THE EFFECT of DENTIN PRETREATMENT on THE MICROTENSILE BOND STRENGTH of SELF-ADHESIVE RESIN CEMENTS

STATEMENT OF PROBLEM Self-adhesive cements have lower bond strength than conventional resin cements that rely on the application of etch-and-rinse adhesive systems. PURPOSE The purpose of this study was to evaluate the effect of tannic and polyacrylic acid on the microtensile bond strength of self-adhesive resin cements to dentin. MATERIAL AND METHODS Eighteen molar crowns were flattened to expose dentin and divided into 3 groups according to the dentin pretreatment: (1) control, without surface treatment; (2) 25% polyacrylic acid solution (Ketac Conditioner); (3) 20% tannic acid. Composite resin (Tescera) blocks were luted to dentin surfaces using 2 self-adhesive resin cements (RelyX Unicem or Maxcem Elite) (n=3). All specimens were stored in distilled water for 24 hours, sectioned into beams (1 mm(2)), and tested to failure using a microtensile method at a crosshead speed of 1 mm/min. The data were statistically analyzed using 2-way ANOVA and Fishers PLSD tests (α=.05). RESULTS The polyacrylic acid significantly improved the microtensile bond strength of RelyX Unicem (14.92 ±4.94 MPa) when compared to the control group (8.35 ±1.99 MPa) and tannic acid treatment (8.38 ±2.67 MPa) (P<.001). Dentin surface treatment did not affect the microtensile bond strength of Maxcem Elite cemented groups (control, 8.45 ±3.21 MPa; polyacrylic acid, 9.53 ±9.95 MPa; tannic acid, 6.89 ±4.45 MPa). CONCLUSIONS Dentin pretreatment with polyacrylic acid improved the microtensile bond strength of RelyX Unicem.

Characterization of water sorption, solubility and filler particles of light-cured composite resins

The goals of this study were to measure the water sorption (WS) and solubility (SO) of 3 composite resins containing different filler contents. Additionally, the size, shape, type and other characteristics of fillers were analyzed by scanning electron microscopy (SEM). Three composites, classified according to filler size, were selected: Filtek Supreme nanofill (3M/ESPE), Esthet-X minifill (Dentsply/Caulk) and Renamel microfill (Cosmedent Inc.). Ten disk-shaped specimens of each resin composite were made and stored in desiccators until constant mass was achieved. Specimens were then stored in water for 7 days, and the mass of each specimen was measured. The specimens were dried again and dried specimen mass determined. The WS and SO were calculated from these measurements. Data analyzed by one-way ANOVA and Tukeys post-hoc test (alpha=0.05). Composite filler particles were observed under SEM after removal of resin matrix by organic solvents. WS values were not significantly different among the resins; however, SO values were lower for Filtek Supreme. The materials presented differences in filler contents (e.g. particle size and shape). The composite resins had similar WS, while the SO was lower for the nanofill than for mini and microfill resins. The filler characteristics varied and were different among the materials.

Comparison of silorane and methacrylate-based composite resins on the curing light transmission

The aim of this study was to investigate the influence of different composite resins - Filtek P90 (silorane-based composite) and Heliomolar (methacrylate-based composite) - on light transmission and decrease in Knoop hardness between the bottom and top of cured specimens. The irradiance of a light-curing unit (LCU) was measured with a power meter (Ophir Optronics; 900 mw/cm2) and spectral distributions were obtained using a spectrometer (USB 2000). Twenty standardized cylindrical specimens (2 mm thick x 7 mm diameter) of each composite resin were obtained by curing using the LCU for 40 s. Light energy transmission through the composite was calculated (n=10). The Knoop hardness number for each surface was recorded as the mean of 3 indentations. The difference in Knoop hardness between the top and bottom (DKH) of the same specimen was calculated (n=10). The irradiance of light that passed through Filtek P90 (272 mW/cm2) was not significantly greater than that the passed through Heliomolar (271 mW/cm2). The DKH of Filtek P90 (25%) was significantly higher than that of Heliomolar (12%). There was a greater degree of subsurface polymerization of the methacrylate-based composite compared to the silorane-based composite.

Enamel susceptibility to red wine staining after 35% hydrogen peroxide bleaching

Concern has been expressed regarding the staining of enamel surface by different beverages after bleaching. This study investigated the influence of 35% hydrogen peroxide bleaching agents on enamel surface stained with wine after whitening treatments. Flat and polished bovine enamel surfaces were submitted to two commercially available 35% hydrogen peroxide bleaching agents or kept in 100% humidity, as a control group (n = 10). Specimens of all groups were immersed in red wine for 48 h at 37°C, immediately, 24 h or 1 week after treatments. All specimens were ground into powder and prepared for the spectrophotometric analysis. Data were subjected to two-way analysis of variance and Fishers PLSD test at 5% significance level. The amount of wine pigments uptake by enamel submitted to bleaching treatments was statistically higher than that of control group, independently of the evaluation time. Results suggested that wine staining susceptibility was increased by bleaching treatments.

Effects of Combined Use of Light Irradiation and 35% Hydrogen Peroxide for Dental Bleaching on Human Enamel Mineral Content

OBJECTIVE The objective of this study was to evaluate the effects of the combined use of light irradiation (LIR, halogen light, or LED/diode laser) and 35% hydrogen peroxide (35%HP) on human enamel mineral content. BACKGROUND DATA The use of high-intensity light has been indicated for acceleration of the rate of chemical bleaching; however, it is not known whether LIR can promote additional effects on enamel surfaces during the bleaching. MATERIALS AND METHODS One hundred enamel samples were obtained from third molars and randomly divided into 10 groups (n = 10). The control group (CG) remained untreated. Three whitening products were used: Whiteness HP Maxx, Pola Office, and Opalescence Xtra. Bleaching consisted of one session, and the products were applied three times to each specimen for 10 min each. The products were subjected, or not, to LIR during treatment with halogen light or LED/diode laser. The mineral concentration of enamel was determined before and after treatments using an FT-Raman spectroscope (FT-RS), and the amount of calcium lost from the bleached enamel surfaces was quantified with an atomic absorption spectrometer (AAS). RESULTS FT-RS results showed a decreased mineral content after all treatments, with the exception of Pola Office when irradiated with LED/diode laser and the CG. The losses of calcium detected for Pola Office and Opalescence Xtra were similar for the three situations (without or with light irradiations), whereas for Whiteness HP Maxx the lowest calcium loss was detected without LIR. CONCLUSION Most of the bleaching treatments investigated, in combination with LIR or not, can reduce the mineral content of enamel surface. LIR increased the calcium loss for Whiteness HP Maxx; no effects were observed for Pola Office and Opalescence Xtra.

Radiation-related caries and early restoration failure in head and neck cancer patients. A polarized light microscopy and scanning electron microscopy study

IntroductionRadiation-related caries is a disease with high potential of teeth destruction in patients who have undergone radiotherapy in the head and neck region. Also, it is a challenge for dentistry due to high rates of recurrent caries and early restorations failure.PurposeThis study aims to analyze the early restoration failures in order to better understand the etiology of dental restorations reduced longevity in irradiated teeth.MethodsFifteen restored permanent teeth extracted from 11 patients who had finished head and neck radiotherapy were studied. Sections from each tooth were prepared and a qualitative description of the interface between restorations and dentin was performed by using polarized light microscopy and scanning electron microscopy.ResultsUnfavorable anatomical shape of restorations, residual caries, and secondary caries affecting dentin adjacent to restorative materials were widely found. The morphological patterns of these carious lesions were similar to conventional dentin lesions with superficial demineralized zone and translucent zone.ConclusionsEarly dental restoration failure in teeth affected by radiation-related caries may have the same etiological factors from ordinary dental restoration failure and direct radiogenic damage to dentition would not be essential to early restorations failure in radiation-related caries.

Effects of the Addition of Fluoride and Calcium to Low-Concentrated Carbamide Peroxide Agents on the Enamel Surface and Subsurface

OBJECTIVE The present study aimed to determine if the addition of fluoride (F) and calcium (Ca) to home-applied 10% carbamide peroxide (CP) bleaching agents reduces mineral loss on the enamel surface and subsurface. BACKGROUND Home-applied 10% carbamide peroxide effectively whitens teeth. However, bleaching possibly promotes adverse side effects such as tooth hypersensitivity and morphological and chemical alterations, such reductions in calcium and fluoride, that affect the enamel surface. METHODS Sixty human enamel slabs were selected based on evaluation of their surface microhardness (n = 10) and submitted to the application of either a placebo agent, Whiteness™, Opalescence™, Pola Night,™ or one of two experimental carbamide peroxide gels: the first with 0.2% fluoride added and the second with 0.2% calcium added. The gel was applied for 6 h/day for 14 days. Samples were submitted to FT-Raman spectroscopy to evaluate surface mineral content before and after treatment. Additionally, cross-sectional microhardness and polarized light microscopy were carried out, to determine enamel subsurface inorganic loss and lesion depth. RESULTS A significant decrease in mineral content was observed after carbamide peroxide treatment, which resulted in increased lesion depth (p < 0.05). Cross-sectional microhardness assessment and polarized light microscopy demonstrated that all groups (except the placebo) exhibited enamel subsurface mineral loss; however, the inorganic deficit could be effectively controlled by the addition of F and Ca to the experimental bleaching agents. CONCLUSIONS The addition of F and Ca to home-applied bleaching agents may reduce enamel mineral loss.

Effect of Hydrogen Peroxide at 35% on the Morphology of Enamel and Interference in the De-remineralization Process: An In Situ Study

This study evaluated the microhardness and histomorphology of bovine enamel when 35% hydrogen peroxide is used. A total of 44 specimens were adapted to removable devices used by 11 individuals subjected to dental caries challenge. A decrease in microhardness was observed for all groups after the cariogenic challenge. Microscopic analysis revealed that fragments subjected to cariogenic challenge associated with bleaching had more intense superficial histologic changes, but the depth of the lesions remained unchanged. It was concluded that 35% hydrogen peroxide enhanced the reduction in hardness and histomorphologic changes in the enamel surface exposed to cariogenic challenge.

Can green tea be used to reverse compromised bond strength after bleaching

The objective of this study was to evaluate the antioxidant effect of green tea on microtensile bond strength (μTBS) to bleached enamel. Forty-two human third molars were randomly divided into six experimental groups (n = 7 each group): group 1, no treatment; group 2, bleaching (10% carbamide peroxide); group 3, bleaching + 10% sodium ascorbate gel (SA); group 4, bleaching + 10% green tea gel (GT); group 5, SA; and group 6, GT. In groups 2, 3, and 4, bleach was applied onto the enamel surface for 6 h, every day for 14 d. In groups 3 and 5, SA was applied for 1 h; and in groups 4 and 6, GT was applied for 1 h. Immediately after treatment, the specimens were bonded with Adper Single Bond 2 and Filtek Z350 XT. The μTBS of the specimens was tested using a universal testing machine. Fracture mode analysis of the bonded enamel surface was performed using scanning electron microscopy. The mean μTBS values for each group were: group 1, 33.2 ± 5.8 MPa; group 2, 22.6 ± 5.5 MPa; group 3, 30.0 ± 5.2 MPa; group 4, 31.6 ± 3.8 MPa; group 5, 29.1 ± 4.2 MPa; and group 6, 32.2 ± 4.5 MPa. All groups had a higher percentage of mixed failures. In conclusion, green tea can be used as an alternative antioxidant on bleached enamel before bonding procedures.

Surface detail reproduction and dimensional accuracy of stone models: influence of disinfectant solutions and alginate impression materials

This study compared the surface detail reproduction and dimensional accuracy of stone models obtained from molds disinfected with 2% sodium hypochlorite, 2% chlorhexidine digluconate or 0.2% peracetic acid to models produced using molds which were not disinfected, with 3 alginate materials (Cavex ColorChange, Hydrogum 5 and Jeltrate Plus). The molds were prepared over matrix containing 20-, 50-, and 75-µm lines, performed under pressure with perforated metal tray. The molds were removed following gelation and either disinfected (using one of the solutions by spraying followed by storage in closed jars for 15 min) or not disinfected. The samples were divided into 12 groups (n=5). Molds were filled with dental gypsum Durone IV and 1 h after the start of the stone mixing the models were separated from the tray. Surface detail reproduction and dimensional accuracy were evaluated using optical microscopy on the 50-µm line with 25 mm in length, in accordance with the ISO 1563 standard. The dimensional accuracy results (%) were subjected to ANOVA. The 50 µm-line was completely reproduced by all alginate impression materials regardless of the disinfection procedure. There was no statistically significant difference in the mean values of dimensional accuracy in combinations between disinfectant procedure and alginate impression material (p=0.2130) or for independent factors. The disinfectant solutions and alginate materials used in this study are no factors of choice regarding the surface detail reproduction and dimensional accuracy of stone models.
