Fistulotomy and Lay Open Technique

Fistulotomy cures fistulas, few arguing with its use for low anal fistulas where recurrence rates are of the order of 5 %. Actually, higher tracts can also be laid open with similar success. All fistulotomy carries a risk of minor incontinence (flatus and slight staining) in between a quarter and one third of patients. The trick is to avoid unintended worse incontinence through proper history taking, clinical evaluation and imaging and to properly counsel patients as to what to expect.
