International prevalence measurement of care problems: results.

AIM This report describes the results from the last international prevalence measurement of care problems in the Netherlands, Austria and Switzerland, including the course of the prevalence rates during the past 4 years. BACKGROUND Basic care problems such as pressure ulcers, malnutrition and falls occur frequently in healthcare organizations. Measuring these care problems provides insight into their occurrence, and, while a measurement is included of the prevention, treatment and structural quality indicators, this gives institutions the possibility of improving their care regarding these care problems. DESIGN An annual cross-sectional multicentre study. METHOD The prevalence measurement of care problems is conducted annually on one specific day in different healthcare settings, among which are hospitals and care homes. Data are collected by means of a comprehensive, standardized questionnaire that comprises three levels: institutional, ward/department and patient level. RESULTS Besides general characteristics of patients, results are presented for prevalence rates, prevention, treatment and quality indicators regarding each care problem for each country.

Involvement of older people in the development of fall detection systems: a scoping review

BackgroundThe involvement of users is recommended in the development of health related technologies, in order to address their needs and preferences and to improve the daily usage of these technologies.The objective of this literature review was to identify the nature and extent of research involving older people in the development of fall detection systems.MethodsA scoping review according to the framework of Arksey and O’Malley was carried out. A key term search was employed in eight relevant databases. Included articles were summarized using a predetermined charting form and subsequently thematically analysed.ResultsA total of 53 articles was included. In 49 of the 53 articles, older people were involved in the design and/or testing stages, and in 4 of 53 articles, they were involved in the conceptual or market deployment stages. In 38 of the 53 articles, the main focus of the involvement of older people was technical aspects. In 15 of the 53 articles, the perspectives of the elderly related to the fall detection system under development were determined using focus groups, single interviews or questionnaires.ConclusionsUntil presently, involvement of older people in the development of fall detection systems has focused mainly on technical aspects. Little attention has been given to the specific needs and views of older people in the context of fall detection system development and usage.

Involvement of the end user: exploration of older people's needs and preferences for a wearable fall detection device – a qualitative descriptive study

Purpose To explore the needs and preferences of community-dwelling older people, by involving them in the device design and mock-up development stage of a fall detection device, consisting of a body-worn sensor linked to a smartphone application. Patients and methods A total of 22 community-dwelling persons 75 years of age and older were involved in the development of a fall detection device. Three semistructured focus group interviews were conducted. The interview data were analyzed using qualitative descriptive analysis with deductive coding. Results The mock-up of a waterproof, body-worn, automatic and manual alerting device, which served both as a day-time wearable sensor and a night-time wearable sensor, was welcomed. Changes should be considered regarding shape, color and size along with alternate ways of integrating the sensor with items already in use in daily life, such as jewelry and personal watches. The reliability of the sensor is key for the participants. Issues important to the alerting process were discussed, for instance, who should be contacted and why. Several participants were concerned with the mandatory use of the smartphone and assumed that it would be difficult to use. They criticized the limited distance between the sensor and the smartphone for reliable fall detection, as it might restrict activity and negatively influence their degree of independence in daily life. Conclusion This study supports that involving end users in the design and mock-up development stage is welcomed by older people and allows their needs and preferences concerning the fall detection device to be explored. Based on these findings, the development of a “need-driven” prototype is possible. As participants are doubtful regarding smartphone usage, careful training and support of community-dwelling older people during real field testing will be crucial.

Impact of a Mental Health Nursing Training-Programme on the Perceived Recovery-Orientation of Patients and Nurses on Acute Psychiatric Wards: Results of a Pilot Study

ABSTRACT In mental health nursing, recovery-orientation is regarded as a best practice approach for patient care. To assess the effect of an intervention utilising a recovery-oriented approach in acute practice, a controlled pilot trial was conducted. On intervention wards, mental health nurses received special training. Recovery Self-Assessment Scale was used to assess the effects of the training. No statistically significant effects were found, between the experimental conditions. Reasons for this result may be methodological, practical or due an intervention that was not powerful enough. Recovery-oriented intervention studies in mental health nursing should be conducted with caution.

Adaptation of Clinical Practice Guideline Recommendations in Hospitals for People Living With Dementia and Their Caregivers

BACKGROUND Dementia is common in older people in general hospitals. To improve the quality of their care, the use of nonpharmacological interventions based on the best evidence from clinical guidelines is recommended. Many international clinical guidelines exist, but their recommendations are often not used because they do not fit the local setting or the country. AIM The aim of this study was to adapt the international clinical guidelines and their recommendations to the Austrian context regarding nonpharmacological interventions for people with dementia living in the general hospital setting. METHODS The ADAPTE process was chosen as a method for the adaptation. A search for international clinical guidelines was conducted in seven databases within this process. The guidelines which met the inclusion and quality criteria were assessed with the AGREE II instrument by two independent reviewers. The recommendations of the guidelines were extracted. Those that did not fit the Austrian context were excluded, and recommendations with similar statements were summarized. The selected and modified recommendations were translated into German. RESULTS Out of 206 guidelines, three met the inclusion criteria and two, the quality criteria. One hundred and fifty-two recommendations were extracted from these two guidelines, 42 of which were suitable for the Austrian setting and 20 of which had similar statements and could be summarized. Finally, 32 recommendations were identified that were applicable to the general hospital setting in Austria. LINKING EVIDENCE TO ACTION The adaptation of clinical guidelines with the ADAPTE process generated seven topics with 32 recommendations. Many international guidelines exist, but they cannot be applied verbatim in every country due to the fact that some recommendations are not applicable with respect to the national and local context. Creating an adaption of international guidelines is an effective way to provide and link evidence from research to national nursing practice.

Zusammenfassung. Im Berufsfeld der Pflege hat ein Wandel stattgefunden. Neben neuen Ausbildungsmoglichkeiten und den dadurch veranderten Kompetenzen sowie neuer Moglichkeiten der Arbeitsteilung beschaftigt der zunehmende Mangel an Pflegefachfrauen und Pflegefachmanner (Hohere Fachschule / Fachhochschule) sowie Fachfrauen und Fachmanner Gesundheit (Berufslehre) das Management von Spitalern, Alters- und Pflegeheimen sowie der spitalexterne Pflege. Hohe Anforderungen an die Pflegenden stellen zudem die zunehmend auf den ambulanten Bereich ausgerichtete Gesundheitsversorgung und das damit verbundene Schnittstellenmanagement, die anspruchsvolle interdisziplinare Zusammenarbeit, eine selbstbewusstes Klientel, die rasant fortschreitende Technologisierung und Digitalisierung sowie die zunehmende Diversitat und Alterung der Gesellschaft. Zusammenfassung. Dr. Joanne DeSanto Iennaco (PhD) ist Associate Professor der Pflege und der Psychiatrie an der Yale University School of Nursing. Sie ist Psychiatric Mental Health Pacticioner mit Bachelor Abschluss (PMHNP-BC), Psychiatric Mental Health Clinical Nurse Specialist mit Bachelor Abschluss (PMHCNS-BC) und Advanced Practice Registered Nurse (APRN). Als APRN arbeitet sie noch einen Tag pro Woche im Connecticut Medical Health Center in New Haven in einem Ambulatorium, das niederschwellig Behandlung und Pflege anbietet. Sie ist seit 1983 in der psychiatrischen Pflege tatig.

Mitsprache in der Digitalisierung: Systematischer und praxisnaher Einbezug der Nutzenden von gesundheitsrelevanten Technologien

Mitsprache in der Digitalisierung und systematischer, praxisnaher Einbezug von Nutzenden in die Entwicklung gesundheitsrelevanter Technologien ist keine Selbstverstandlichkeit. Dieser Beitrag zeigt auf, dass der Einbezug von Nutzenden in die Entwicklungsprozesse komplex ist. Diese Komplexitat beginnt schon bei der Definition der Nutzenden. Entsprechend ihrer Charakteristiken gestaltet sich ihr Einbezug unterschiedlich aufwendig. Denn Nutzende konnen einerseits Gesundheitsfachpersonen aber andererseits auch Personen mit Erkrankung, Beeintrachtigung und Behinderung sowie ihre Angehorigen sein. Ohne eine sorgfaltige Analyse der Situation, in der sich die Nutzenden befinden und mit welchen krankheits- oder beeintrachtigungsbedingten Herausforderungen sie konfrontiert sind, kann kein umfassender Einbezug erfolgen. Dies verlangt von den Personen, welche gesundheitsrelevante Technologien gemeinsam mit den Nutzenden entwickeln, einerseits hohe fachliche Kompetenzen und andererseits Kompetenzen in den Bereichen Kommunikation, Moderations- und Gruppentechniken sowie Kenntnisse uber die Situation, in der sich die Nutzenden befinden. Ohne interdisziplinare Vernetzung und Zusammenarbeit scheinen diese Anforderungen kaum erfullbar. Der Beitrag fuhrt in die Thematik ein, zeigt die Herausforderungen aber auch die Notwendigkeit der klaren Charakterisierung der Nutzenden auf und beschreibt, wie ein systematischer Einbezug von Nutzenden gesundheitsrelevanter Technologien gelingen kann. Zudem werden Lucken im erfolgreichen Einbezug von Nutzern beschrieben und Anregungen fur zukunftige Forschungs- und Entwicklungsarbeit aufgezeigt.
