Clinical analysis of the patients with primary colorectal cancer in our hospital and problems in treatments.

1970年8月より1984年1月までの13年6ヵ月間に, 当院外科で取り扱った原発大腸癌症例223例の臨床的分析による治療上の問題点を, 初発症状, 多発癌, 早期癌, 若年者癌, 病期分類別生存率, 術後補助化学療法別生存率を主とした観点で検討した. その結果, 1) sm大腸癌はポリ-プ型, 高分化, 断端浸潤陰性, 脈管侵襲陰性の場合はポリペクトミーでよい. 2) 5年生存率では, 臨床病期IIとIII, Astler-Coller分類B1とB2の間に明瞭な差が認められた. 3) 切除症例の術後補助化学療法別生存率の検討より, 早期大量投与の効果と長期連用投与の必要性が示唆された.

CLINICOPATHOLOGICALLY COMPARATIVE STUDY BETWEEN BREAST CANCER OF YOUNG WOMEN AND OF ELDERLY WOMEN

最近10年 間 の原発乳癌手術例160例 の うち, 34歳 以下16例 を 若 年者乳癌症例, 70歳 以上 21例 を高年者乳癌症例 として,両 群 の臨床病理学的比較検討を行な った. 若年者群 は28か ら34歳,平 均31.1歳,高 年者群は70か ら85歳,平 均75.5歳 であ る. 病悩期間 は, 6カ 月以上 の長期 の症例は若年者群: 56%が 高年者群: 38%よ り多 く,腫 瘍径 も5cm以 上 の大きい症例は若年者群: 32%が 高年者群: 18%よ り多い.す なわ ち若 年者群 の方 が病悩期間 も長 く腫瘍径 も大 きい進行 した症例が多い傾向がみ られ る. 初 診時 の臨床診断 は,高 年群 の91%が 乳癌 ・乳癌疑診 であ るの に比 し,若 年群 では乳 癌: 19%,乳 癌疑診: 31%で あ り,他 には線維腺腫: 38%が 多い.試 切 の比率 は若年群: 81%が 高年 群: 48%よ り多い. 高年者群 にTO乳 癌が2例 あ り,い ずれ も血性乳頭 分泌を主訴 とし, microdochectomy で 確定診断 した 非 浸潤癌 である.最 近2年3カ 月 の 間 のmicrodochectomy施 行症例 は 28例 で,う ち4例(14%)が 乳癌 であ り,乳 癌例 の平均年齢が69.3歳 であるこ とより,高 年者 の乳頭異常分 泌例 に対 して は 積極的な乳管造影, microdochectomyが 必要 と考 えら れ る. 手術術式 は若年 者群 は1例 を除いて定型 あるいは拡大術式を施行 して い るが,高 年 者群 は単乳切,非 定型 が81%で あ る.高 齢 に よる リス クの点 より仕方 のない面 もあ るが,今 後 非定型的乳 房切断術の適応 の検 討に より少 しで も根 治性 の高い術式 を選択 したい. 組織型 は,若 年 者群 は乳頭腺 管癌が 多 く,高 年者 群は髄様腺管癌 の症 例が多い. 組織学的腫瘍悪性度 は若年者群 はGradeIIIが 最 も多 く,次 いでGradeII,高 年者 群は GradeIIが 最 も多 く,次 いでGradeIIIの 順 であ る.

Effects of sympathomimetic and sympatholytic agents on mesenteric hemodynamics in hemorrhagic hypotension

Hemodynamic effects of sympathomimetic and sympatholytic agents on mesenteric circulation both before and after hemorhage have been studied in anesthetized dogs. By acute hemorrhage of 25.8 ml per Kg of body weight, 82 per cent decrease in superior mesenteric artery flow was noted, while cardiac output decreased by 69 per cent. Mesenteric vascular resistance increased by 97 per cent, whereas total peripheral vascular resistance increased by 27 per cent. Administration of norepinephrine resulted in a diphasic, response of superior mesenteric artery flow in controls, i.e., the initial flow decrease was followed by the subsequent increase. However, a number of animals in hemorrhagic hypotension did not show the diphasic flow response because of the lowered response to catecholamines. The hemodynamic changes induced by epinephrine were similar to those by norepinephrine. However, in the initial flow decrease noted following epinephrine adminsstration was less than that of norepinephrine. Administration of isoproterenol resulted in a marked increase (p<0.05) of superior mesenteric artery flow and of cardiac output even in hemorrhagic hypotensive state. By phenoxybenzamine administration superior mesenteric artery flow was significantly (p<0.05) increased in controls, whereas the flow was greatly decreased in several hypotensive animals. Propranolol produced a significant reduction (p<0.05) in cardiac output and a secondary decrease in superior mesenteric artery flow both before and after he morrhage.
