Music Therapy as Part of Integrative Neonatology: 20 Years of Experience – 3 Case Reports and a Review

Background: Numerous varieties of interventions have decreased the mortality and morbidity of preterms. Due to the fact that more infants survive, developmental care has become a crucial topic. In this context, music therapy offers some interesting aspects for parents and children. Starting as an emotional approach, music therapy has developed towards a systematic therapeutic option in neonatal care. Objectives: To assess the relevance of music therapy for neonates. Methods: We present 3 typical case reports and give an overview of the current literature regarding music therapy in neonatology. Case Reports: Case 1: Focus on the child, especially the respiratory system. Case 2: Focus on care and assistance for the mother. Case 3: Focus on mother-father-child interaction. Discussion: These cases show the varieties of indications and possible effects of music therapy. We discuss our own experience ranging over 20 years and give a review on the literature concerning the different aspects of music therapy in neonates. Conclusion: Probably without any severe adverse effects, music therapy has positive effects on basic vital signs, the reduction of pain and on neurological development. Furthermore, it grants the parents’ wish to participate in the well-being of their child. Nevertheless, the need for a specific setting, its methods, and its positive as well as possible negative effects are still open issues and need scientific evaluation.

Systematic review on phytotherapy in neonatology.

Background: There is an enormous economical significance of phytotherapy at least in Germany, the huge demand of patients was repeatedly proven. Simultaneously data exist for different application areas and ages as for younger children and pregnancy until birth. The actual neonatal period can be of interest because phytotherapy is used in younger children and until birth, it is not very likely that there is a gap in this period of life. Aim: To give an overview on the current literature on phytotherapy in neonatology. Methods: A systematic review was performed in the following databases: Cochrane, EM-BASE Alert, AMED, NCCAM, NLM, DIMDI, CAMbase, BIOSIS Previews, DAHTA, SciSearch, and Medline as well as in the archives of GMS, Karger, Kluwer, Thieme, Krause and Pachernegg, Wiley-Interscience/Phytotherapy Research, Springer and Merkurstab publishers. Keywords were neonatology, newborn, preterm, phytotherapy, phytomedicine, and herbal drugs in clinical randomized controlled studies. Results: We found 14 articles that fulfilled the inclusion criteria: 10 case control studies, 1 observational study, 1 study with interview, 1 with a questionnaire, and 1 retrospective study. Topics were aromatherapy, external applications, and internal effects of herbal teas. Conclusion: Indications, study design and explantory power of these publications were quite heterogeneous. Most promising was the study situation in aromatherapy. Due to the fact that phytotherapy is frequently used and there is a high demand of parents, methodologically excellent studies are needed to evaluate the significance of phytotherapy in the neonatal period. The aim is to fulfill scientific as well as economical needs. This review shows that there is no such evidence yet.

The potential role of thyme in the treatment of pyodermia in newborn infants--a retrospective analysis.

Searches in the databases of Cochrane, EMBASE, NCCAM, NLM, DIMDI, CAMbase, and Medline and in the archives of Karger, Kluwer, Thieme, Springer, and the Merkurstab reveal only few specific data on skin infections in the neonatal period [1–3]. Knowledge about the treatment of these skin infections partly derives from the treatment of contagious impetigo of the older child [4–6]. The national and international research literature [7, 8] offers either only vague [9] or no recommendations at all [10]. A specific guideline for newborns does not exist [11]. On the other hand, there are numerous publications on the antimicrobial effect of thyme (Thymus vulgaris) [5, 12, 13], some of them representing popular medical opinions, others a long tradition of empirical use [14]. The disinfectant effect of thyme is supposed to be caused by essential oils, particularly thymol, carvacrol, and tannic acids. Based on traditional experience and the existing scientific literature, thyme has been used for many years as a concentrated tea for the treatment of superficial skin infections in the department of pediatrics, Gemeinschaftskrankenhaus Herdecke, [15, 16], usually supplementarily to systemic antibiotic therapy. The aim of this article is the systematic presentation of our therapeutic procedures for pyodermia in newborns. The focus lies on the documentation of desired and undesired effects as well as complications during the course of therapy. Methods

ESCOP geht online mit dem erweiterten Aufbau einer wissenschaftlichen Dokumentation pflanzlicher Arzneimittel Die European Scientific Cooperative on Phytotherapy (ESCOP) ist die Dachorganisation der nationalen Phytotherapie-Gesellschaften in Europa, mit Delegierten aus Österreich, Belgien, Schweiz, Deutschland, Dänemark, Spanien, Frankreich, Irland, Italien, den Niederlanden, Polen, Türkei und Großbritannien. Sie verfolgt seit über 20 Jahren das Ziel, die wissenschaftliche und regulatorische Anerkennung von pflanzlichen Arzneimitteln in Europa zu fördern. Das Scientific Committee besteht aus Experten der Pharmazie, Medizin, Biologie und Chemie. Es verfasst Monografien mit einer Kontinuität, die ihresgleichen sucht. Die Monografien bieten eine übersichtliche Darstellung der wissenschaftlichen und traditionellen Informationen über die sichere und wirksame Anwendung pflanzlicher Arzneimittel, welche von Herstellern, Gesundheitsbehörden, Universitäten und Wissenschaftlern häufig nachgefragt wird. Das Komitee bearbeitet und wählt aus der zum Teil immensen Anzahl an Publikationen für jede Arzneipflanze die relevanten Arbeiten sorgfältig aus und verwendet gerne auch Vorschläge von spezialisierten externen Experten [1]. Mittlerweile sind von der ESCOP 107 Monografien zu Arzneipflanzen in 2 Bänden [2, 3] publiziert worden. Die Gruppe der Supervising Editors der ESCOP gewährleistet eine redaktionelle Überarbeitung der Monografien, welche die hohe Qualität maßgeblich mitbestimmt. Ganz neu werden Online-Veröffentlichungen von neuen und aktualisierten Monografien in Angriff genommen. Der Vorteil ist, dass die Monografien einzeln erhältlich sind und nicht mehr gesammelt in Buchform erscheinen müssen. Die Monografien werden aktueller sein, als es in Buchform möglich wäre. Die erste Monografie der neuen Online-Reihe (www.escop.com) ist Cimicifugae rhizoma (Traubensilberkerzen-Wurzel). Sie kann nach Bezahlung herunter geladen werden [4]. Weitere 18 Online-Versionen sind in Arbeit. Folgende werden aktualisiert: Uvae ursi folium, Symphytii radix, Plantaginis lanceolatae folium/herba und Hamamelidis aqua. Inhaltlich wird es sich um «State-of-the-art»-Monografien handeln, so wie wir das von der ESCOP gewohnt sind. Die frühen ESCOP-Monografien wurden formell bei der European Medicines Agency (EMA) eingereicht, als Basis für europäische Community Herbal Monographs für die Zulassung bzw. Registrierung von «traditionellen» und «regulären» pflanzlichen Arzneimitteln. Nach der Gründung des Herbal Medicinal Product Commitee (HMPC) sieht es die ESCOP als wichtige Aufgabe, die Entwürfe zu HMPC-Monografien zu prüfen und Kommentare abzugeben, die auf den ESCOP-Monografien basieren.
