OTO Method for Preservation of Actin Filaments in Electron Microscopy

Osmium tetroxide, commonly used as a fixative in electron microscopy, can destroy actin filaments. Thiocarbohydrizide (TCH) is a bipolar substance that binds to the osmium. By sandwiching TCH between two phases of osmium treatment, tissue exposure to osmium could be minimized without destroying actin filaments. The contrast of osmophilic components of cells was also enhanced.

ELISA kit in outbreaks of infectious gastroenteritis among elderly nursing home

序 文 高齢者施設(以下施設)には介護が必要な高齢者が 共同で療養しており,ノロウイルス感染症発生時には, 早期の蔓延防止策の実施が重要である.2006 年 11 月 に管内施設で発生した事例において,ノロウイルス抗 原検出キットを活用した調査を実施したので報告す る. 対象と方法 対象施設は老人保健施設(入所定員:100 名,デイ ケア定員:40 名,職員:55 名,調理従事者:23 名) で,2006 年 11 月に感染性胃腸炎の集団感染が発生し た.当初からノロウイルス感染症を疑い,体外診断用 医薬品として開発されたノロウイルス抗原検出キット NV-AD(II)(デンカ生研)で便検体を検査した.キッ トの使用方法に従い,Index 値(検体の吸光度 陽性 コントロールの吸光度)が 0.5 以上を陽性,0.5 未満 を陰性とした. また,有症者の一部の便については,ノロウイルス の確認のため,平成 15 年 11 月 5 日付食安監発第 1105001 号,厚生労働省医薬食品局食品安全部監視安 全課長通知に記載の方法により名古屋市衛生研究所で RT-PCRを実施した. 結 果 当施設が 11 月 7 日に実施したノロウイルスの自主 検査では,入所者,調理従事者ともに陽性者はなかっ た.集団感染の初発は 11 月 15 日のデイケア利用者で, 発症のピークは 11 月 20~22 日であった(Fig. 1). 保健所への報告は 11 月 19 日にあり,直ちに調査を 行った.喫食および患者の発生状況から,食中毒では なく施設内感染が疑われ,入所者 30 名および職員 41 名の計 71 名について,便のNV-AD(II)検査を実施 した.その結果,有症の入所者 23 名中 9名(39.1%), 職員 19 名中 3名(15.8%)が,無症の入所者 7名中 2 名(28.6%),職員 22 名中 3名(13.6%)が陽性と判 定された.また,RT-PCR検査を実施した有症の入所 者 10 名中 7名および職員 3名中 3名の便からノロウ イルス(GII タイプ)が検出されたことから,ノロウ イルス感染症と診断した.有症者については,継続し てNV-AD(II)検査を実施した.発症後の陽性期間 は職員では 9日間,入所者では 27 日間が最長であっ た(Fig. 1).本事例は有症者数が 96 名(入所者:67 名,デイケア利用者:1名,職員:28 名)に及ぶ集団 発生となったが,再発は認めなかった. 考 察 保健所へ連絡が入るまでの 4日間に,施設内ではデ イケア利用者,入所者,職員の間で感染を繰り返し, 施設全体がノロウイルスで汚染されていたと考えられ る.保健所は届け出後直ちに,可能な限り多くの入所 者および職員のNV-AD(II)検査を実施した.この 結果,原因微生物を早期に特定し,職員,調理従事者 あるいは入所者の家族などに,ノロウイルス感染症の 注意点,対応方法を徹底することができた.ノロウイ ルス抗原検出キットの検出限界を考慮し,無症のNVAD(II)検査陰性の職員でも便中にウイルスが含ま れている可能性があるものとして,標準予防策に準拠 したケア手順を定め,施設職員全員で蔓延防止に努め るよう指導した.有症の職員および無症のNV-AD(II) 検査陽性の職員に対しては,原則 1週間後さらに 3週 間後に検査を行った.その結果が陰性でかつ症状が発 現していない者のみを日常業務に復帰させた.原因微 短 報

Cellular and subcellular distribution of iron in the lamina propria of rat duodenum

SummaryCellular and subcellular distribution of iron in the lamina propria of rat duodenum was studied after a single i.p. injection of iron dextran, using electron microscopy and peroxidase cytochemistry. X-ray spectrum microanalysis was used for positive identification of iron. Ironcontaining particles (IP) were found in the cytoplasm of three cell types, viz. macrophages, pericytic reticular cells and sheathing fibrocytes. IP-containing organelles in lamina propria cells were more heterogeneous compared to absorptive cells and, in addition, some differences were noted in the subcellular distribution of IP in the 3 cell types. A common denominator in these 3 cell types was the presence of endogenous peroxidase, also shared by Kupffer cells which are known to be involved in iron storage. Peroxidase activity was absent in absorptive epithelial cells. It is hypothesized that the cells of the lamina propria, like Kupffer cells, may be the site of storage of excess iron absorbed, releasing iron upon demand and migrating into the lumen to prevent iron overload. In this fashion they may regulate the exchange of iron with the environment. The presence of peroxidase in these as well as Kupffer cells, and its absence in absorptive cells also raises the possibility that this enzyme may be related to certain aspects of iron storing process.
