Patient Care Indicator: Degenerative-Nondegenerative Patients and Compounded-Non-Compounded Prescription in One of Community Pharmacy in Bandung

Pharmacy is one of facilities for pharmacist to do a pharmaceutical care. It has been reported that 50% of patients failed to receive a treatment properly because of error in prescription and dispensing practices. Pharmacist has to be more involved in patient-oriented of pharmaceutical care. This study was conducted to evaluate pharmaceutical care in Apotek Pendidikan Universitas Padjadjaran (Unpad) Bandung from July–October in 2017, by assessing several factors in drugs dispensing using WHO patient indicators instrument. This cross-sectional study was conducted with a minimum sample size of 20 patients. The indicators that we used based on WHO assessment were average consultation time, dispensing time, percent of drugs that can be delivered, percent of labeling, and knowledge of dosage. The results showed that the average of consultation time was 99.03 s, average of dispensing time was 264 s, percentage of delivered drug was 97.18%, and 100% of correctly labeled drugs. Patients’ knowledge about their drug was only 17.07%, and consultation time varied from 10 to 370 s, compared with recommended time which is 60 s. Patients’ knowledge about dose was only 21.17%. Pharmaceutical care in Apotek Pendidikan Unpad Bandung could be assessed by WHO indicators and can be seen that patients’ knowledge of drug dose was influenced by type of disease which is degenerative and nondegenerative diseases, while time of drug administration and correct drug labeling was influenced by type of prescription of patients that is prescription of compounded medicine or non-compounded medicine. Pharmacists need to increase their service time so that patients can receive the drug quickly. Low patient’s knowledge is also expected to increase by drug information service and counseling.

PROTON PUMP INHIBITORS FOR STRESS ULCER BLEEDING PROPHYLAXIS IN CRITICALLY ILL PATIENTS: A COST ANALYSIS STUDY

Objective:Stress ulcer prophylaxis is generallyadministered for the prevention stress-related mucosal disease (SRMD) in critically ill patients. Proton Pump Inhibitors (PPIs) are most commonly prescribed in preventing bleeding from SRMD. Pantoprazole iv and omeprazole iv are the most effective, but clinically effective is not always efficient. Thisstudy aimed to investigate whether the most efficient PPIs for prophylaxis in ICU settings.Methods:An observational study was conducted on June 2014 with comparative design by using medical records in January 2012−November 2014 in a private hospital in Bandung City. Both retrospective and prospective data collection was performed in this study. Paired t-test analysis was used to compare average cost of the drugs with significant level p<0.05.Results:The results showed average cost of pantoprazole iv 458.142 IDR/patient and omeprazole iv 575.573 IDR/patient, there were significant differences of average drug cost between pantoprazole iv and omeprazole iv (p=0.0085).Conclusion:Both pantoprazole iv and omeprazole iv were effective but pantoprazole ivhad been found more efficient.

SATISFACTION LEVEL OF PATIENTS, PHYSICIANS, AND PRIVATE PRIMARY HEALTHCARE CENTER MANAGERS WITH CHRONIC DISEASE MANAGEMENT PROGRAM IN INDONESIA

Objective: Program PengelolaanPenyakitKronis (PROLANIS) or Chronic Disease Management Program (CDMP) is a healthcare system that was conducted by Healthcare and Social Security Agency in Indonesia to improve patients’ quality of life. The objective of this study was to analyze level satisfaction of patients, physicians, and private Primary Healthcare Centers (PHCs) with this program, focusing on hypertension care. Methods: This study was conducted in 7 private PHCs in Bandung City, Indonesia. A cross sectional study was performed to measure patient satisfaction with PSQ-18 on 143 PROLANIS patients with hypertension. A total number of 8 physicians and 7 private PHC managers were involved in this study. Level satisfaction of physicians and private PHC managers were observed by using in-depth interviews. Results: The results showed that patient satisfaction was estimated to be 68.52±8.54, which could be interpreted that patient satisfy with PROLANIS. In-depth interviews showed that physicians did not satisfy with PROLANIS due to several factors (e.g., unintegrated prescription systems, lack of medicines, uncompleted laboratory facilities, lack of physicians, unintegrated referral services, and lack of collaboration between primary and secondary healthcare system). On the other hand, private PHC managers were quite satisfied with PROLANIS due to its linearity with other activities in private PHCs. Conclusion: Level satisfaction of patients with PROLANIS is quite good, while physicians might not satisfy with this program. In particular, private PHC managers confirmed that they are satisfied with this program
