AIM To investigate the discoloration potential of endodontic materials using a bovine tooth model. METHODOLOGY Two hundred and 10 dentine-enamel cuboid blocks (10 × 10 × 3.5 mm) were prepared out of the middle thirds of bovine tooth crowns. Standardized cavities were prepared in the walls of the pulp chamber leaving 2 mm of enamel and dentine on the labial wall of the crown. The specimens were randomly assigned to 14 groups (n = 15). Endodontic materials were placed into the cavities as follows: group A: empty, group B: blood, group C: calcium hydroxide, group D: ApexCal, group E: Ultracal XS, group F: Ledermix, group G: triple antibiotic paste (3Mix), group H: grey MTA(GMTA), group I: GMTA + blood, group J: white MTA (WMTA), group K: WMTA + blood, group L: Portland cement (PC), group M: PC + blood and group N: AH Plus. The cavities were sealed with composite and stored in water. Standardized colour measurement (VITA Easyshade compact) was performed at the following intervals: prior to (T0) and after placement of the filling (T1), 1 week (T2), 1 month (T3), 3 months (T4), 6 months (T5) and after 1 year (T6). Colour change (ΔE) values were calculated. A two-way analysis of variance was used to assess significant differences between the endodontic materials. The mean values of all groups were compared using the Tukey multiple comparison test (α = 0.05). RESULTS Significant differences were detected amongst the experimental groups after 12 months (P < 0.0001). The lowest colour change values were observed in the groups N (AH Plus, 3.2 ± 1.5), A (empty, 3.8 ± 1.4), L (PC, 4.1 ± 1.7), C (calcium hydroxide, 4.7 ± 1.5), E (Ultracal XS, 5.1 ± 1.9) and J (WMTA, 7.9 ± 6.7). The most discoloration was measured in groups G (3Mix, 66.2 ± 9.9) and F (Ledermix, 46.2 ± 11.6). PC showed the best colour stability amongst the Portland cement-based materials; however, when contaminated with blood (group M), a significantly higher ΔE value (13.6 ± 4.2) was detected (P = 0.032). CONCLUSION Materials used in endodontics may stain teeth. Therefore, the choice of material should not rely solely on biological and functional criteria, but also take aesthetic considerations into account.

This study was designed to determine the efficiency of hand and rotary instruments in shaping oval root canals. Seventy-five oval canals were equally divided into three groups. The apical third was prepared with rotary LightSpeed instruments either to size 52.5 (mandibular incisors) or to size 57.5 (distal root of mandibular molars). The middle third with an oval cross-section was shaped with Hedström hand files using circumferential technique, with 6% taper rotary Hero files in a circumferential filing movement or with rotary LightSpeed instruments in a step-back technique. Instrumentation was performed under clinical conditions in a phantom head. The teeth were sectioned at two levels in the middle third of the root. An assembly technique allowed comparing the canal outline before and after instrumentation. The photographed root sections were superimposed and traced under a stereomicroscope. The ratio of prepared to unprepared canal outline was calculated for each section. The lowest values were observed in the LightSpeed group (mean: 0.42; 95% confidence interval (CI): 0.37; 0.47). Significantly higher values were recorded in the Hero group (mean: 0.58; 95% CI: 0.53; 0.64) and in the Hedström group (mean: 0.56; 95% CI: 0.49; 0.62). No instrumentation technique was capable of completely preparing dentin walls of oval root canals. Circumferential filing of the middle third of oval root canals with either 6% taper Hero files or conventional Hedström hand files gave comparable results.

Prosthodontists face the difficult task of judging the influence and significance of multiple risk factors of periodontal, endodontic, or prosthetic origin that can affect the prognosis of an abutment tooth. The purpose of this review is to summarize the critical factors involved in deciding whether a questionable tooth should be treated and maintained, or extracted and possibly replaced by dental implants. A MEDLINE (PubMed) search of the English, peer-reviewed literature published from 1966 to August 2009 was conducted using different keyword combinations including treatment planning, in addition to decision making, periodontics, endodontics, dental implants, or prosthodontics. Further, bibliographies of all relevant papers and previous review articles were hand searched. Tooth maintenance and the acceptance of risks are suitable when: the tooth is not extensively diseased; the tooth has a high strategic value, particularly in patients with implant contraindications; the tooth is located in an intact arch; and the preservation of gingival structures is paramount. When complete-mouth restorations are planned, the strategic use of dental implants and smaller units (short-span fixed dental prostheses), either tooth- or implant-supported, as well as natural tooth abutments with good prognoses for long-span FDPs, is recommended to minimize the risk of failure of the entire restoration.

AIM To investigate the use of cone beam computed tomography (CBCT) in assessing furcation involvement (FI) and concomitant treatment decisions in maxillary molars. MATERIAL AND METHODS Twelve patients with generalized chronic periodontitis were consecutively recruited and CBCT was performed in maxillary molars (n=22) with clinical FI and increased probing pocket depths. CBCT images were analysed and FI, root length supported by bone and anatomical features were evaluated. FI and treatment recommendations based on clinical examinations and periapical radiographs were compared with data derived from CBCT images. RESULTS The estimated degree of FI based on clinical findings was confirmed in 27% of the sites, while 29% were overestimated and 44% revealed an underestimation according to CBCT analyses. Among degree I FI, 25% were underestimated, among degree II and II-III, the underestimation was as high as 75%, while all sites with degree III FI were confirmed in the CBCT. Discrepancies between clinically and CBCT-based therapeutic treatment approaches were found in 59-82% of the teeth, depending on whether the less invasive or the most invasive treatment recommendation was selected for comparison. CONCLUSIONS CBCT images of maxillary molars may provide detailed information of FI and a reliable basis for treatment decision.

AIM To assess the accuracy of cone beam computed tomography (CBCT) in detecting furcation involvement (FI) in maxillary molars. MATERIAL AND METHODS Fourteen patients with generalized advanced chronic periodontitis were consecutively recruited and treated non-surgically. In maxillary molars considered for furcation surgery due to increased FI and/or increased probing pocket depths during re-evaluation, CBCT was performed and the degree of FI was evaluated from the CBCT images. Furcation surgery was performed in 25 maxillary molars. Intra-surgical FI assessments were compared with data derived from CBCT images. RESULTS Overall, 84% of the CBCT data were confirmed by the intra-surgical findings (weighted kappa=0.926, 95% confidence interval: 0.681-1.0). While 14.7% (11 sites) were underestimated (CBCT less than intra-surgical value), in only 1.3% (one site) did the CBCT data lead to an overestimation compared with the intra-surgical analysis. The agreement between both assessments was the highest in distopalatal furcation entrances, followed by buccal and mesiopalatal. CONCLUSIONS CBCT images demonstrate a high accuracy in assessing the loss of periodontal tissue and classifying the degree of FI in maxillary molars.

The effects of different growth conditions on the susceptibility of five taxa to alkaline stress were investigated. Enterococcus faecalis ATCC 29212, Streptococcus sobrinus OMZ 176, Candida albicans ATCC 90028, Actinomyces naeslundii ATCC 12104, and Fusobacterium nucleatum ATCC 10953 were grown as planktonic cells, allowed to adhere to dentin for 24 hours, grown as monospecies or multispecies biofilms on dentin under anaerobic conditions with a serum-enriched nutrient supply at 37 degrees C for 5 days. In addition, suspended biofilm microorganisms and 5-day old planktonic multispecies cultures were used. Microbial recovery upon direct exposure to saturated calcium hydroxide solution (pH 12.5) for 10 and 100 minutes was compared with control exposure to physiologic saline. Planktonic microorganisms were most susceptible; only E. faecalis and C. albicans survived in saturated solution for 10 minutes, the latter also for 100 minutes. Dentin adhesion was the major factor in improving the resistance of E. faecalis and A. naeslundii to calcium hydroxide, whereas the multispecies context in a biofilm was the major factor in promoting resistance of S. sobrinus to the disinfectant. In contrast, the C. albicans response to calcium hydroxide was not influenced by the growth condition. Adherence to dentin and interspecies interactions in a biofilm appear to differentially affect the sensitivity of microbial species to calcium hydroxide.

AIM To compare the preparation quality of two rotary systems and NiTi-hand files in oval root canals, and to evaluate the effect of canal dimensions on the preparation. METHODOLOGY Ninety roots with oval root canals were selected. The middle third was cross sectioned at two levels and photographed. The maximum and minimum diameters of the root and canal were recorded. Teeth were distributed in three groups (n = 30) using stratified randomization, and prepared under simulated clinical conditions with Mtwo, ProTaper, or NiTi-hand files. The pre- and post-preparation photographs were traced and superimposed, the thickness of dentine removed was measured and the ratio of prepared canal outline was calculated. The impact of preparation system and canal dimensions on the quality of the preparation was evaluated using regression analysis. RESULTS With regards to the ratio of prepared canal outline, no statistical significant difference was found between Mtwo (0.75 [95%CI: 0.69; 0.81]) and ProTaper (0.75 [95%CI: 0.69; 0.80]), but both systems performed significantly better than NiTi-hand files (0.65 [95%CI: 0.60; 0.71]). In six root canals in Mtwo-group (20%), and eight root canals in ProTaper-group (27%), the minimal thickness of dentine-wall after preparation was less than 0.5 mm. In contrast to the maximum diameter of the root canal, the minimum diameter influenced the quality of the preparation (P = 0.0006). CONCLUSIONS No instrumentation technique was able to circumferentially prepare the oval outline of root canals. Nevertheless, instruments with greater taper (ProTaper and Mtwo) were more efficient than NiTi- hand files, but this was, in some cases, at the expense of remaining dentine-wall thickness.

AIM To investigate (i) the impact of post fit (form-congruence) and (ii) the influence of post length on the fracture resistance of severely damaged root filled extracted teeth. METHODOLOGY Ninety-six single-rooted human teeth were root filled and divided into four groups (n = 24 per group). Post spaces were prepared with a depth of 6 mm (group 1, 3) and 3 mm (group 2, 4). Form-congruence with a maximal fit of the post within the root canal space was obtained in groups 1 and 2, whereas there was no form-congruence in groups 3 and 4. In all groups, glass fibre reinforced composite (FRC) posts were adhesively cemented and direct composite crown build-ups were fabricated without a ferrule. After thermo-mechanical loading (1200000x, 5-50 degrees C), static load was applied until failure. Loads-to-failure [in N] were compared amongst the groups. RESULTS Post fit did not have a significant influence on fracture resistance, irrespective of the post length. Both groups with post insertion depths of 6 mm resulted in significantly higher mean failure loads (group 1, 394 N; group 3, 408 N) than the groups with post space preparation of 3 mm (group 2, 275 N; group 4, 237 N). CONCLUSIONS Within the limitations of this study, the fracture resistance of teeth restored with FRC posts and direct resin composite crowns without ferrules was not influenced by post fit within the root canal. These results imply that excessive post space preparation aimed at producing an optimal circumferential post fit is not required to improve fracture resistance of roots.

The Swiss Health Surveys are conducted every 5 yr, and alternate surveys contain information on oral health. Worldwide the population is ageing and oral health is improving. The aim of this study was to identify if these trends are continuing in a relatively affluent society with low levels of edentulousness. Participants in the 1992 and 2002 surveys completed a written questionnaire including items on oral health (response rates 75% and 86% respectively). Data were weighted and bivariate analyses were performed to calculate the average number of missing teeth and the prevalence of different prosthetic dental restorations for each cohort. Over the 10-yr period the proportion of subjects retaining all natural teeth increased, and the mean number of teeth increased, on average, by 1.3. Among those who required prosthetic dental restorations, fixed restorations increased and complete denture use was reduced. Strikingly, 4.4% of this sample reported having oral implants in 2002. Greater numbers of missing teeth and a higher prevalence of use of removable prostheses were still seen in women, in those who had only completed compulsory education, in subjects from families with low income, in those who smoke, and in those who were overweight, in 2002.

Fifty-eight extracted incisors were artificially stained to compare the efficacy of sodium perborate preparations used for intracoronal bleaching. All teeth were bleached for a 6-day period and the bleaching pastes replaced at days 1 and 3 (walking bleach technique). Sodium perborate-monohydrate, trihydrate, or tetrahydrate in conjunction with 30% H2O2 as well as tetrahydrate, either mixed with H2O or prepared as an experimental gel, were placed intracoronally at a level of 1-mm below the vestibular cementoenamel junction. Standardized slides were used to evaluate the color changes during bleaching. Success rates between 46 and 77% could be achieved, but no significant differences in final bleaching results between any of the sodium perborate types were observed. The use of the experimental gel resulted in comparable esthetic results (54%), although the portion of tetrahydrate in the gel was lower than that of the other preparations. In contrast to general recommendations that bleaching agents be left for 3 to 7 days in the access cavity before replacement, shorter bleaching intervals did not seem to affect the success.
