Accuracy of Impressions of Multiple Implants in the Edentulous Arch: A Systematic Review

PURPOSE Because there is a paucity of clear-cut evidence regarding which materials and techniques are most accurate for complete-arch, multiple-implant impressions, the current study sought to analyze the data and draw useful conclusions based on the evidence for application in clinical practice. MATERIALS AND METHODS Relevant studies published between 1990 and December 2012 were included in the review. The articles were located through PubMed and manually through reviewing references in the literature. Papers examining implant impression accuracy in completely edentulous arches (three or more implants) were included. Clinical case reports, technique articles, abstracts, and review papers were excluded. RESULTS One of the 34 studies selected for evaluation was clinical; the remaining 33 were in vitro investigations. Ten studies compared polyvinyl siloxane (PVS) and polyether (PE); eight found that these were statistically equal in terms of impression accuracy. The splint effect was examined by 24 studies; 10 failed to observe any differences between splinted and nonsplinted impressions, whereas 7 (> 25%) showed that the splinted technique was better than the nonsplinted technique. Thirteen studies investigated the differences between pickup and transfer impression techniques; six favored pickup over transfer, and five found insignificant differences between the techniques. The effect of nonparallel implants on edentulous multiple-implant impression accuracy was examined by only two studies. Significant differences in accuracy were observed for 15 degrees of angulation. CONCLUSION Most of the evidence supports PVS and PE as the most accurate impression materials for edentulous multiple-implant situations, with no clear advantage of either. Conflicting evidence exists regarding the most accurate impression technique (splinted/nonsplinted, pickup/transfer), and no clear recommendation can be made. Inadequate research exists regarding several other factors that might affect edentulous implant impression accuracy. There is a lack of clinical research to support in vitro findings.

Patient-Based and Clinical Outcomes of Implant Telescopic Attachment- Retained Mandibular Overdentures: A 1-Year Longitudinal Prospective Study

PURPOSE The purpose of this study was to evaluate and compare Oral Health-Related Quality of Life (OHRQoL), denture satisfaction, and masticatory performance in edentulous patients provided with mandibular implant-supported overdentures (ISODs) retained with telescopic attachments and those of conventional complete dentures (CCDs). Peri-implant soft tissue changes were also evaluated at various intervals during a 1-year observation period. MATERIALS AND METHODS Participating patients received new CCDs and later received two mandibular interforaminal implants and had their mandibular CCDs converted into ISODs with telescopic attachments. Questionnaires were used to assess OHRQoL (Shortened Oral Health Impact Profile-14, Malaysian version) and denture satisfaction at different stages of treatment with CCDs and ISODs. Objective masticatory performance with the CCDs and ISODs was recorded with a mixing ability test. Evaluations were carried out at 3 months with the new CCDs, 3 months after mandibular ISOD provision, and 1 year after receiving the ISOD. Peri-implant parameters were additionally assessed at specific intervals during the treatment period. The data obtained were statistically analyzed and compared. RESULTS In the 17 patients who completed the protocol, significant improvements were observed in OHRQoL and patient satisfaction when CCDs were modified to ISODs, after 3 months, and at 1 year. Significantly better mixing ability with the ISOD was noted, with the highest values observed at 1 year. Statistically insignificant differences were observed for all the peri-implant parameters, except for gingival recession, for which significant changes were observed 6 months after ISOD delivery (values had stabilized by 1 year). CONCLUSION Telescopic crown attachment-retained mandibular ISODs improved OHRQoL, dental prosthesis satisfaction, and masticatory performance compared to CCDs. Peri-implant soft tissue response and implant stability were found to be favorable after 1 year.

Treatment of a maxillary dento-alveolar defect using an immediately loaded definitive zygoma implant-retained prosthesis with 11-month follow-up: a clinical report

This article describes the dental implant-based rehabilitation of a partially edentulous patient with a unilateral maxillary dento-alveolar defect. A screw-retained prosthesis with a modified design was fabricated on zygomatic and regular dental implants. One section of the implant prosthesis has cemented crowns and the other section is conventional screw-retained. The design of the prosthesis overcame the hard and soft tissue deficit and provided the desired esthetics.

Full-Arch Metal-Resin Cement- and Screw-Retained Provisional Restoration for Immediately Loaded Implants

Abstract This article describes the clinical and laboratory procedures involved in the fabrication of laboratory-processed, provisional, screw-retained, implant-supported maxillary and mandibular fixed complete dentures incorporating a cast metal reinforcement for immediate loading of implants. Precise fit is achieved by intraoral luting of the cast frame to milled abutments. Effective splinting of all implants is attained by the metal substructure and retrievability is provided by the screw-retention of the prosthesis.

Fixed rehabilitation of patient with aggressive periodontitis using zygoma implants

Treatment of patients with aggressive periodontitis has always been a challenge to the clinician. Both young and old are known to be affected by this progressive destructive condition of the supporting dental structures. Although dental implants have been offered as a viable treatment alternative for such patients, additional procedures (like bone grafting) and delayed protocols have limited their usage. This case report describes the treatment of a young patient with aggressive periodontitis using a graftless implant solution. Zygoma implants in conjunction with conventional implants were used with immediate loading.

Metal-Ceramic Screw-Retained Implant Fixed Partial Denture With Intraoral Luted Framework to Improve Passive Fit

Passive fit of a long-span screw-retained implant prosthesis is an important criteria for the success of the restoration. This article describes a technique for fabricating a ceramometal implant fixed dental prosthesis (FDP) for a long-span partially edentulous situation by altering the conventional screw-retained design. The possibility of a passive fit is maximized by intraoral luting of the cast frame to milled abutments, and the potential framework distortion during fabrication is compensated to a major extent. Retrievability is ensured by screw retention of the prosthesis to the implants. Compared with conventional porcelain fused to metal screw-retained FDP, this prosthesis is relatively inexpensive to fabricate.

Zygomatic implant-retained fixed complete denture for an elderly patient.

Dental rehabilitation of a completely edentulous geriatric patient has always been a challenge to the clinician, especially in treating those with higher expectations and demands. Treatment duration and the amount of residual alveolar bone available are often important considerations when planning for dental implant-based fixed treatment for these patients. With the introduction of zygomatic implants, a graftless alternative solution has emerged for deficient maxillary bone with provision for immediate loading. This article describes the treatment of a completely edentulous elderly patient using zygomatic implants in conjunction with conventional implants. The implants were immediately loaded using a definitive acrylic resin fixed denture reinforced with a cast metal framework, to provide function and aesthetics.

The effect of bur preparation on the surface roughness and reline bond strength of urethane dimethacrylate denture base resin.

BACKGROUND The clinical success of relining depends on the ability of reline resin to bond to denture base. Surface preparations may influence reline bond strength of urethane-based dimethacrylate denture base resin. AIM To investigate the effect of bur preparation on the surface roughness (R a ) of eclipse denture base resin and its shear bond strength (SBS) to an intra-oral self-curing reline material. The mode of reline bonding failure was also examined. MATERIALS AND METHODS Twenty-four cylindrical Eclipse™ specimens were prepared and separated into three groups of eight specimens each. Two groups were subjected to mechanical preparation using standard and fine tungsten carbide (TC) burs and the third group (control) was left unprepared. The R a of all specimens was measured using a contact stylus profilometer. Subsequently, relining was done on the prepared surface and SBS testing was carried out a day later using a universal testing machine. RESULTS One-way ANOVA revealed significant differences (P<0.05) in R a and SBS values for all the groups. Post-hoc Tukeys HSD test showed significant differences (P<0.05) between all the groups in the R a values. For SBS also there were significant differences (P<0.05), except between standard bur and control. CONCLUSIONS 1) There was a statistically significant difference in the R a of Eclipse™ specimens prepared using different carbide burs (P<0.05). 2) There was a statistically significant difference in the relined SBS (P<0.05) when prepared using different burs, but the difference between the standard bur and the control group was not statistically significant.

Immediate placement and loading of implants in anterior maxilla using an altered screw-retained implant fixed prosthesis

This article describes the immediate placement and loading of implants in the aesthetic zone using an implant-retained, fixed prosthesis with a modified design. One section of the implant prosthesis has cemented crowns and the other section is the conventional screw-retained. This combined approach significantly offsets the unsuitable implant position, alignment or angulation, while ensuring the easy retrievability, repair and maintenance of the prosthesis at the same time.
