Depression, anxiety and stress in primary health care users

Os objetivos deste estudo foram descrever niveis de depressao, ansiedade e estresse dos usuarios de um centro de saude urbano/rural, analisar diferencas de sexo e a comorbilidade entre depressao, ansiedade e estresse. Este e um estudo descritivo correlacional, aplicando-se uma versao portuguesa da DASS-21, em amostra consecutiva (n=343). Tem-se como resultados que 40,52, 43,48 e 45,06% dos individuos apresentam, respetivamente, algum grau de depressao, ansiedade e estresse. Identificaram-se niveis graves ou muito graves de ansiedade em 20,87% dos individuos, de estresse em 22,38% e de depressao em 12,24%. As mulheres apresentam niveis medios de depressao, ansiedade e estresse mais elevados. Depressao, ansiedade e estresse estao forte e positivamente associados. Esses resultados apontam para elevada prevalencia - mais alta que em outros paises - e revelam diferencas de sexo e comorbilidade. Com base neles, poder-se-a desenvolver estrategia de intervencao local e comunitaria para a promocao da saude mental e prevencao da doenca, particularizando as mulheres.AIM Describe levels of depression, anxiety and stress among users of an urban/rural health centre, analyse sex differences and comorbidity between depression, anxiety and stress. METHOD Descriptive co-relational study with a Portuguese version of DASS-21; consecutive sample (n=343). RESULTS 40.52%, 43.48% and 45.06% of individuals present some degree of depression, anxiety and of stress, respectively. Severe or very severe levels of disturbance were found in 20.87% (anxiety), 22.38% (stress) and 12.24% (depression) of individuals. Women present higher levels of depression, anxiety and stress. Depression, Anxiety and Stress are strongly and positively associated. CONCLUSION These results show high point prevalence--higher than in other countries--and reveal differences in sex as well as comorbidity. They may allow for the development of a local and community intervention strategy for mental health promotion and disease prevention, particularly for women.

Development of the e-Baby Serious Game With Regard to the Evaluation of Oxygenation in Preterm Babies Contributions of the Emotional Design

The present study aimed to describe the development process of a serious game that enables users to evaluate the respiratory process in a preterm infant based on an emotional design model. The e-Baby serious game was built to feature the simulated environment of an incubator, in which the user performs a clinical evaluation of the respiratory process in a virtual preterm infant. The user learns about the preterm baby’s history, chooses the tools for the clinical evaluation, evaluates the baby, and determines whether his/her evaluation is appropriate. The e-Baby game presents phases that contain respiratory process impairments of higher or lower complexity in the virtual preterm baby. Included links give the user the option of recording the entire evaluation procedure and sharing his/her performance on a social network. e-Baby integrates a Clinical Evaluation of the Preterm Baby course in the Moodle virtual environment. This game, which evaluates the respiratory process in preterm infants, could support a more flexible, attractive, and interactive teaching and learning process that includes simulations with features very similar to neonatal unit realities, thus allowing more appropriate training for clinical oxygenation evaluations in at-risk preterm infants. e-Baby allows advanced user-technology-educational interactions because it requires active participation in the process and is emotionally integrated.

Self-confidence for emergency intervention: adaptation and cultural validation of the Self-confidence Scale in nursing students

Profesor Coordenador, Escola Superior de Enfermagem de Coimbra, Coimbra, Portugal.Objective: develop the cultural adaptation and validation of a Portuguese version of the Self-confidence Scale. Method: descriptive and exploratory methodological research for the adaptation and validation of a measuring instrument. The translation, synthesis, back-translation, revision, pretest and semantic evaluation phases were accomplished. The evaluation involving 178 students from a Teaching Diploma Program in Nursing. The ethical principles were complied with. Results: the internal consistency analysis of the scale reveals good Alpha coefficients (0.92 for the global scale and superior to 0.83 for the different dimensions). The factor analysis presents a three-factor solution with rational meaning. Conclusion: The scale is easy to answer and understand. Based on the obtained results, it can be affirmed that the scale reveals good psychometric properties, with great potential to be used in future research.

Saúde e bem-estar de crianças em idade escolar

A percepcao que as criancas tem sobre a sua propria saude e um importante indicador para a definicao de estrategias de promocao de saude infantil. Este estudo correlacional analisa a percepcao de saude de uma amostra probabilistica acidental de 216 criancas portuguesas do ensino basico. O questionario de recolha de dados (CHIP-CE) revelou boas propriedades psicometricas (alfa=0,84). Os resultados indicam medias e desvios-padrao de cinco fatores: Conforto (4,34; 0,45); Evitamento de risco (4,18; 0,53); Satisfacao (4,03; 0,56); Resiliencia (3,75; 0,60); Realizacao (3,75; 0,60). Resiliencia e evitamento de risco sao fatores com valores mais baixos. A dificuldade em realizar os trabalhos de casa e conviver com os colegas e indicadora da dimensao resiliencia mais problematica. Os resultados permitem descrever o perfil de saude e planejar uma intervencao de educacao para a saude das criancas, em contexto escolar, especialmente na area do relacionamento e motivacao para o estudo.

Effectiveness of heparin versus 0.9% saline solution in maintaining the permeability of central venous catheters: a systematic review

OBJETIVO Determinar qual e a solucao (flush heparina comparado com oflushde soro fisiologico 0.9%) mais eficaz na reducao do risco de oclusoes de cateteres venosos centrais (CVC) em adultos. METODO A revisao sistematica seguiu os principios propostos pelo Cochrane Handbook; a analise critica, a extracao e a sintese dos dados foram realizadas por dois investigadores, isoladamente; e a analise estatistica efetuada com recurso ao programa RevMan 5.2.8. RESULTADOS Foram incluidos oito estudos randomizados controlados e um estudo de coorte e os resultados da meta-analise mostram nao existir diferencas (RR=0.68, IC 95%=0.41-1.10; p=0.12). A analise por subgrupos mostra que nos CVC totalmente implantados nao se verificaram diferencas (RR=1.09, IC 95%=0.53-2.22; p=0.82); nos CVC com varios lumens existiu um efeito benefico no grupo da heparina (RR=0.53, IC 95%=0.29-0.95;p=0.03); nos CVC de duplo lumen para hemodialise (RR=1.18, IC 95%=0.08-17.82; p=0.90) e nos CVC de insercao periferica (RR=0.14, IC 95%=0.01-2.60; p=0.19) tambem nao se verificaram diferencas. CONCLUSAO O soro fisiologico e suficiente para manter a permeabilidade dos cateteres venosos centrais, prevenindo os riscos associados a administracao da heparina.OBJECTIVE Determining which is the most effective solution (heparin flush compared to 0.9% saline flush) for reducing the risk of occlusions in central venous catheters (CVC) in adults. METHOD The systematic review followed the principles proposed by the Cochrane Handbook; critical analysis, extraction and synthesis of data were performed by two independent researchers; statistical analysis was performed using the RevMan program 5.2.8. RESULTS Eight randomized controlled trials and one cohort study were included and the results of the meta-analysis showed no difference (RR=0.68, 95% CI=0.41-1.10; p=0.12). Analysis by subgroups showed that there was no difference in fully deployed CVC (RR=1.09, CI 95%=0.53-2.22; p=0.82); Multi-Lumen CVC showed beneficial effects in the heparin group (RR=0.53, CI 95%=0.29-0.95; p=0.03); in Double-Lumen CVC for hemodialysis (RR=1.18, CI 95%=0.08-17.82; p=0.90) and Peripherally inserted CVC (RR=0.14, CI 95%=0.01-2.60; p=0.19) also showed no difference. CONCLUSION Saline solution is sufficient for maintaining patency of the central venous catheter, preventing the risks associated with heparin administration.

Objectives: To culturally adapt the Cardiac Patients’ Learning Needs Inventory for use in Brazil and to test its reliability (internal consistency and stability) in Brazilian patients with coronary artery disease. Methods: The study included 65 patients with acute myocardial infarction, hospitalized in a public hospital in the state of Sao Paulo. For data collection, we used an instrument for sociodemographics characteristics and the Portuguese version of the Cardiac Patients’ Learning Needs Inventory. Internal consistency was estimated based on Cronbach’s alpha. The stability was established using the test-retest method and calculated using the Student’s t-test. The level of significance was 0.05. Results: We identified high internal consistency (0.96 in the first step, and 0.78 in the second). The domain that presented better internal consistency was Risk Factors (α = 0.91).
