Huge Popliteal Arterial Aneurysms in Behçet's Syndrome: Is Ligation an Alternative Treatment?

Behçets syndrome is a multisystemic disease characterized by relapsing uveitis, oral and genital ulcerations, and vascular system involvement. The vascular involvement is seen as venous occlusion, arterial occlusion, and aneurysm formation in this disease, and the surgical treatment of a Behçets aneurysm has technical difficulties. In this report, we suggest that the huge popliteal artery aneurysm in Behçets syndrome can be treated by ligation of the popliteal artery. A 58-year-old male patient was admitted to our clinic because of an infrapopliteal great mass at the left leg. Color Doppler ultrasonography and arteriography revealed a 71 × 54 mm aneurysmal dilatation at the distal popliteal artery. Surgery did not reveal any suitable arterial formation for bypass to the distal area of the popliteal artery and tibial arteries. For this reason, we applied ligation of aneurysmal dilatation at the distal popliteal artery. The patient tolerated the operation well and had no signs of ischemia during the postoperative period. In conclusion, especially in aneurysm of arteries such as the popliteal artery, which has critical importance for maintaining distal perfusion, ligation may be a treatment method if there are no other alternatives.

Bilateral Extracranial Internal Carotid Artery Aneurysm: Original Image

A 63-year-old woman was admitted to our clinic with the complaint of dizziness. Doppler ultrasound and computerized tomography angiography scan demonstrated bilateral fusiform internal carotid artery aneurysms (Figure 1). Left internal carotid artery’s diameter was 21 mm and right internal carotid artery’s diameter was 11 mm. The patient refused endovascular stent grafting, therefore medical treatment was given including antiaggregants and antihypertensives.

Koroner bypass ameliyatı öncesi trimetazidin kullanımının oksidatif parametreler üzerine etkileri

Amac: Koroner arter bypass greft cerrahi (KABG)\sinde olusan miyokardiyal iskemi- reperfuzyon hasari uzerinde trimetazidin (TMZ) kullaniminin koruyucu etkisinin prospektif olarak arastirilmasi amaclandi. Yontemler: 2008-2009 yillari arasinda klinigimizde elektif sartlarda KABG ameliyatina alinan 35 hasta calismaya dahil edildi. Olgular randomize olarak iki gruba ayrildi. Olgularin 17\sine ameliyattan iki hafta oncesinden TMZ verildi, kontrol grubundaki 18 hastaya verilmedi. Hastalardan ameliyat oncesi, ameliyat esnasi ve ameliyat sonrasinda venoz ve kardiyopulmoner bypass baslarken ve sonrasi donemde koroner sinus kan ornekleri alindi. Calisma sonunda total oksidatif durum (TOS) ve total antioksidan kapasite (TAS) calisildi ve oksidatif stres indeksi (OSI) hesaplandi. Oksidatif parametreler uzerinde ameliyat stresi ve trimetazidin kullaniminin etkisi tekrarlayan olcumler varyans analizi ile incelendi. Bulgular: Trimetazidin kullaniminin total antioksidan kapasite, total oksidatif stres ve oksidatif stres indeksi uzerinde istatistiksel anlamli etkisi olmadigi tespit edildi (p>0,05).
