CARE OF CHILDREN WITH A CHRONIC CONDITION IN PRIMARY CARE: CHALLENGES TO THE HEALTHCARE MODEL

Estudo de abordagem qualitativa com o objetivo de analisar o modelo de atencao que orienta o cuidado a crianca em condicao cronica na atencao primaria a saude. Os sujeitos foram enfermeiros, tecnicos de enfermagem e auxiliares de enfermagem, atuantes em 16 unidades basicas de saude, no municipio de Belo Horizonte, Minas Gerais. Os dados foram coletados por meio de entrevista com roteiro semiestruturado e analisados a partir da perspectiva critica procurando-se identificar os temas comuns no conjunto do material empirico. Os dados permitiram apreender aspectos da organizacao dos profissionais das unidades basicas de saude para identificar e captar crianca em condicao cronica, alem de acoes de cuidado para essas criancas. DESAFIOS DA FORMAÇÃO DO ENFERMEIRO NO CONTEXTO DA EXPANSÃO DO ENSINO SUPERIOR a

Resumen This study aimed to identify aspects that challenge nursing education in a context of growth in participation in higher education. The research was conducted in 17 Bachelor of Nursing courses in the state of Minas Gerais with focus groups formed by professors and students. The results indicate changes in the profile of nursing students being admitted to the universities. In this context of growth nursing schools have contributed to the transformation of the models of care and to the incorporation of new concepts on health system. However there are problems to articulate the changes required in the nursing degree courses with the new profile of students that decide on a nursing course. It is necessary that the schools review their teaching project and the organization of the curriculum that should focus on care qualification and the transformation of the health care system.Resumen This study aimed to identify aspects that challenge nursing education in a context of growth in participation in higher education. Serviços de atenção domiciliar na saúde suplementar e a inserção da enfermagem em Belo Horizonte/MG

Objective: To evaluate modalities of home care services, discussing the inclusion of nursing. Methods: A qualitative case study conducted with four health plan providers in the city of Belo Horizonte / Minas Gerais (Brazil). Data were obtained from interviews and case management. Results: The home care services are grouped into three modalities: long-term care at home; temporary care at home; and, use of intensive technologies and care. In these modalities, nurses assume care actions, management of the care, and qualifying the care through the appropriate use of soft technologies. There is a trend towards outsourcing of nursing services to home care. Conclusion: The inclusion of the work of nurses in home care modalities represents both a component of cost reduction and contributes to the technological transition by establishing new forms of care at home.ReSuMo Objetivo: Analisar modalidades de servico de atencao domiciliar discutindo a insercao da enfermagem. Metodos: Estudo de caso qualitativo realizado em quatro operadoras de planos de saude no municipio de Belo Horizonte/Minas Gerais. Os dados foram obtidos de entrevistas e acompanhamento de casos. Resultados: Os servicos de atencao domiciliar sao agrupados em tres Modalidades: Cuidados prolongados no domicilio; Atencao provisoria no domicilio e Uso intenso de tecnologias e de cuidados. Nas modalidades, a enfermagem assume acoes assistenciais e de gestao do cuidado e qualifica a atencao por meio do uso apropriado das tecnologias leves. Ha uma tendencia a terceirizacao do servico de enfermagem na atencao domiciliar. Conclusao: A inclusao do trabalho do enfermeiro nas modalidades de atencao domiciliar representa ao mesmo tempo um componente de reducao de custos e contribui para a transicao tecnologica pelo estabelecimento de novas formas de cuidado no domicilio. METHODS : A qualitative study with data obtained from in-depth analysis that included participant observation, interviews with managers, coordinators, professionals and participants from 29 practices reported as successful for promoting health in six municipalities of the metropolitan region of Belo Horizonte, MG, Southeastern Brazil, in 2011. The variables of the study were concept, dimension, dissemination and ease of access, identified in practices guided by content analysis. RESULTS : The results indicate a conceptual and methodological uncertainty about health promotion as evidenced by conflicting objects and contradictory purposes. The practices differ in size, coverage and ease of access, determined by inter-sector coordination and political and financial investment. O direito à saúde: desafios revelados na atenção domiciliar na saúde suplementar

O objetivo do estudo foi analisar programas de atencao domiciliar na saude suplementar do ponto de vista do direito a saude, a partir de estudo de casos, de natureza qualitativa, desenvolvido em quatro operadoras de saude no municipio de Belo Horizonte, Minas Gerais. Os dados empiricos foram obtidos de entrevistas com gestores de operadoras e prestadoras que ofertam programas de atencao domiciliar. Os resultados sao discutidos a partir de tres eixos analiticos: a oferta da atencao domiciliar e os desafios da universalizacao do direito a saude; a relacao da saude suplementar com o sistema publico de saude e com as familias; e a judicializacao como caminho para se garantir o direito a saude no campo da atencao domiciliar na saude suplementar. A desregulamentacao da atencao domiciliar na saude suplementar foi revelada como um desafio para a garantia do direito a saude, prevalecendo a oferta segundo os interesses das operadoras que adotam estrategias para evitar a judicializacao, como, por exemplo, nao divulgar o beneficio. A suspensao da assistencia por decisao unilateral da operadora e a transferencia de responsabilidades para o sistema publico de saude e para as familias afrontam o direito dos usuarios de receber servicos na modalidade que melhor responda a determinadas situacoes de saude. Conclui-se que a atencao domiciliar na saude suplementar e permeada por tensoes que evidenciam a urgencia de maior regulamentacao no campo.
