Inferior turbinate composite graft for repair of nasal septal perforation.

Background Repair of nasal septal perforations is one of the most challenging procedures in nasal surgery. The aim of this prospective clinical study was to determine the efficacy of using an inferior turbinate composite graft (ITCG) for the repair of nasal septal perforation. Methods Between 2009 and 2011, 27 consecutive patients with nasal septal perforation underwent endoscopy-assisted, endonasal septal perforation repair by using an ITCG alone or in combination with bipedicled mucosal advancement flap. Results Complete closure of the perforation was achieved in 24 of 27 (88.8%) patients, and incomplete closure was observed in 2 patients with medium-sized perforation and 1 patient with large perforation. Conclusion The ITCG technique provides three-layer repair of the defect under no tension in closure of small perforations located anteriorly, posterior perforations with mucosal atrophy, or previous unsuccessful surgical repair. In graft-depleted revision rhinoplasty cases with small-sized septal perforations, this technique provides a simple solution with autogenous grafts. In cases involving larger perforations, the ITCG technique can easily be combined with bipedicled flap and allows for more options to solve a challenging problem. Current data from this prospective study suggest that this surgical technique may be used in the repair of nasal septal perforation.

Skin Prick Test Reactivity in Patients with Chronic Eczematous External Otitis

Objectives To investigate the incidence of skin prick test (SPT) positivity in patients with eczematous external otitis. Methods Forty-six patients with eczematous external otitis and forty-four healthy volunteers were included in the study. All the patients were skin-tested by prick test. Reactions were assessed by the degree of redness and swelling and the size of the wheal produced. Results According to SPT positivity and total immunoglobulin E values, the difference between the study and the control groups was statistically significant (P<0.05). The most common skin reactions were against to mites and grasses in this study. Conclusion Eczematous external otitis is perhaps the most difficult to treat of all forms of external otitis because the provocative agents usually remain undiagnosed. Patients suffering from eczematous external otitis symptoms should be investigated for allergens and be informed for prevention of the causative agents. SPT might be performed in cases of prolonged or treatment-resistant external otitis.

Oxidative Stress and Antioxidant Status in Patients with Bell’s Palsy

Summary Background: Bell’s palsy (BP) is the most common acute mononeuropathy of unilateral facial paralysis. Immune, infective and ischaemic mechanisms are potential contributors to the development of BP, but the precise cause remains unclear. Recently, oxidative stress has been proposed as a risk factor of various idiopathic diseases. The aim of this study was to investigate the possible role of oxidative stress in patients with BP. Methods: Thirty-two patients with BP and 30 healthy controls were included in this study. Serum total oxidant status (TOS), total antioxidant status (TAS) and oxidative stress index (OSI) were measured by the Erel method. Results: Serum TOS activities and OSI values were significantly higher in patients with BP compared with the control group (P <0.003 for all comparisons), whereas there was no significant difference between the groups in terms of TAS levels (P >0.05). Conclusions: The data suggest that oxidative stress is increased in BP. These results of high oxidative stress in patients with BP may be helpful to clarify the etiopathogenesis of BP and contribute to improvement in the management or prevention of the disease.

Long-term voice results of injection with hyaluronic acid-dextranomere in unilateral vocal fold paralysis.

Abstract Conclusion: Subjective and objective voice evaluation of patients injected with hyaluronic acid-dextranomere (HA-D) in the long term was not different from their preoperative values. HA-D injection does not provide an increase in the voice quality in the long term. Objective: This study was conducted to evaluate the long-term voice outcomes of injection with HA-D in patients with unilateral vocal fold paralysis (UVFP). Methods: Seventeen patients with UVFP underwent injection of HA-D. The patient group was composed of 13 women and 4 men with an age range of 34–56 years. The voice samples of patients were obtained before the injection and after the longest available follow-up. Long-term results were evaluated subjectively by the GRBAS scale and objectively by acoustic analysis. Results: The follow-up period was between 6 and 42 months. Fundamental frequency, jitter, and noise-to-harmonics ratio were decreased in the postoperative period. Shimmer was nearly the same. An increase in the S score and a decrease in R, B, and A scores were observed. Neither the GRBAS scale parameters nor the acoustic analysis results were statistically significantly different before injection and after long-term follow-up.

A unique case of a postrhinoplasty epidermoid cyst.

The first case of a postrhinoplasty cyst lined with unusual stratified squamous epithelium, unlike the other published mucous ones, is described in this article. There are two different theories accounting for this complication: namely mucosal herniation and the free graft theory. Although the cyst was very close to the marginal rim incision, there was no connection between the cyst and vestibuler skin. Thus the free graft theory is more logical than the herniation theory to explain the development of this cyst.

An evaluation of peripheral arterial tonometry for the diagnosis of obstructive sleep apnea

Objective: Peripheral arterial tonometry (PAT) as a portable method of monitoring sleep quality is a relatively recent innovation. The aim was to compare the results of PAT and polysomnography (PSG) and to evaluate the role of PAT in diagnosing obstructive sleep apnea syndrome (OSAS). Methods: This study included adult patients who admitted to ENT clinic with OSAS complaints (excessive daytime sleepiness, snoring, and witnessed apnea), undergone sleep monitorization using PAT system (WatchPAT 200TM; Itamar Medical Ltd., Caesarea, Israel) and had single-blind, level 1 polysomnography at sleep laboratory of Chest Diseases Department. Results: For the two sleep monitoring sessions as conducted at different times: the apnea-hypopnea index (AHI) and respiratory disturbance index (RDI) values were strongly correlated between sessions (r=0.749, r=0.753; p<0.001). The oxygen desaturation index (ODI) values were very strongly correlated (r=0.861; p<0.001). When the AHI scores calculated using PAT for the patients enrolled in the trial were taken into consideration; 89.7% of the patients were correctly diagnosed with OSAS (AHI≥5); for RDI calculated (RDI≥5) using PAT, 100% of OSAS diagnoses were correct; for AHI values calculated with the PAT method, taking 15 as cut-off point, the sensitivity was found to be at an extremely high level of 96.1%. Conclusion: PAT and PSG values were highly correlated. This finding demonstrated that the reproducibility of the results obtained with PAT was also high. This study shows that PAT can be used as a screening test for OSAS and in a group of patients who are highly suspected for OSAS.

The Effects of Curcumin on Wound Healing in a Rat Model of Nasal Mucosal Trauma

We explored the effects of topical curcumin on the healing of nasal mucosal wounds. A total of 32 Sprague-Dawley Albino rats were randomized in equal numbers into four groups, and unilateral nasal wounds were created using an interdental brush. Group 1 (the sham-control group) contained untreated rats with traumatized right-side nasal cavities; Group 2 and 3 rats were similarly traumatized and treated with topical curcumin (5 and 10 mg/mL) dissolved in dimethyl sulfoxide daily for 7 days after trauma; Group 4 rats were treated with topical dimethyl sulfoxide only. All rats were decapitated on day 15 and the healing sites evaluated by blinded observers in terms of the presence of cellular hyperplasia, goblet cell hypertrophy and degeneration, leucocytic infiltration, ciliary loss and degeneration, edema, and vascular dilation. On histopathological evaluation, all of cellular hyperplasia, leukocytic infiltration, and edema were significantly reduced in Group 3 compared with Group 1 (p = 0.001, p = 0.004, and p = 0.008, resp.). Thus, curcumin reduced the inflammatory response and significantly accelerated wound healing.
