Nexos e desafios na formação profissional do enfermeiro

The objective of the study was to know the linkages and challenges between professional training and practice and the Curriculum Guidelines for Nursing Baccalaureate Degree Programs. It was a qualitative study, with 34 graduates in nursing, who answered an e-mailed questionnaire. Data analysis was based on Michael Foucaults discourse analysis. The ethical aspects were observed. Analysis showed three themes in graduates discourses - Unified Health System, generalist professional training and humanization. The study revealed an approximation of concepts and guidelines of the Curriculum Guidelines for Nursing Baccalaureate Degree Programs and of the Unified Health System with the reality of professional practice along with challenges in the training of professional nurses.A investigacao teve como objetivo conhecer os nexos e desafios entre a formacao e a praxis profissional e as recomendacoes das Diretrizes Curriculares Nacionais do Curso de Graduacao em Enfermagem. Estudo qualitativo, com 34 diplomados em Enfermagem, que responderam ao questionario por e-mail. A analise dos dados foi baseada na analise de discurso de Michael Foucault. Os aspectos eticos foram respeitados. A analise apontou tres tematicas nos discursos dos diplomados - Sistema Unico de Saude, formacao generalista e humanizacao. O estudo revelou uma aproximacao das concepcoes e orientacoes das Diretrizes Curriculares e do SUS com a realidade da praxis profissional, bem como desafios para a formacao do enfermeiro.

Saberes e verdades acerca da enfermagem: discursos de alunos ingressantes

Trata-se de um estudo de abordagem qualitativa, com o qual se propoe analisar regimes de verdades que perpassam a profissao Enfermagem, manifestados por alunas(os) ingressantes no Curso de Graduacao, 2004/2, 2005/1 e 2005/2. Como opcao metodologica para a analise dos dados foi realizada a Analise de Discurso, utilizando-se referenciais de Michel Foucault. Nesse contexto de analise e discussao emergiram tres grandes tematicas: atravessamentos de saberes de genero na pratica da Enfermagem; a Enfermagem como saber hierarquizado; o fazer como poder, no cotidiano academico e profissional da Enfermagem. Evidenciaram-se concepcoes sociais que sao atravessadas por saberes invisiveis que legitimam praticas de saude e de enfermagem, tornando-as inquestionaveis.It is a study with a qualitative approach that proposes to analyze truths regimes that pass by and along Nursing as a profession, as manifested by students entering in the Graduate Course 2004/2, 2005/1 and 2005/2. The Discourse Analysis has been used as a methodological option for analyzing the data, under Michel Foucaults approaches. Three great themes have come out of such context of analysis and discussion: the traversing of gender knowledges in Nursing practice; Nursing as a knowledge made hierarchical; making as a might in Nursings academic and professional day-by-day. Social conceptions became self evident, going through invisible knowledges that legitimate health and nursing practices, making them unquestionable.

Validação Concorrente de Escores de Enfermagem (NEMS e TISS-28) em terapia intensiva pediátrica

OBJECTIVE: Examine the concurrent validity of the Nine Equivalents of Nursing Manpower Use Score (NEMS) in comparison to the Therapeutic Intervention Scoring System-28 (TISS-28) in a Pediatric Intensive Care Unit (PICU). METHODS: Prospective observational cohort study conducted in a PICU of a Brazilian university hospital over a period of two years with a sample of 816 patients. A total of 7,702 observations were obtained for each of the scores. RESULTS: The average maximum score of the NEMS was 26.6±9.2 and for the TISS-28 it was 21.3±8.2. The TISS-28 was lower than the NEMS (p<0.001) for all the averages. A good correlation was observed between them (r2=0.704) for all observations. Agreement between the TISS-28 and the NEMS was good, presenting only a 6.2% difference between the scores. CONCLUSION: The results show good correlation and agreement between the TISS-28 and the NEMS, enabling the NEMS validation in this population of pediatric patients.

Aplicação do índice de intervenção terapêutica em unidade de terapia intensiva pediátrica

Introduction: Indices of gravity aim to estimate the disease’s severity. The Therapeutic Intervention Scoring System – 28 (TISS-28) is used in Intensive Care Units as an indirect index to estimate the disease`s gravity interval, which has also been recently used to relate the nursing workload Objective: To review each operational item/definition of the Therapeutic Intervention Scoring System (TISS-28), making a re-reading in order to systematize the data collection practice in Pediatric Intensive Care Units (PICUs). Materials and Methods: An integrative revision research of medical and nursing literature through the base of data from Medical Literature Analysis and Retrieval System Online (MEDLINE) and Scientific Electronic Library Online (SciELO) using the terms Intensive Care Units, scales and Therapeutic Intervention Scoring System-28 or TISS-28. The articles were selected according to their relevance, as the authors opinion. Results: Preliminary studies show that the use of TISS-28 in ICU of adults patients, has helped in the evolutionary clinical assessment of the patient’s worsening, besides associations between death and elevated scores. The items/operational definitions evaluated in this score are reviewed in order to facilitate the interpretation of each item in the TISS -28 daily application in Pediatric Intensive Care Unit (PICU). Conclusion: The daily assessment of the 28 TISS variables allows the acquirement of an evolutionary profile of hospitalized children, which can help to increase the knowledge of the hospitalized child clinical picture and its prognosis.
