O quadrilátero da formação para a área da saúde: ensino, gestão, atenção e controle social

This article presents the concept of a four-way approach to training in the health field, based on teaching, management, care, and social control. Based on an experiment under way as a training policy for the Unified National Health System (SUS), the authors seek to formulate a toolbox theory allowing a critical analysis of the education that has been implemented in the Brazilian health sector and the establishment of challenging paths for training. The image of a four-way training approach fosters the development and organization of health education as an integral part of health system management, reshaping the image of health management and health care services and enhancing social control.O artigo apresenta o conceito de quadrilatero da formacao para a area da saude: ensino, gestao, atencao e controle social. Os autores buscam, a partir de uma pratica em experimentacao como politica de educacao para o Sistema Unico de Saude, formular uma teoria-caixa de ferramentas que permita a analise critica da educacao que temos feito no setor da saude e a construcao de caminhos desafiadores. A imagem do quadrilatero da formacao serve a construcao e organizacao de uma gestao da educacao na saude integrante da gestao do sistema de saude, redimensionando a imagem dos servicos como gestao e atencao em saude e valorizando o controle social.

Mudança na graduação das profissões de saúde sob o eixo da integralidade

This article discusses the issue of legal, technical, and political legitimacy for formulating a public policy aimed at organizing the training of health professionals in accordance with the populations health needs, while highlighting the competencies of the education and health sectors in this construction. Comprehensiveness is viewed as the main thrust for proposing and supporting the changes needed in the training of professionals in education and health, since it involves an expanded understanding of health, the linkage among multi-professional and interdisciplinary knowledge and practices, and dialogue/exchange with health system users, aimed at innovation of practices in all scenarios of health care and health sector management.

Educação e saúde: ensino e cidadania como travessia de fronteiras

The article seeks to build, based on a memory of the Brazilian Sanitary Reform and on approximations between the scientific areas of Education and Health, a microperception (matter for thought, learning, and getting to know) about the surfacing of a domain of knowledge designated by Health Education and Teaching. This emerging domain would seem to be largely associated to the invention of Collective Health, in the scientific field of health, and to the invention of Social Control in Health, in the field of political intervention in this sector. The new knowledge domain would be characterized by a singular implication of teaching with citizenship, allowing the bridging of the boundaries between education and health via permanent education in health. The teaching and citizenship themes are problematized with explicit help or not (via its readers) from a few philosophers and contemporaneous thinkers such as Michel Foucault, Michel Serres, Gilles Deleuze, Felix Guattari, Francisco Varela, Humberto Maturana, and Ilya Prigogine.

O artigo apresenta as responsabilidades previstas em lei e imputadas pela sociedade ao exercicio da conducao legal e legitima do setor saude, permitindo que se possa, no tocante a formacao e desenvolvimento dos recursos humanos em saude, avaliar o atendimento, omissoes e descumprimento daquilo que ja foi pensado, planejado e formulado para a educacao de profissionais de saude no Brasil pelo proprio Sistema Unico de Saude (SUS).

A emergência da educação e ensino da saúde: interseções e intersetorialidades

The article approaches the emergence of a knowledge domain as interface or intersection between the sectors of health and education in the public politics and between the areas of the scientific knowledge of the Education (of the Professionals of Health) and of the Public Health. This domain was designated by Health Education and Teaching, having its emergence demonstrated by the movements in the Education (change in the education of the health professionals), in the Health (health care reform) and in the society (constitution of guidelines of management for the work in the area of the health). A block of time between 1985 and 2007 can be perceived that established a historical and epistemic passage of evidence and vigour for this domain. It is concluded that the time has come, made by conceptual, politics and pragmatic accumulation, of conformation of a territory of potentialities for the knowing and the practices concerning specifically to the formation and the development of professionals of the health.

Acolhimento coletivo: um desafio instituinte de novas formas de produzir o cuidado

No desafio de implementar uma forma de acolhimento que integralizasse a equipe de trabalhadores de saude e estes com os usuarios, uma equipe de profissionais do programa de saude da familia propoe o acolhimento coletivo, um espaco de encontro entre os trabalhadores e usuarios, tendo por objeto as necessidades de saude destes. Neste espaco criador o trabalho vivo ganha forca na sua relacao com os atos normativos, e por meio de atos comunicacionais transforma tensionamentos em entendimentos. Ha a busca de um equilibrio metaestavel onde o trabalho se reconfigura diante de cada novo desafio, construindo relacoes mais solidarias e proporcionando aprendizado de novas formas de producao de cuidado.Within the challenge of implementing a form of welcome in which the team of healthcare workers would be made comprehensive, and would be thus in relation to users, a team of professionals from the family health program has proposed collective w elcoming. This is a meeting space between workers and users that is focused on their health n eeds. Within this creative space, active work becomes stronger in relation to normative acts and, through communicative acts, transforms tension into understandings. There is a search for a metast able balance in which work is reconstituted in the light of each new challenge, thereby building relat ionships of greater solidity and providing learning for new ways of producing care.

A formação e especialização de profissionais de saúde e a necessidade política de enfrentar as desigualdades sociais e regionais

Professional education and practice cannot follow parallel lines in the development of health systems; they need to be organically related. Medical education allows for services, conditions for recruiting and holding professionals, for the possibility of creating teams, the development and evaluation of care and assistance technologies and for building critical understanding and sensibilities. The net of health systems and services generates fields of practices, sceneries for intervention according to local demands, the scientific and technological bases, social inclusion and opportunities for understanding life. Once all this is brought together one can speak of being committed to relieving the regional and social inequalities, that is, only then we can establish conditions for facing the damage caused by poverty and social inequities, producing knowledge of scientific merit and social relevance and educate health professionals meeting the health demands of the population.

Imaginários da formação em saúde no Brasil e os horizontes da regulação em saúde suplementar

This paper is part of a research into the imaginaries present in the training of health professionals with respect to the regulation and exercise of their profession. The professions selected were medicine, odontology and psychology, for which emerging courses require evaluation by the National Health Council. The imaginaries were understood as operators of the virtual and the real with the potential of affirming or denying forms and contents in relation to being a professional or being in the profession. There was evidence for an imaginary of free exercise of the profession in the public sector on state level, where more experience with diseases and diversities of suffering would be gained. The ideal work place would be the private sector allowing for free choices for both professionals and users, but not without connection to the public sector for providing experience, study opportunities and the possibility of research scholarships and overseas training. Despite the expectations in relation to the private sector, currently there is no education in the field of supplementary care or about the meanings of the regulation proposed by the Brazilian Health System.

Sao Leopoldo, RS, Brasil. fernandobilibio@ uol.com.br This paper discusses healthcare practices relating to consultation offices in the street, which are a service delineated within the Brazilian National Health System that is directed towards caring for people living on the streets. The intention was to pose questions regarding healthcare and reception strategies, along with the guidelines or values of this work. These are often discordant with each other, like the programmed actions of tracking and moral authority over people living on the streets and the disruptive actions of the urban model for healthy and safe cities, in relation to strong inclusion of people who, for various reasons, live in such situation. Field diaries written by workers at one these consultation offices, located in Porto Alegre, Brazil, comprise an analysis resource. In these workers’ day-to-day routine, they pass through the streets and health and intersectoral networks with all their difficulties and strengths. Keywords: Consultation office in the street. Street dwellers.Este artigo discute praticas de cuidado do Consultorio de/na rua, servico que se delineia no Sistema Unico de Saude, destinado a atencao as pessoas em situacao de rua. A intencao e problematizar as estrategias de acolhimento e cuidado em saude, bem como as diretrizes ou valores desse trabalho, muitas vezes destoantes entre si, como: as acoes programadas de rastreamento e autoridade moral sobre pessoas com vida na rua e as acoes disruptivas do modelo urbano de cidade saudavel e segura, para uma forte inclusao de pessoas que, por variados motivos, levam esse tipo de vida. Como recurso de analise, estao diarios de campo escritos pelos trabalhadores de um desses consultorios – localizado em Porto Alegre, Brasil – que, em seu cotidiano, percorrem as ruas e redes de saude e intersetorial com todas as suas dificuldades e potencias.Este articulo discute practicas de cuidado del “Consultorio de/en la calle”, un servicio delineado en el Sistema Brasilleno de Salud destinado a la atencion de las personas que viven en la calle. La intencion es problematizar las estrategias de acogida y de cuidado de la salud, asi como las directrices o valores de este trabajo que muchas veces no concuerdan entre si, como las acciones programadas de rastreo y autoridad moral sobre personas que viven en la calle y las acciones disruptivas del modelo urbano de ciudad saludable y segura para una fuerte inclusion de personas que viven en esa situacion. Como recursos de analisis estan los diarios de campo escritos por los trabajadores de uno de estos consultorios, localizado en Porto Alegre, Brazil, que en su cotidiano recorren las calles y las redes de salud e intersectorial con todas sus dificultades y puntos fortes.
