Echo intensity is associated with skeletal muscle power and cardiovascular performance in elderly men

The purpose of the present study was to investigate the relationship between echo intensity, neuromuscular and cardiorespiratory performances in the elderly. Thirty-one healthy elderly men (65.5±5.0) participated in this study. Echo intensity of rectus femoris and quadriceps femoris muscle thicknesses was determined by ultrasound images. Lower-body isometric and isokinetic peak torques (60, 180 and 360°(.s-1)), as well as rate of force development were evaluated as strength parameters. In addition, torque per unit of muscle mass was evaluated by the quotient between isometric peak torque of the knee extensors and the quadriceps femoris muscle thickness. The peak oxygen uptake (VO(2peak)), maximum aerobic workload (W(máx)), absolute (VT(1) and VT(2)) ventilatory thresholds, as well as workloads at VT(1) and VT(2) (W(VT1) and W(VT2)) were evaluated during a maximal incremental test on a cycle ergometer. There were significant negative correlations between the individual values of echo intensity with the corresponding individual values of isometric and isokinetic peak torques (60, 180 and 360°(.s-1)) (r=-0.48 to r=-0.64; P<0.05), as well as with W(VT1) (r=-0.46) and W(VT2) (r=-0.50) (P<0.05). In addition, significant positive correlations were observed between torque per unit of muscle mass and cardiovascular parameters (r=0.52 to r=0.60; P<0.001). The present results suggest that the echo intensity analysis using computer-aided gray-scale analysis is a low cost, easily accessible, and a safe method to evaluate the muscle quality, and may contribute to the research of neuromuscular and cardiovascular performances in the elderly.

Physiological effects of concurrent training in elderly men.

The aim of the present study was to investigate the effects of concurrent strength and endurance training on neuromuscular and hormonal parameters in elderly men. 23 healthy men (65±4 years) were randomly assigned to 1 of 3 groups: concurrent (CG, n=8), strength (SG, n=8) or endurance group (EG, n=7). The programs consisted, of strength training, endurance training on a cycle ergometer or a combination of both in the same session 3 times per week over a duration of 12 weeks. Subjects were evaluated on parameters related to muscle strength, muscle activation and serum hormones. There were significant increases in lower-body strength in all groups (P<0.05), with higher increases in SG (67%) than CG (41%) and both were higher than EG (25%) (p<0.01). Only SG and CG increased upper-body strength (p<0.01), with no significant difference between the 2 groups. Furthermore, there were significant decreases in free testosterone in EG after training. Significant increases in isometric strength and maximal muscle activation (p<0.05) as well as decreases in the submaximal muscle activation to the same load, were only seen in SG (p<0.05). The present results suggest that the interference effect observed due to concurrent strength and endurance training could be related to impairment of neural adaptations.

Strength prior to endurance intra-session exercise sequence optimizes neuromuscular and cardiovascular gains in elderly men

This study investigated the effects of different intra-session exercise sequences in the cardiovascular and neuromuscular adaptations induced by concurrent training in elderly. Twenty-six healthy elderly men (64.7±4.1years), were randomly placed into two concurrent training groups: strength training prior to (SE, n=13) or after (ES, n=13) endurance training. Subjects trained strength and endurance training 3 times per week performing both exercise types in the same training session. The peak oxygen uptake (VO(2peak)), maximum aerobic workload (W(máx)), absolute (VT(1) and VT(2)) and relative (VT(1)% and VT(2)%) ventilatory thresholds, as well as workloads at VT(1) and VT(2) (W(VT1) and W(VT2)) were evaluated during a maximal incremental test on a cycle ergometer before and after the training. In addition, muscle quality (MQ) was evaluated by the quotient between maximal dynamic strength (one repetition maximum test) of the knee extensors and the quadriceps femoris muscle thickness determined by ultrasonography. There were no modifications after training in the VT(1), VT(2), VT(1)%, and VT(2)%. There was significant increase in the W(VT1) only in SE (P<0.05), as well as significant increase in the W(VT2) in both groups (P<0.001). There was significant increase in the VO(2peak), with both groups showing increases (P<0.001), with no difference between groups; as well significant increase in the W(máx) (P<0.001) with no difference between SE and ES. The force per unit of muscle mass of knee extensors increased in both groups (P<0.001), but the increase was significantly higher in SE than in ES (27.5±12.7 vs. 15.2±10.3%, P<0.02). Hence, the intra-session exercise sequence had no influence in the maximal endurance power adaptations to concurrent training, but had influence in the magnitude of the muscle quality enhancements.

Strength and Endurance Training Prescription in Healthy and Frail Elderly

Aging is associated with declines in the neuromuscular and cardiovascular systems, resulting in an impaired capacity to perform daily activities. Frailty is an age-associated biological syndrome characterized by decreases in the biological functional reserve and resistance to stressors due to changes in several physiological systems, which puts older individuals at special risk of disability. To counteract the neuromuscular and cardiovascular declines associated with aging, as well as to prevent and treat the frailty syndrome, the strength and endurance training seems to be an effective strategy to improve muscle hypertrophy, strength and power output, as well as endurance performance. The first purpose of this review was discuss the neuromuscular adaptations to strength training, as well as the cardiovascular adaptations to endurance training in healthy and frail elderly subjects. In addition, the second purpose of this study was investigate the concurrent training adaptations in the elderly. Based on the results found, the combination of strength and endurance training (i.e., concurrent training) performed at moderate volume and moderate to high intensity in elderly populations is the most effective way to improve both neuromuscular and cardiorespiratory functions. Moreover, exercise interventions that include muscle power training should be prescribed to frail elderly in order to improve the overall physical status of this population and prevent disability.

Effects of Strength, Endurance, and Concurrent Training on Aerobic Power and Dynamic Neuromuscular Economy in Elderly Men

Twenty-three healthy men (65 ± 4 years) were divided into 3 groups: concurrent (CG, n = 8), strength (SG, n = 8), and aerobic training group (EG, n = 7). Each group trained 3 times a week for 12 weeks, strength training, aerobic training, or both types of training in the same session. The maximum aerobic workload (Wmax) and peak oxygen uptake (&OV0312;O2peak) of the subjects were evaluated on a cycle ergometer before and after the training period. Moreover, during the maximal test, muscle activation was measured at each intensity by means of electromyographic signals from the vastus lateralis (VL), rectus femoris (RF), biceps femoris long head, and gastrocnemius lateralis to determine the dynamic neuromuscular economy. After training, significant increases in &OV0312;O2peak and Wmax were only found in the CG and EG (p < 0.05), with no difference between groups. Moreover, there was a significant decrease in myoelectric activity of the RF muscle at 50 (EG), 75 and 100 W (EG and CG) and in the VL for the 3 groups at 100 W (p < 0.05). No change was seen in the electrical signal from the lateral gastrocnemius muscle and biceps femoris. The results suggest specificity in adaptations investigated in elderly subjects, because the most marked changes in the neuromuscular economy occurred in the aerobically trained groups.

Echo intensity is negatively associated with functional capacity in older women

Muscle quality is an important component of the functional profile of the elderly, and previous studies have shown that both muscle quantity and quality independently contribute to muscle strength of the elderly. This study aimed to verify the association between quadriceps femoris muscle quality, analyzed by specific tension and echo intensity (EI), and rate of torque development (RTD) of the knee extensor muscles with the functional performance in elderly active women. Forty-five healthy, active elderly women (70.28 ± 6.2) volunteered to participate in this study. Quadriceps femoris muscle thickness and EI were determined by ultrasonography. Knee extension isometric peak torque and RTD were obtained from maximal isometric voluntary contraction curves. The 30-s sit-to-stand-up (30SS) test and usual gait speed (UGS) test were applied to evaluate functional performance. Rectus femoris EI presented a significant negative correlation with 30SS (r = −0.505, P < 0.01), UGS (rs = −0.347, P < 0.05), and isometric peak torque (r = −0.314, P < 0.05). The quadriceps femoris EI correlated negatively with 30SS (r = −0.493, P < 0.01) and isometric peak torque (r = −0.409, P < 0.01). The EI of the quadriceps femoris and all quadriceps muscle portions significantly correlated with RTD. RTD significantly correlated with physical performance in both functional tests (30SS = r = 0.340, P < 0.05; UGS = rs = 0.371, P < 0.05). We concluded that muscle EI may be an important predictor of functional performance and knee extensor power capacity in elderly, active women.

Low- and high-volume strength training induces similar neuromuscular improvements in muscle quality in elderly women.

The aim of this study was to compare the effects of low- and high-volume strength training on strength, muscle activation and muscle thickness (MT) of the lower- and upper-body, and on muscle quality (MQ) of the lower-body in older women. Twenty apparently healthy elderly women were randomly assigned into two groups: low-volume (LV, n=11) and high-volume (HV, n=9). The LV group performed one-set of each exercise, while the HV group performed three-sets of each exercise, twice weekly for 13 weeks. MQ was measured by echo intensity obtained by ultrasonography (MQEI), strength per unit of muscle mass (MQST), and strength per unit of muscle mass adjusted with an allometric scale (MQAS). Following training, there was a significant increase (p≤0.001) in knee extension 1-RM (31.8±20.5% for LV and 38.3±7.3% for HV) and in elbow flexion 1-RM (25.1±9.5% for LV and 26.6±8.9% for HV) and in isometric maximal strength of the lower-body (p≤0.05) and upper-body (p≤0.001), with no difference between groups. The maximal electromyographic activation for both groups increased significantly (p≤0.05) in the vastus medialis and biceps brachii, with no difference between groups. All MT measurements of the lower- and upper-body increased similarly in both groups (p≤0.001). Similar improvements were also observed in MQEI (p≤0.01), MQST, and MQAS (p≤0.001) for both groups. These results demonstrate that low- and high-volume strength training promote similar increases in neuromuscular adaptations of the lower- and upper-body, and in MQ of the lower-body in elderly women.

However, there has been little investigation into the relationship between muscular strength and economy of movement during aerobic exercise in elderly subjects. Thus, the purpose of this study was to investigate the possible relationship between strength, aerobic performance, and neuromuscular economy in older men. Twenty-eight aged men (65 ± 4 years old) were evaluated in dynamic (1 repetition maximum test), isometric strength (maximal voluntary contraction), and rate of force development. Peak oxygen uptake, maximal workload, and ventilatory threshold were determined during a ramp protocol on a cycle ergometer. Throughout the same protocol, the neuromuscular economy (electromyographic signal) of the vastus lateralis was measured. Significant correlations were found between muscular strength, cardiorespiratory fitness, and neuromuscular economy (r = 0.43-0.64, p < 0.05). Our results suggest that cardiorespiratory capacity and economy of movement are associated with muscular strength during aging.

Hormonal responses to concurrent strength and endurance training with different exercise orders.

J Strength Cond Res 26(12): 3281–3288, 2012—The purpose of this study was to examine the influence of the intrasession sequencing of concurrent strength and aerobic training on the acute testosterone (TT) and cortisol (COR) responses. Ten recreationally strength-trained young men (23.5 ± 0.9 years) performed 2 exercise interventions: aerobic-strength (AS) and strength-aerobic (SA), which consisted of 30 minutes of aerobic exercise on a cycle ergometer at 75% of maximal heart rate and 3 sets of 8 repetitions at 75% of 1 repetition maximum (1RM) in 4 strength exercises. Maximal heart rate was determined using a maximal incremental test on a cycle ergometer. Blood samples were collected before, between exercise modalities, and immediately after the concurrent training sessions to determine basal and acute total TT and COR concentrations. There were significant increases in TT after the first modality in both exercise orders (p < 0.05). However, the TT level remained significantly higher than the resting levels after the second exercise modality only in the AS (p < 0.05) which resulted in a significant higher relative total change after the complete concurrent training session compared with SA (p < 0.05). Regarding COR, there were significant increases after the first modality in both AS and SA orders (p < 0.05), but the COR returned to resting levels after the second modality in both AS and SA interventions. During AS and SA, the change observed after the first modality performance was greater than that after the second in both hormones. The present results suggest that the TT response is optimized after the AS order, whereas both AS and SA produced similar hormonal levels at all time points. However, it is important to state that the present results should be applied only when short duration and moderate intensity aerobic training is performed.

3 Different Types of Strength Training in Older Women

The objective of the present study was to evaluate and compare the neuromuscular, morphological and functional adaptations of older women subjected to 3 different types of strength training. 58, healthy women (67 ± 5 year) were randomized to experimental (EG, n=41) and control groups (CG, n=17) during the first 6 weeks when the EG group performed traditional resistance exercise for the lower extremity. Afterwards, EG was divided into three specific strength training groups; a traditional group (TG, n=14), a power group (PG, n=13) that performed the concentric phase of contraction at high speed and a rapid strength group (RG, n=14) that performed a lateral box jump exercise emphasizing the stretch-shortening-cycle (SSC). Subjects trained 2 days per week through the entire 12 weeks. Following 6 weeks of generalized strength training, significant improvements occurred in EG for knee extension one-repetition (1RM) maximum strength (+19%), knee extensor muscle thickness (MT, +15%), maximal muscle activation (+44% average) and onset latency ( -31% average) for vastus lateralis (VL), vastus medialis (VM) and rectus femoris (RF) compared to CG (p<0.05). Following 6 more weeks of specific strength training, the 1RM increased significantly and similarly between groups (average of +21%), as did muscle thickness of the VL (+25%), and activation of VL (+44%) and VM (+26%). The onset latency of RF (TG=285 ± 109 ms, PG=252 ± 76 ms, RG=203 ± 43 ms), reaction time (TG=366 ± 99 ms, PG=274 ± 76 ms, RG=201 ± 41 ms), 30-s chair stand (TG=18 ± 3, PG=18 ± 1, RG=21 ± 2) and counter movement jump (TG=8 ± 2 cm, PG=10 ± 3 cm, RG=13 ± 2 cm) was significantly improved only in RG (p<0.05). At the end of training, the rate of force development (RFD) over 150 ms (TG=2.3 ± 9.8 N·s(-1), PG=3.3 ± 3.2 N·s(-1), RG=3.8 ± 6.8 N·s(-1), CG=2.3 ± 7.0 N·s(-1)) was significantly greater in RG and PG than in TG and CG (p<0.05). In conclusion, rapid strength training is more effective for the development of rapid force production of muscle than other specific types of strength training and by consequence, better develops the functional capabilities of older women.
