Usefulness of Myotonometer for Measurement of Tissue Compliance on Medialis Gastrocnemius in Patients with Stroke

The aim of this study was to find useful parameters of the between myotonometer and surface electromyography(sEMG) on the medialis gastrocnemius of stroke patients and investigate between these parameters and modified Ashworth scale(MAS) relationship. 5 years clinical experience physical therapist using the Modified Ashworth Scale(MAS) was selected 15 patients with ankle spasticity and divided randomly MAS2, MAS3, MAS4 groups. Myotonometer and sEMG was measured during relaxed and maximum voluntary contractions of the gastrocnemius muscle. The results of this study, the higher MAS score was the lowered the relaxation and contraction state tissue compliance and muscle activity and in the correlation analysis the higher MAS score during voluntary contraction in the cylinder receiving low-intensity correlation could see that increased than relaxation. Therefore, the myotonometer is a useful clinical and research tool with spasticity muscle and can provide objective quantitative data about the efficacy of physical therapy interventions.

Effects of Vibration Stimulation Method on Upper Limbs Spasticity in Patients with Brain Lesion

We studied the effects of vibration stimulation method on upper limbs spasticity in patients with brain lesion. 21 patients with spasticity of the upper limbs selected and divided randomly 3 groups. And then vibratory stimulation was applied to the triceps brachii muscle in group Ⅰ(n=7), to biceps brachii muscle in group Ⅱ (n=7), and to both muscles in group Ⅲ (n=7). Using Neuro-EMG_Micro to investigate the changes in spinal neuronal excitability, F-waves were measured at before and directly after stimulation, and 10 minutes later and 20 minutes later after stimulation especially. MAS(Modified Ashworth Scale) test for muscle tone and MFT(Manual Function Test) for the upper extremity motor function were performed before stimulation and 20 minutes later after stimulation for the purpose of clinical evaluation. In our study, MAS was significant decreased in all groups, F wave and F/M ratio parameters were decreased in all groups and more decreased specially in group Ⅲ. MFT was increased in group Ⅱ and Ⅲ, and more increased specially in group Ⅲ. Vibration stimulation reduced the neuronal excitability of spinal cord and also muscle tone, and improved the motor function of the upper extremity. These results suggested that vibration stimulation giving to both muscles(triceps and biceps brachii muscle) at the same time was more efficiency in reducing the neuronal excitability of spinal cord and improving the motor function of the upper limbs.

Dual-afferent sensory input training for voluntary movement after stroke: A pilot randomized controlled study

BACKGROUND Stimulation through afferent sensory input is necessary to improve voluntary functional movement in stroke patients. OBJECTIVES Dual-afferent sensory input, which combines electromyography-triggered functional electric stimulation (ETFES) and action observation, was investigated to determine its effects on voluntary movements in stroke patients. METHODS This study was conducted on 18 patients with left hemiplegia diagnosed between 6 and 24 months prior. The 9 subjects in the dual-afferent sensory input (DASI) group underwent ETFES with action observation training for 4 weeks (20 min/d, 5 d/wk), while the 9 control group subjects underwent functional electric stimulation (FES) for the same duration. The outcome measures were the movement-related cortical potential (MRCP), H-reflex, electromyography (EMG), and balance. RESULTS The control and DASI groups showed significant increases in MRCP, muscle activity, and balance, while H-reflex was significantly decreased. MRCP and balance showed significant differences between DASI and control groups. CONCLUSIONS DASI stimulates voluntary movement in patients, causes rapid activation of the cerebral cortex, and reduces excessive excitation of spinal motor neurons. Therefore, DASI, which stimulates voluntary movement, has a greater effect on brain activation in stroke patients.

A Study on Muscle Architectural and Tissue Compliance of Biceps Brachii in Stroke Patient Based on Elbow Joint Angle

The aim of this study was to find on muscle architectural and tissue compliance of biceps brachii in stroke patient based on elbow joint angle. The subjects of this study were twelve hemiplegic adults after stroke with passive range of motion in the elbow from to and Modified Ashworth Scale score 1 to 3 were recruited. Ultrasonography and Myotonometer was used to measure biceps brachii muscle pennation angle, fascicle length, and tissue compliance at the rest condition and pennation angle, fascicle length, and tissue compliance of the biceps brachii muscle were measured in the affected and unaffected sides of people after stroke at 9 different elbow angles ranging from to at the rest condition. The results of this study, comparisons found that the pennation angles of the affected biceps brachii muscle were significantly larger(p), whereas the affected fascicle lengths were significantly shorter(p>), and the affected tissue compliance were significantly lower(p) Therefore, pennation angles, fascicle lengths, and tissue compliance were found to be joint-angle-dependent in both the affected and unaffected sides at the rest condition. Suggest that, the results data can be used as a muscle architectural changes and clinical treatment research in stroke patients.
