patient is still in follow-up without recurrence of the lesion after one year.

Reconstruction of the parotid duct

Ana Luiza Lima Medeiros Paz; Rafael da Silva Caetano; Álvaro Henrique Borges; Luiz Evaristo Ricci Volpato

The most common causes of parotid duct lesions are injuries with sharp instruments. Late alterations after such lesions lead to esthetic defects and complicates the treatment. This case report presents an alternative surgical technique for late reparation of the parotid duct. A 31-year-old male patient was admitted with a history of physical aggression by a glass bottle, resulting in cutting injuries in face and drainage of extraoral transparent fluid from the buccinatory region lesion Antibiotic therapy was initiated immediately, and the surgery was performed nine days after the incident. The repair of the parotid duct lesion was achieved adapting a venous catheter that was passed through the distal portion of the duct from the oral orifice. The proximal segment of the duct was cannulated, the lacerated ends of the duct were approximated on the device and sutured with prolipopylene thread. At the 14-day post-operative return, the saliva was draining through the catheter, and then it was removed. Patients with cutaneous lesions in the buccinatory region require a thorough examination to identify injuries to the parotid duct. The surgical treatment using the presented alternative surgical technique was an effective and cheaper option when compared to other alternatives and showed no complications.

Produção endodôntica no atendimento público odontológico do Centro-Oeste do Brasil

Thiago Machado Pereira; Álvaro Henrique Borges; Laura Maria de Amorim Santana Costa; Durvalino de Oliveira; Luiz Evaristo Ricci Volpato

Little is known about the use of public dental services in the center-west region of Brazil, and even less about the number of endodontic treatments. Endodontic procedures through the Brazilian Unified Health System (Sistema Unico de Saude; SUS) are performed mainly in specialized public centers for dental care, and these data are available from the database of the Brazilian Ministry of Health (DATASUS). This study aimed to determine the number of endodontic procedures performed in state capitals in the center-west of Brazil. We conducted a cross-sectional study with retrospective data collection of all endodontic procedures performed through the SUS from January 2011 to December 2016. Data were obtained from the DATASUS and from the website of the Brazilian Institute of Geography and Statistics and were compared by analysis of variance, using Kolmogorov-Smirnov and Levene’s tests followed by Tukey post hoc test at a significance level of p<0.05. Campo Grande had the largest number of endodontic treatments per inhabitant (0.063), followed by Cuiaba and Goiânia, with similar rates (0.028 and 0.026 respectively), and Brasilia (0.006). Teeth with three or more root canals had significantly more endodontic interventions than teeth with one or two root canals (p<0.05). Higher education institutions must be attentive to the changes in the model of dental care offered by the SUS so that their graduates are trained in accordance with the epidemiological profile of the population as well as with health public policies.
