Attitudes of surgical nurses towards pressure ulcer prevention

STUDY AIMnNurses play the key role in prevention of pressure ulcers, and negative attitudes of them may affect preventive care strategies. This research aimed to identify surgical nurses attitudes towards pressure ulcer prevention.nnnMATERIALS AND METHODSnA descriptive and cross-sectional study design was used to collect data between March 31-May 16, 2016 on 101 voluntary nurses (66.01% of all nurses) working in surgical units of a university hospital in Turkey. Attitude towards Pressure Ulcer Prevention Instrument and Nurse Information Form were used to investigate nurses attitude toward preventing pressure ulcer. Descriptive statistics, Mann-Whitney U test, independent samples t-test, Pearsons chi-squared, and correlation tests were used to analyse the data.nnnRESULTSnThe mean age of nurses was 32.0xa0±xa06.3 years and, the average duration of clinical experience was 72.3xa0±xa061 months. A total of 54.5% were working in surgical wards and, 40.6% gained additional education about pressure ulcer care. The mean total attitude score was found to be 80.5% (41.8xa0±xa04.8). Previous education significantly affected the total attitude score of nurses (pxa0<xa00.05).nnnCONCLUSIONnThe total scale score showed that surgical nurses had a positive attitude towards pressure ulcer prevention and nurses who had previous education about pressure ulcer care has higher attitude score than who had not. It is recommended that effective in-service education programs be developed at hospitals or that nurses be required to attend courses to help them to improve their pressure ulcer care.

The effect of hand exercise on reducing the symptoms in hemodialysis patients with carpal tunnel syndrome

Context: Carpal tunnel syndrome (CTS) is one of the most common peripheral neuropathies, and there is no consensus on the preferred method of treatment. Aims: The aim of this study is to determine the effects of hand exercise performed with a ball on symptoms and to evaluate the results in hemodialysis (HD) patients with CTS. Settings and Design: This study was conducted in patients with HD who were treated in dialysis centers and state hospitals in Turkey between 2011 and 2012. Subjects and Methods: This study included 19 patients (28 hands) that were diagnosed as CTS. For exercise treatment, a hand and finger exercise ball that consists of two parts: a foam body and flexible rubber cords was used. Patients placed their fingers through each cord, squeezed it for 1 s, and then opened their fingers against the cord for 1 s. Patients repeated this exercise for 30 s to 1 min/day. Statistical Analysis Used: The McNemar, Friedman ANOVA, and the Wilcoxon test with a Bonferroni correction were used on SPSS 20.0 software package program. P < 0.05 was considered statistically significant. Results: Results showed that recovery was performed from physical examination results, grip strength, and Boston questionnaire scores. About 21.4% of patients electrophysiological results were negative at the end of the 1st month and 32.1% of them at the end of the 3rd month. Conclusion: Due to the slowed progress of CTS and detection of slight improvement in evaluation parameters, this self-applicable and practical exercise can be used as an alternative treatment of mild CTS in patients with HD.

CERRAHİ BİRİMLERDE ÇALIŞAN HEMŞİRELERİN ETİK DUYARLILIKLARININ BELİRLENMESİ

OZET Amac: Cerrahi birimlerde calisan hemsirelerin etik ikilemlere karsi duyarlilik duzeylerinin belirlenmesidir. Yontem: Tanimlayici turde yapilan bu arastirma, 22 Aralik 2013-15 Ocak 2014 tarihleri arasinda calismaya katilmayi kabul eden 90 hemsire ile yurutuldu. Veriler, bireysel ozellikler formu ile Ahlaki Duyarlilik Anketi kullanilarak toplandi. Verilerin analizinde tanimlayici istatistikler ile Mann Whitney U ve Kruskal-Wallis testi kullanildi. Bulgular: Cerrahi hemsirelerinin Ahlaki Duyarlilik Anketi toplam puan ortalamasinin 89.77±19.43 oldugu; otonomi alt boyut puan ortalamasinin 18.35±5.51, catisma alt boyut puan ortalamasinin 13.96±2.97, yarar saglama alt boyut puan ortalamasinin 12.92±3.95, uygulama alt boyut puan ortalamasinin 12.34±3.94, butuncul yaklasim alt boyut puan ortalamasinin 12.00±4.88 ve oryantasyon alt boyut puan ortalamasinin 8.13±3.88 oldugu saptandi. Hemsirelerin calisma sureleri ile butuncul yaklasim ve oryantasyon alt boyut puan ortalamalari arasindaki farkin anlamli oldugu saptandi (p<0.05). Mesleki derneklere uye olma durumu ile Ahlaki Duyarlilik Anketi toplam ve alt boyut puan ortalamalari karsilastirildiginda; otonomi, butuncul yaklasim ve oryantasyon alt boyut puan ortalamalari arasindaki farkin anlamli oldugu bulundu (p<0.05).xa0 Sonuc: Cerrahi birimlerinde calisan hemsirelerin orta duzey etik duyarliliga sahip oldugu saptandi. Hemsirelerin etik sorunlari tanima ve cozum saglamada mezuniyet oncesi egitim programlarinin guclendirilmesi, mezuniyet sonrasi surekli egitim programlarinin ise hemsirelerin yas gruplari ve deneyim suresi goz onune alinarak planlanmasi, onerilebilir. ABSTRACT Determining e thical sensitivity of nurses employed in surgical units Aim: It is the determination of sensitivity levels of the nurses employed in surgical units to ethical dilemmas. Method: This descriptive study was conducted with 90 nurses who acknowledged to participate in the study betweenxa0 December 22 2013xa0 andxa0 January 15 2014. Data was gathered using the Individual Characteristics form and the Moral Sensitivity Survey form. Descriptive statistics, Mann Whitney U and Kruskal-Wallis tests were used in the data analysis. Results: It was found that total mean score of Moral Sensitivity Survey of the surgical nurses was 89.77±19.43; the subscale mean score of autonomy was 18.35±5.51, subscale mean score of conflict was 13.96±2.97, subscale mean score of benefitting was 12.92±3.95, subscale mean score of implementation was 12.34±3.94, subscale mean score of holistic approach was 12.00±4.88 subscale mean score of orientation was 8.13±3.88. It was found that there was a statistically significant difference between the employment period of the nurses and the subscale mean scores of holistic approach and orientation (p<0.05). When compared the status of being member of professional associations with the total score of the Moral Sensitivity Survey and subscale mean scores, the difference between the subscale mean scores of autonomy, holistic approach and orientation was significant in statistical manner (p<0.05).xa0 Conclusion: It was found that the nurses employed in the surgical units have a moderate ethical sensitivity. It can be suggested to strengthen the pre-graduation education programs and to plan the post-graduation continuous education programs by taking into account the age groups and years of experience of nurses in recognizing the ethical problems and in providing solution. Kolonoskopi yapılan hastalarda retrospektif analizle yetersiz bağırsak hazırlığı nedeniyle tamamlanamayan kolonoskopi işlemlerinin sıklığının belirlenmesi

Giris ve Amac: Kolonoskopi isleminin basarisinda bagirsak hazirliginin yeterliligi cok onemlidir. Bagirsak hazirliginin yetersiz olmasi kolonoskopinin basarisini olumsuz etkilemektedir ve islemin tekrarlanmasina sebep olmaktadir. Tekrarlanan kolonoskopiler hastalarin yeniden bagirsak hazirligi surecine maruz kalmasina ve hastanenin mali yukunun ve hastane personelinin is yukunun artmasina ve hastanin invaziv bir islem olan kolonoskopi surecinde anksiyete yasamasina neden olmaktadir. Bu calismanin amaci; 2015 yilinda kolonoskopi yapilan hastalarda retrospektif analizle yetersiz bagirsak hazirligi nedeniyle tamamlanamayan kolonoskopi islemlerinin sikligini belirlemektir.Gerec ve Yontem: Cesitli endikasyonlar ile 01.01.2015- 31.12.2015 tarihleri arasinda bir universite hastanesinin genel cerrahi poliklinigi endoskopi unitesinde kolonoskopi yapilan 320 hastanin kolonoskopi raporu incelendi. Veriler SPSS programinda frekans ve yuzde kullanarak ve t testten yararlanilarak analiz edildi. Bulgular: Hastalarda yetersiz bagirsak hazirligi nedeniyle tamamlanamayan kolonoskopi veya rektosigmoidoskopi orani %14 bulundu. Bagirsak hazirliginin 45 yas ustu grupta daha yetersiz oldugu ve yas gruplarina gore bu farkin istatistiksel acidan anlamli oldugu saptandi (p<0.05). Sonuc: Saglik personeli; hastalarin bagirsak hazirliginda yeterli duzeye ulasabilmeleri icin hastalara onerilen diyet, oral laksatifler ve lavmanlari etkin bir sekilde uygulamalari konusunda detayli bilgilendirme yapmalidir. Sonuc olarak, hastalarin bagirsak hazirligi surecinde yasadigi problemlerin ve hastalarin bagirsak hazirligina uyum saglayamamalarinin nedenlerinin arastirilmasi onemli bir belirtec olacaktir.
