Embolization of nonvariceal portosystemic collaterals in transjugular intrahepatic portosystemic shunts

Percutaneous embolization of large portosystemic collaterals was performed in three patients following placement of a transjugular intrahepatic portosystemic shunt in order to improve hepatopetal portal flow. Improved hepatic portal perfusion was achieved in these cases, thereby theoretically reducing the risk of chronic hepatic encephalopathy.

Utilidad de la endoscopia en la hemorragia digestiva por vasculitis

Vasculitides constitute a heterogeneous group of diseases characterized by inflammation of blood vessels. The skin is mainly affected, although the gastrointestinal tract mucosa can also be involved. The contribution of endoscopy in these cases has not been clearly determined. We report three cases of systemic vasculitis (polyarteritis nodosa, Schonlein-Henoch purpura and Behcets disease) presenting with acute digestive bleeding. Endoscopy was an effective technique for completing the diagnosis and in establishing an effective nonsurgical therapeutic approach in these potentially lethal cases of gastrointestinal hemorrhage.

OP08.01: Role of endoscopic ultrasound guided FNA (EUS-FNA) for confirming pelvic/abdominal recurrence in gynecological cancer: a feasibility study: Short oral presentation abstracts

The objective of the study was to assess the accuracy of 5 ultrasound parameters in determining chorionicity after 14 weeks gestation. Methods: Women with twin pregnancies at first scan after 14 weeks gestation were recruited. Fetal genders, placental number, Tor lambda sign, membrane thickness and number of layers in the membrane were recorded. Placentas and membranes were examined macroscopically and histologically. Results: 85 patients were included. 56 sets of placentas were assessed, and histologically 12 cases were monochorionic (MC) twins and 44 cases dichorionic (DC). The 5 ultrasound parameters were compared with histological chorionicity. Discordant gender excluded MC pregnancy. A single placenta was present in 56%, but the finding of two placentas excluded MC twins. Lambda sign was only commented on in 30 pregnancies. Its presence at any gestation confirmed dichorionicity. Membrane thickness was measured in 46 pregnancies. The sensitivity of a membrane < 2 mm for MC was 90.9% and specificity 71.4%. In 35 pregnancies, the number of layers in the dividing membrane was counted. When 3-4 layers were seen, sensitivity for DC pregnancy was 100%. When 1-2 layers were seen, specificity for MC pregnancy was only 76.9%. Conclusions: Adding membrane thickness and number of layers into the diagnostic process decreases the number of twins of unknown chorionicity. A diagnostic algorithm is proposed.
