Apical Root Canal Transportation and Remaining Dentin Thickness Associated with ProTaper Universal with and without PathFile

INTRODUCTION This study aimed to compare, using digital subtraction radiography, apical transportation in mesiobuccal root canals of extracted human maxillary molars instrumented with ProTaper Universal F3 and F4 files (Dentsply Maillefer, Ballaigues, Switzerland) with and without the prior creation of glide paths using the PathFile system and to compare, using cone-beam computed tomographic imaging, remaining dentin thickness (RDT) after complete preparation. METHODS Forty mesiobuccal roots with curvatures of 20°-35° were used. In group A, glide paths were created with PathFile #1, #2, and #3 to the working length; in group B, no glide paths were used. All canals were instrumented up to F4 to the working length. Digital subtraction radiographic images were obtained in buccolingual and mesiodistal directions. Apical transportation associated with F3 or F4 was calculated based on the difference between the tip of the first and the last instrument analyzed in each group. RDT was calculated based on cone-beam computed tomographic images. RESULTS Apical transportation was higher after F4 regardless of the use of the PathFile system. No statistically significant differences between groups A and B were observed in apical transportation after instrumentation with F3 or F4 (P > .05). Group A showed greater RDT values at 2 (buccal and mesial walls, P < .05) and 3 mm (buccal and distal walls, P < .05) from the apex but not at 1 mm. CONCLUSIONS The use of the PathFile system before root canal preparation with ProTaper Universal F3 and F4 did not influence apical transportation but was associated with greater RDTs at 2 and 3 mm from the apex.

Occurrence of Contralateral Lymph Neck Node Metastasis in Patients with Squamous Cell Carcinoma of the Oral Cavity

Introduction: Squamous cell carcinoma represents about 90% of malignancies of the mouth and about 38% of the head and neck tumors. The behavior of the cancer is very aggressive, presenting early cervical metastasis and, often contralateral ranging from 0.9 to 36%. Objectives: This study aims to analyze clinical and pathological factors that may influence metastasis of squamous cell carcinoma in cervical lymph nodes and relate this occurrence in the contralateral primary tumor, with disease prognosis and the interference of this type of metastasis in the survival rate of patients with this pathology. Material and Metohds: It was conducted a retrospective study from medical records of patients with Squamous Cell Carcinomas with homolateral lymph node metastasis and contralateral attended at the clinic of Head and neck surgery of Hospital São Vicente de Paulo in Passo Fundo – RS - Brazil, from 2000 to 2008. Results: Analyzing the charts of patients with metastatic and metastatic contralateral side it was observed that patients with initial stage presented a higher survival with statistical significance (p=0,035). Conclusions: The occurrence of metastases in lymph nodes of contralateral position to the primary lesion was not the main fator that influenced the survival of the group. Sinusitis Maxilar de Origen Odontogénica: Diagnóstico y Tratamiento Quirúrgico

RESUMEN: La sinusitis odontog”nica afecta la mucosa de los senos maxilares y puede ser causada por la evolu-ciSn de una lesiSn en el diente con formaciSn de pus periapical que avanza en el interior de la cavidad sinusal. Se presentaun caso de un paciente con dens in dente en el diente incisivo lateral superior derecho, que sufriS el proceso de cariesevolucionando en la regiSn˚ periapical,˚ contaminando la mucosa sinusal de este lado, lo que causS el dolor, abultamientoy el desplazamiento del diente. La cirug™a se realizS a trav”s de acceso de Caldwell-Luc para el curetaje de la lesiSn, lo queindica la presencia en el diagnSstico histopatolSgico la presencia de quiste˚ abscedado˚ y˚ sinusitis. Despu”s de 18 mesesde control, se observS la regeneraciSn Ssea˚ y el reposicionamiento alveolar espont⁄neo de los˚ dientes˚ implicados. Losautores advierten que c™ngulos profundos y dens in dente son˚ las enfermedades de los dientes que merecen atenciSn parael diagnSstico y la prevenciSn de los procesos de caries con el fin de evitar complicaciones mayores.˚

Peri-implant assessment via cone beam computed tomography and digital periapical radiography: an ex vivo study

OBJECTIVES: This research evaluated detail registration in peri-implant bone using two different cone beam computer tomography systems and a digital periapical radiograph. METHODS: Three different image acquisition protocols were established for each cone beam computer tomography apparatus, and three clinical situations were simulated in an ex vivo fresh pig mandible: buccal bone defect, peri-implant bone defect, and bone contact. Data were subjected to two analyses: quantitative and qualitative. The quantitative analyses involved a comparison of real specimen measures using a digital caliper in three regions of the preserved buccal bone – A, B and E (control group) – to cone beam computer tomography images obtained with different protocols (kp1, kp2, kp3, ip1, ip2, and ip3). In the qualitative analyses, the ability to register peri-implant details via tomography and digital periapical radiography was verified, as indicated by twelve evaluators. Data were analyzed with ANOVA and Tukey’s test (α=0.05). RESULTS: The quantitative assessment showed means statistically equal to those of the control group under the following conditions: buccal bone defect B and E with kp1 and ip1, peri-implant bone defect E with kp2 and kp3, and bone contact A with kp1, kp2, kp3, and ip2. Qualitatively, only bone contacts were significantly different among the assessments, and the p3 results differed from the p1 and p2 results. The other results were statistically equivalent. CONCLUSIONS: The registration of peri-implant details was influenced by the image acquisition protocol, although metal artifacts were produced in all situations. The evaluators preferred the Kodak 9000 3D cone beam computer tomography in most cases. The evaluators identified buccal bone defects better with cone beam computer tomography and identified peri-implant bone defects better with digital periapical radiography.

Association of Temporomandibular Dysfunction and Stress in University Students

El objetivo de este estudio fue evaluar la influencia de los factores emocionales en los trastornos temporomandibulares (TTM) en estudiantes universitarios. Fueron estudiados 696 estudiantes de ambos sexos, de la Universidad de Passo Fundo (UPF) - RS, que recibieron el Cuestionario de Fonseca, con el fin de verificar el nivel de TTM y la Escala de Reajuste Social (ERS), para evaluar el grado de nivel de estres, en dos momentos distintos, al principio y al final del semestre, tanto en estudiantes que ingresaron a la Universidad como en estudiantes que se graduan. Los datos fueron tabulados y analizados estadisticamente mediante la prueba de chi-cuadrado, con un nivel de significacion del 5%. Del total, 489 (70,25%) sujetos tenian algun grado de TTM, siendo el tipo leve de TTM el mas frecuente (309) en todas las areas del conocimiento, sin diferencia estadistica entre las areas estudiadas (p= 0,004). No hubo correlacion entre los TTM y el estres-ERS (r= 0,217). Existe un bajo porcentaje de las personas que estan libres de TTM. La correlacion entre los TTM y el estres era debil en todos los grupos estudiados, independientemente del area de conocimiento, periodo academico y grado escolar.
