Acitretin treatment of a systematized inflammatory linear verrucous epidermal naevus.

Sir, Inflammatory linear verrucous epidermal naevus (ILVEN) is a rare, yet clinically characteristic disease. ILVEN was characterized as a type of epidermal naevus by Altman & Mehregan (1) in 1971. From a clinical and histological point of view, ILVEN and psoriasis are similar. Sometimes only the clinical course of the disease, the persistent resistance to local therapy and the strong itching may suggest the diagnosis. About 5% of epidermal naevi are ILVEN; most are solitary lesions. In contrast, systematized ILVEN involving wide areas of the integument has only rarely been reported (2, 3). Local treatment of systematized ILVEN with potent corticosteroids or tretinoin 0.1% and good clinical response to oral retinoids in systematized verrucous epidermal naevus (4) have been described. ZusammenfassungDas seltene Multiple-Lentigines (LEOPARD)-Syndrom, ein komplexes Fehlbildungssyndrom mit Haut-, Herz-, Skelett- und anderen Auffälligkeiten wird anhand eines 36-jährigen männlichen Patienten und dessen 9-jähriger Tochter beschrieben. Ätiopathogenetische Überlegungen, Klinik und differenzialdiagnostische Aspekte werden diskutiert.Beim Auftreten multipler Lentigines sollte stets eine weiterführende, insbesondere kardiale Diagnostik veranlasst werden.AbstractThe rarely occurring multiple lentigines (LEOPARD) syndrome represents a complex of skin, heart, skeleton and other malformations and is described in a 36-year-old male and his 9-year-old daughter. With the occurrence of multiple lentigines, the diagnostic search for further malformations should always be undertaken.Its differential diagnosis and its pathogenetic and clinical aspects are discussed in this paper.

cANCA-positive Vaskulitis der Haut und Nieren im Verlauf einer Acne vulgaris conglobata

ZusammenfassungBericht über einen 19-jährigen Patienten mit schwerer Acne vulgaris conglobata und ulzerierenden Pyoderma-gangraenosum-artigen Hautveränderungen im Gesicht, der zunächst mit Isotretinoin (Roaccutan®), dann mit Prednisolon, Diaminodiphenylsulfon (Dapson-Fatol®) und Mycophenolatmofetil (CellCept®) behandelt wurde. Unter der Erhaltungsdosis einer immunsuppressiven Therapie mit 2,5 mg Prednisolon und 1 g CellCept® pro Tag kam es akut zu Schwächegefühl, Muskel- und Gelenkschmerzen. Ansteigende Retentionsparameter mit Proteinurie sowie jetzt stark erhöhte antineutrophile zytoplasmatische Antikörper (cANCA) und der histologische Befund waren richtungsweisend für die Diagnose einer cANCA-assoziierten leukozytoklastischen Vaskulitis der Haut und Nieren. Gutes Ansprechen auf eine Cyclophosphamid (Endoxan®)-Stoßtherapie zusammen mit der kontinuierlichen Gabe von Prednisolon®. Die genaue Einordnung der Erkrankung innerhalb eines Klassifikationsschemas war jedoch nicht möglich.AbstractA 19 year old man with severe acne conglobata and ulcerated pyoderma gangraenosum-like skin lesions on the face was first treated with isotretinoin (Roaccutan®), then immunosuppressively with prednisolone, diaminodiphenylsulfone (Dapson-Fatol®) and mycophenolate mofetil (Cellcept®). Under a daily maintenance dose of immunosuppressive treatment with 2.5 mg prednisolone and 1 g mycophenolate mofetil, weakness, muscle and joint aches appeared. Due to proteinuria, haematuria and an elevated antineutrophil cytoplasmic antibody (cANCA) and the histological detection of a leukocytoclastic vasculitis, the diagnosis of cANCA positive vasculitis of the skin and kidneys was established. Therapy with cyclophosphamide (Endoxan®) along with prednisolone was effective. An exact classification of this disease process was not possible.
