Porokeratosis ptychotropica: successful treatment with the dermatome.

In 1985, Helfman and Poulos described a reticulated porokeratosis as a special form of porokeratosis involving the groin, genitals, and thighs. Ten years later, Lucker and colleagues coined the term ‘‘porokeratosis ptychotropica,’’ describing a case of porokeratosis involving the gluteal area with a different morphology and severe pruritus. Stone and colleagues described a second case of porokeratosis ptychotropica and characterized it as ‘‘bilateral perianal inflammatory verrucous porokeratosis.’’ Few similar cases have been described so far; two of them also showed dermal amyloid deposits. Recently, we described a case of the inflammatory stage of disseminated superficial porokeratosis that emphasizes the heterogeneity of porokeratosis.

Erfolgreiche Therapie eines Jungen mit hyperplastischer und granulomatöser Gingivitis unter Verwendung von Fumarsäureestern

Ein ansonsten gesunder 13-jähriger Junge mit einer achtmonatigen Anamnese von moderater Schwellung der Oberlippe und einer auffälligen Zahnfleischhypertrophie wurde zu unserer ambulanten Klinik überwiesen (Abbildung 1). Da er ausdrücklich erklärte, keine gastrointestinalen Beschwerden zu haben, wurden keine weiteren invasiven Eingriffe durchgeführt. Die klinische Untersuchung der Zunge und der Gesichtsnerven war unauffällig. Eine lokale Infektion und mögliche Zahnbeteiligung wurden ausgeschlossen. Ein Röntgenbild des Kiefers ließ einen normalen Zahnund Knochenzustand erkennen. Die histopathologische Untersuchung des Zahnfleisches zeigte eine chronische granulomatöse Entzündung, welche die Diagnose der orofazialen Granulomatose (OFG) bestätigt (Abbildung 2a, b). Der Patient wurde zunächst oral mit täglich 1 mg/kg (50 mg) Prednisolon für eine Woche behandelt. Die Dosierung wurde anschließend in den Wochen 2 und 3 auf jeweils 0,5 und 0,25 mg/kg herabgesetzt. Erythema and erosions of the perianal region in a 49-year-old man

A 49-year-old man presented with an unclear erythema and erosions of the perianal region combined with severe pruritus, which had developed over several months. Proctologic examination had been performed and excluded any primary anal disease. The patient reported loose stools and used large amounts of toilet paper. Topical treatment with corticosteroids did not produce satisfactory improvement. The medical history for allergies was negative, but he had mild atopic eczema. He did not take any medication.
