Associations among socio-demographic and clinical factors and the quality of life of ostomized patients

This study identifies the socio-demographic and clinical factors of patients with irreversible colostomy secondary to colorectal cancer and correlates them with quality of life (QOL). It is a cross-sectional study. Socio-demographic and clinical data were collected through interviews and the WHOQOL-bref to assess QOL. The sample comprised 60 patients. Most of the patients were male, elderly individuals, half were married and half did not have a sexual partner, with complete primary education, receiving up to two times the minimum wage, carried a stoma for three months on average, were instructed they would carry a stoma, but did not have their stoma marked prior to surgery. The average QOL score was 75.500, while the psychological, social and physical domains were the most affected. No statistically significant differences were found in QOL in relation to the following socio-demographic and clinical factors: female gender, low income, no sexual partners, and lack of instruction. The patients with an intestinal stoma presented a satisfactory QOL.Os objetivos deste estudo foram identificar os fatores sociodemograficos e clinicos dos pacientes com estoma intestinal definitivo, secundario ao câncer colorretal, e correlaciona-los a qualidade de vida (QV). Como metodos usaram-se o estudo transversal, utilizando a entrevista como instrumento de coleta de dados sociodemograficos e clinicos, e, para avaliacao da QV, o WHOQOL-bref. A amostra foi composta por 60 pacientes. Como resultados tem-se que a maioria dos pacientes era do sexo masculino, idoso, casado, sem parceiro sexual, com ensino fundamental completo, recebia ate dois salarios-minimos, tempo medio de estoma de tres meses, foram orientados que portariam um estoma, mas nao foi demarcado para a cirurgia. A media da QV foi de 75,00, sendo que os dominios psicologico, social e fisico foram os mais afetados. Os fatores sociodemograficos e clinicos: sexo feminino, baixa renda, nao ter parceiros sexuais e falta de orientacao apresentaram diferencas estatisticas significantes na QV. Conclui-s que os pacientes portadores de estoma intestinal demonstraram QV satisfatoria.

Nurses' performance on hospital discharge: patients' point of view

Objective: To know the hospital discharge process in place and the nurses’ performance in preparing patients for discharge. Methods: A descriptive study using semi-structured interviews was used to collect data from 43 patients of medical-surgical units of a major teaching hospital in the state of Sao Paulo, Brazil. Results: The majority of patients (83.72%) received tailored discharge instructions. However, a great number of patients (72.08%) reported discharge instructions were not given by nurses. Almost a half of patients (48.84%) reported that discharge instructions were given by their physicians. Conclusion: The findings of this study provide insights to improve the educational process of new nurses and their preparation to provide effective discharge instructions. There is also a need to design and implement a hospital discharge process that promotes the participation of interdisciplinary health care providers who are involved in patient clinical care. This discharge process might be an effective way to change health care providers’ attitude toward discharge instructions.

Atuação do enfermeiro na alta hospitalar: reflexões a partir dos relatos de pacientes

Elaboração de um instrumento da assistência de enfermagem na unidade de hemodiálise

This study aimed at developing an instrument to record nursing assistance systematization in a hemodialysis unit. For this development, we used the instrument formulated from literature on nursing diagnosis in nephropathic patients. The record instrument has been analyzed according to: layout and process facilitation. The process relied on the Basic Human Needs Theory and the taxonomy of the North American Nursing Diagnosis Association. The instrument has been facilitating the implementation of the nursing process in the hemodialysis unit. Health care service for ostomy patients: profile of the clientele

The study aimed to identify the profile of ostomy patients in a Health Care Service in Sao Jose do Rio Preto, Sao Paulo, Brazil. This is an exploratory, descriptive and retrospective study. Data were obtained by registration forms of patients assisted from January 1st, 2000, to December 31st, 2010. Out of the 252 ostomy patients, 51.1% were females and 48.9% were males; the age group with the highest concentration was from 68 to 78 years old (26.3%) for both genders, with mean age of 73 years old. The main reason for making the stoma was rectal (35.0%) and colon neoplasm (14.1%). The prevalent stoma was temporary colostomy (41.4%) and the period of permanence of the collecting equipment was longer than 36 months. Even though the service provides full assistance to the ostomy patients, it is necessary to review human resources aspects to provide appropriate assistance to its clientele.

Estratégias de enfrentamento dos enfermeiros em serviço hospitalar de emergência

Objective: To identify the coping strategies of nurses in hospital emergency services, and relate them to sociodemographic and professional variables. Methods: Cross-sectional study with 89 nurses. Research instruments included a form to characterize the research subjects and the Ways of Coping Questionnaire by Folkman and Lazarus. Results: The most commonly used coping strategies were problem solving and positive reappraisal, whereas the least used was confrontation. The strategies of confrontation, positive reappraisal and escape-avoidance were associated with the male sex, not having a partner and working night shifts, respectively. Conclusion: Coping strategies can be aided by listening, monitoring, educational programs and creation of a space for discussion of work-related difficulties.

Complications of central venous catheter in patients transplanted with hematopoietic stem cells in a specialized service

Abstract Objective: to identify the model, average length of stay on site and complications of central venous catheter in patients undergoing transplant of hematopoietic stem cells and verify the corresponding relationship between the variables: age, gender, medical diagnosis, type of transplant, implanted catheter and insertion site. Method: a retrospective and quantitative study with a sample of 188 patients transplanted records between 2007 and 2011. Results: the majority of patients used Hickman catheter with an average length of stay on site of 47.6 days. The complication fever/bacteremia was significant in young males with non-Hodgkins lymphoma undergoing autologous transplant, which remained with the device for a long period in the subclavian vein. Conclusion: nurses should plan with their team the minimum waiting time, recommended between the catheter insertion and start of the conditioning regimen, as well as not to extend the length of time that catheter should be on site and undertake their continuing education, focusing on the prevention of complications.Objetivo: identificar o modelo, tempo medio de permanencia e complicacoes de cateter venoso central em pacientes submetidos ao transplante de celulas-tronco hematopoieticas e verificar a relacao de correspondencia entre as variaveis: idade, sexo, diagnostico medico, tipo de transplante, cateter implantado e local de insercao. Metodo: retrospectivo, quantitativo, com amostra de prontuarios de 188 pacientes transplantados, entre 2007 e 2011. Resultados: a maioria dos pacientes utilizou o cateter de Hickman com permanencia media de 47,6 dias. A complicacao febre/bacteremia foi significante em jovens do sexo masculino, com linfoma nao Hodgkin, submetidos ao transplante autologo, que permaneceram com o dispositivo por longo periodo, em veia subclavia. Conclusao: os enfermeiros devem planejar com a equipe o aguardo do tempo minimo preconizado entre o implante do cateter e inicio do regime de condicionamento, assim como nao estender o periodo de permanencia e realizar educacao permanente, enfocando a prevencao das complicacoes.

Relationship between anxiety, depressive symptoms and compulsive overeating disorder in patients with cardiovascular diseases

ABSTRACT Objectives: to identify the presence of compulsive overeating disorder in patients with cardiovascular diseases and to verify its relation with sociodemographic, clinical variables and the presence of anxiety and depressive symptoms. Method: cross-sectional, correlational study with a sample of 111 patients with cardiovascular diseases. The presence of anxiety and depressive symptoms was assessed by the Hospital Anxiety and Depression Scale instrument and compulsive overeating disorder was assessed through a likert instrument called the Periodic Eating Disorder Scale (Binge Eating Scale). Results: there was a predominance of patients without compulsive overeating disorder (n=91, 82%), followed by moderated compulsive overeating (n=15, 13.5%) and severe (n=5, 4.5%) associating to high levels of body mass index (p=0.010) and the presence of anxiety (p=0.017). Conclusion: Compulsive overeating disorder was present in 18% of the patients, being associated with body mass index and anxiety, suggesting that health professionals should pay attention to the comprehensive evaluation of patients with cardiovascular diseases. Important results emerged from this study, emphasizing the need to implement programs to improve the patients’ mental and physical health in both primary and specialized care services.
