New observations of intraoperative dermoscopic features of the nail matrix and bed in longitudinal melanonychia

In recent years, the following four patterns have been defined in intraoperative dermoscopy of the nail matrix and bed in longitudinal melanonychia (LM): (i) a regular gray pattern; (ii) a regular brown pattern; (iii) a regular brown pattern with globules or blotches; and (iv) an irregular pattern. Histopathological findings in these patterns are, respectively: (i) melanocyte activation; (ii) mostly melanocytic hyperplasia (melanocytic activation and melanoma can also be seen); (iii) only melanocytic nevi; and (iv) only melanoma.

Clenched fist syndrome with palmar lichen planus

A 16‐year‐old girl had a 4‐year history of hyperkeratosis and fissures on the palm. The skin lesions had gradually progressed to thick hyperkeratosis with mild pruritus, diffusely covering the right palm, and leading to flexion contractures of the fingers with mild pain. There was no family history of similar lesions. Her father had died 4 years previously. Dermatologic examination revealed hyperkeratosis, scaling, a mild yellow color, fissures, and maceration on the palm and volar surface of the fingers of the right hand ( Fig. 1 ). There was an approximately 3 cm × 4 cm hyperkeratotic lesion on the palm of the left hand.

Facial and extrafacial involvement of lupus miliaris disseminatus faciei with Kaposi's sarcoma-like lesions: Correspondence

Lupus miliaris disseminatus faciei (LMDF) is an uncommon chronic infl ammatory skin disorder characterized by yellowish-red or red-brown papules [ 1 ] . While the predilection sites are the face and eyelids [ 1 ] , extrafacial involvement of LMDF lesions has also been reported [ 2 ] . Predominantly occurring in young adults, LMDF is very rarely seen in elderly individuals [ 3 ] . Herein, we describe a patient with LMDF who presented with typical as well as Kaposi’s sarcoma-like lesions on the face, neck, and axillae. Anxiety Among Patients Undergoing Nail Surgery and Skin Punch Biopsy Effects of Age, Gender, Educational Status, and Previous Experience

Background: Patient anxiety about nail surgery relates mainly to pain associated with needle puncture, anesthetic flow during the procedure, and postoperative care, as well as possible past traumatic experience. Objective: The aims of this study were to compare anxiety levels among patients undergoing nail surgery and skin punch biopsy and to assess the effects of demographic characteristics on anxiety. Methods: Forty-eight consecutive patients who were referred to a dermatological surgery unit for nail surgery intervention (group 1) and 50 age- and sex-matched patients referred to the same unit for skin punch biopsy (group 2) were enrolled in the study. Patients’ anxiety levels were measured using Spielberger’s State-Trait Anxiety Inventory. Results: There was no significant difference in median anxiety level between group 1 (42.00; interquartile range, 6.50) and group 2 (41.00; interquartile range, 8.25) (P = .517). The demographic factors of patient sex, educational status, and prior surgery showed no significant effects on anxiety levels. Conclusion: Nail surgery does not seem to cause significantly greater anxiety than skin punch biopsy.

Acemetacin-induced fixed drug eruption

Fixed drug eruption (FDE) is an adverse effect observed with various drugs such as nonsteroidal anti-inflammatory drugs (NSAIDs) and various antibiotics. Acemetacin, a prodrug of indomethacin, is an NSAID licensed for use in rheumatic disease and other musculoskeletal disorders. We present a case of acemetacin-induced FDE in a 49-year-old woman. To the best of our knowledge, this is the second case report detailing clinical and histopathological findings of a patient with FDE caused by acemetacin.
