The Effect of Reminiscence Therapy on Cognition, Depression, and Activities of Daily Living for Patients With Alzheimer Disease

The purpose of this study was, conducted with experimental design, to investigate the effect of reminiscence therapy on cognition, depression, activities of daily living of institutionalized mild and moderate Alzheimer patients. The study was conducted with a total of 62 patients (31 intervention group and 31 control group) in four home care in Ankara, Turkey. Study was done between the July 1, 2013 and December 20, 2014. Reminiscence therapy sessions were held with groups consists of 4-5 patients, once a week with 30-35 minute duration for 12 weeks. Standardized Mini Mental Test was used in sample selection. Patients were listed through their mini mental test scores, and randomized as odd numbers to control group and even numbers to intervention group. Data were collected with forms developed by researcher ‘Data Sheet’ and ‘Activities of Daily Living Follow-up Form’ as well as scales ‘Standardized Mini Mental Test’ and ‘Geriatric Depression Scale’. Chi-square, Mann Whitney-U test, variance analyses in repeated measures and Bonferroni tests were used for analysis. The increase in mean Standardized Mini Mental Test score and the decrease in mean Geriatric Depression Scale score of the individuals in the intervention group compared to the control group at the end of the reminiscence therapy was statistically significant (P < 0.05). At the end of reminiscence therapy sessions, increase in cognition and decrease in depression were found statistically significant in intervention group.

How Much Do ED Patients Know About Medication Prescribed for Them on Discharge? A Pilot Study in Turkey

INTRODUCTION Hospital emergency departments provide health care to patients with various ailments and illnesses. If necessary, doctors write prescriptions for patients who visit emergency departments for their use after discharge from hospitals. It is important to inform patients about their prescribed medications because compliance with the prescription plays an important role in the success of the treatment. If a patient must use more than one medication, this might result in negative drug interactions. These undesirable developments may adversely affect the treatment process and cause many unplanned patient visits to emergency departments. This study was carried out to determine patient knowledge as related to the names, dosage, frequency, purpose and course of medications given on discharge from emergency departments. METHODS Study subjects were patients who came to the emergency department between the hours of 8 am and 8 pm during a period of 1 month. Data were collected through use of a questionnaire. RESULTS In this study, it was found that 37% of the patients (37 patients) had no knowledge at all about the prescribed medications; however, out of 63 patients, 61.9% had knowledge of when to take the medications, 57.1% knew the purpose of the particular medications, and 52.3% were aware of the appropriate dosage. Furthermore, 31.7% knew the name of the medications and 25.3% knew something about their prescribed course. CONCLUSION Upon discharge from emergency departments, patients should be fully and properly informed about their prescribed medications through a written document. Providing patients with information concerning the correct use of their prescribed medications enables them to use the medications appropriately, thereby increasing not only their satisfaction but also their compliance with the treatment plan. As a result, this vital information may help to decrease rehospitalizations.

Home nursing care with the self-care model improves self-efficacy of patients with chronic obstructive pulmonary disease.

AIM This study was carried out as an intervention study to determine the effect of nursing care provided at home with the self-care model on the self-efficacy of patients with chronic obstructive pulmonary disease in northern Turkey. METHODS The study sample totaled 106 patients with chronic obstructive pulmonary disease divided into two groups. The intervention group and the control group each included 53 patients. To collect data, the patient data sheet, Orems self-care model-based data collection form, symptom evaluation form, Medical Research Council dyspnea scale, and the chronic obstructive pulmonary disease self-efficacy scale (CSES) were used. The nursing care applied for the intervention group consisted of four visits, two during the first month and two for the next 2 months. An educational guidebook was distributed to the intervention group during the first visit, and at all visits, education, care, and supervision were provided according to the care plan. All data were analyzed with the number, percentage, χ(2) -test, Kruskal-Wallis H test, Wilcoxon rank sum test, and Mann-Whitney U-test. RESULTS The dyspnea level measured at the last visit for the control group was high compared with the intervention group (P < 0.05). The chronic obstructive pulmonary disease self-efficacy scale scores for subscale dimensions at the last visit and the general score for the intervention group were high compared with the first visit scores (P < 0.05). CONCLUSION According to the study results, home nursing care based on the Orems self-care model is effective in improving self-efficacy of patients with chronic obstructive pulmonary disease.

Yatağa bağımlı hastaların evde yaşadıkları sağlık sorunlarına yönelik evde bakım hizmet gereksinimleri

Hemangiomas are the most common tumors of infancy. Most of them require no treatment, but treatment is needed if dramatic aesthetic, and/or functional impairment as visual or airway obstruction or ulceration arises. We reported a 6-month-old infant presented with a 6-week history of a rapidly growing cutaneus hemangioma on the right eyelid and caused visual impairment. The patient was successfully treated with the use of oral propranolol therapy. We suggest that propranolol can be considered as a first line treatment in a patient with infantil hemangioma.Objectives: The aim of this study was to examine levator palpebralis superior muscle histologically in patients with congenital blepharoptosis and to investigate the relationship between these findings and age, sex and degree of blefaroptosis in this patient group. Materials and methods: Levator muscle of 13 patients with congenital ptosis, who had applied to Dicle University Medical Faculty Ophthalmology Clinic and had undergone levator palpebralis superior muscle resection between january 2009-january 2010, has been examined histopathologically in Histology and Embriology Deparment. During preoperative period, ptosis amount, levator function (LF), tear functions, Bell\s phenomenon and jaw-winking phenomenon were evaluated. All patients underwent resection of levator palpebralis superior muscle. Received postoperative levator muscle was examined by light microscopy. Results: The average age of 9 (69.2%) male and 4 (30.8%) female cases were 10.61 p 4.77 (4- 19) years. In histological examination, the quality and quantity of the levator muscle fibrils have been assessed. There was no relationship detected between histological features of levator palpebralis superior muscle and patient\s age and gender (p>0.05). Patients with weak levator palpebralis superior muscle were detected to have fatty degeneration histologically. The higher the levator palpebralis superior muscle function revealed decreased fatty degeneration and increased skeletal muscle fibrils. Conclusion: More ultrastructural studies in larger populations are needed to support the relationship between structure and function of levator palpebralis superior muscle in patients with congenital blepharoptosis.Amac: Lateral epikondilit (tenisci dirsegi) en fazla tani konulan dirsek yan agri nedenidir. Bu calismanin amaci tenisci dirsegi tedavisinde tek doz kortikosteroid enjeksiyonu ile otolog trombositten zengin plazma (TZP) enjeksiyonunun etkilerini karsilastirmaktir. Gerec ve yontem: Dirsek yan kisminda agri sikâyeti sonucu klinigimize muracaat edip, lateral epikondilit tanisi konan 15 hastanin (6 erkek ve 9 kadin) 15 dirsegi calisma kapsamina alindi. Olgularin 1. grubuna tek doz 0,5 ml Bethametasone ve 0.5 ml Prilokain karisimi, 2. grup olgulara 1ml otolog TZP lokal olarak uygulandi. Bulgular: Verhaar skorlama sistemine gore alinan sonuclara gore ilk takiplerde lokal steroid enjeksiyonu yapilan hasta grubunda sonuclarin daha iyi oldugu fakat sonraki takiplerde iyi sonuclarin goruldugu vaka sayisinda azalma oldugu goruldu. TZP grubunda ise ilk takiplerde sonuclarin kotu oldugu fakat sonraki takiplerde daha iyi sonuclarin alindigi goruldu. Sonuc: Otolog TZP enjeksiyonun lateral epikondilitte iyi yonde etkinliginin zamanla artigi soylenebilir, fakat bunun daha iyi anlasilmasi icin olgu sayisi ve takip suresi artirilmis yeni calismalarin yapilmasi gereklidir.

Physiological problems in patients undergoing autologous and allogeneic hematopoietic stem cell transplantation

Objective: Stem cell transplantation is usually performed in an effort to extend the patients life span and to improve their quality of life. This study was conducted to determine the postoperative physiological effects experienced by patients who had undergone autologous and allogeneic stem cell transplantation. Methods: The research is a descriptive study conducted with a sample of 60 patients at Stem Cell Transplantation Units in Ankara. Percentile calculation and chi-square tests were used to evaluate the data. Results: When a comparison was made between patients who had undergone allogeneic Hematopoietic stem cell transplantation (HSCT) and those who had undergone autologous HSCT, results indicated that problems occurred more often for the allogeneic HSCT patients. The problems included: Digestion (94.3%), dermatological (76.7%), cardiac and respiratory (66.7%), neurological (66.7%), eye (56.7%), infections (26.7%) and Graft Versus Host Disease (5 patients). Furthermore, the problems with pain (50%), numbness and tingling (40%), and speech disorders (3 patients) were observed more often in autologous BMT patients. Conclusion: Autologous and allogeneic patients experienced most of physical problems due to they receive high doses of chemotherapy. Therefore, it is recommended that an interdisciplinary support team approach should be usedtohelp reduce and manage the problems that may arise during patient care.

Effect of progressive muscle relaxation on the caregiver burden and level of depression among caregivers of older patients with a stroke: A randomized controlled trial: Effect of progressive muscle relaxation

AIM This study aimed to investigate the effect of progressive muscle relaxation on the caregiver burden and level of depression among the caregivers of older patients with a stroke. METHODS This randomized, controlled experimental study was conducted between December, 2015 and July, 2017 with the caregivers of older patients with a stroke who had been admitted to the home care unit of a state hospital. The caregivers were randomly assigned to intervention (n = 23) and control (n = 21) groups, based on the studys inclusion criteria. The caregivers in the intervention group practiced progressive muscle relaxation exercises at home 3 days per week for 8 weeks. The control group had no intervention. RESULTS The mean age of the older patients with a stroke was 80.15 ± 9.86 years and the mean duration of the disease was 39.07 ± 44.69 months. The mean age of the caregivers was 50.29 ± 12.62 years, 84.1% of whom were female, 38.6% were elementary school graduates, and 86.4% were married. It was found that there was a statistically significant decrease in the mean Zarit Caregiver Burden Scale and Beck Depression Scale scores of the caregivers in the intervention group after practicing progressive muscle relaxation, but this decrease was not statistically significant when the intervention and control groups were compared. CONCLUSION This study showed that there was a statistically significant decrease in the caregiver burden and level of depression among the caregivers in the intervention group after progressive muscle relaxation exercises, but that there was no statistically significant difference when the intervention and control groups were compared.

Öğrenci Gözüyle Nöroloji Hastasına Bakım Vermek: Nitel Bir Çalışma

Purpose: This study was conducted to determine the difficulties experienced by nursing students who gave care for individuals with neurological system disorders. Method: Data were collected through focus group interviews which is a qualitative study method. The sample of the study consisted of 24 nursing students who agreed to participate in the research from second year students of the nursing faculty of a university in Ankara (due to their clinical practice experience in the neurology clinic), and a total of 4 focus group interviews were conducted with 6 students in each group. The questionnaire including the demographic characteristics of the students, and the semi-structured questionnaire were used in the collection of data. Results: The average age of the students was 20.4 and 75% of them were female. It was found that all of the students gained knowledge from lectures about neurological diseases, and 75% of them had no relatives with neurological diseases. Majority of students expressed their feelings such as pity, sadness, fear, and anxiety towards patients. All of them stated that they experienced difficulties while working with individuals with neurological system disease. They specified these difficulties such as communication difficulties, complex situations in the care of the individual with neurological system disease, and working with patients and relatives who fear death and uncertainty. They remarked they received support from educators, clinical nurses and physicians to cope with these difficulties. Conclusion: As a result of the research, it was found that all 2nd grade nursing students had difficulties while working with individuals having neurological system disease. It was determined that the students experienced negative feelings such as pity, sadness, and anxiety towards the patients, and had difficulty in communicating with and giving care to these patients .

AGING FROM THE PERSPECTIVE OF ELDERLY PEOPLE IN TURKEY: A FOCUS GROUP STUDY OF A NURSING HOME

This research was aimed to know about character building in teaching learning proces observed in 63 elementary schools in Jakarta in Indonesia. This survey research was conducted at 63 elementary schools in Jakarta. Data was collected through interviewing and observing. Character building in teaching learning process observed was related to preliminary, core, and closing activities. Preliminary activities of teaching learning process integrated character building in class preparation, students’ presence, and apperception. Character building in core activities in teaching learning process involved teaching method, two-way communication, students activities, learning resources. Integration of character building in closing activities of teaching learning process included concluding activities, giving feed back, reflecting, and following up. The result of this research showed that mean of character building through teaching learning process at 63 elementary schools in Jakarta reached was 44.87 or 97.54 % of the achievement of theoretical maximum score.from maximum score. It can be concluded that in teaching learning process, character building integrated has already been effective for 65.1 % of 63 elementary schools observed in Jakarta. On the other hand 9.6 % of 63 elementary schools observed in Jakarta has not been optimal in character building integration in teaching learning process.

The Effect of Progressive Relaxation Exercises on Fatigue and Sleep Quality in Individuals With COPD

This randomized controlled experimental study was conducted to investigate the effect of progressive muscle relaxation exercises on dyspnea, fatigue, and sleep quality in individuals with chronic obstructive pulmonary disease. A Descriptive Questionnaire and the Chronic Obstructive Pulmonary Disease and Asthma Fatigue Scale, Chronic Obstructive Pulmonary Disease and Asthma Sleep Scale and Medical Research Council Dyspnea Scale were used for data collection. The decrease in the mean dyspnea, fatigue, and sleep scores in the intervention group was statistically significantly more pronounced than the patients in the control group (P < .05). Progressive relaxation exercises can be implemented to decrease the dyspnea, fatigue, and sleep problems seen in patients with moderate and advanced chronic obstructive pulmonary disease by nurses working in the clinic.

Nursing students' perspectives on assisting cancer patients

Objective: The objective of this study was to examine the experiences of student nurses who have provided care to cancer patients. Methods: A mixed method approach consisting of semistructured focus groups (n = 61) and a survey questionnaire (n = 129) was used in the study. Student nurses were first interviewed, and then, a questionnaire was developed for them to answer. Following the content analysis, three themes and 19 subthemes were identified. Frequency and percent were used for qualitative data. Results: Among the student nurses, 80.6% reported that working with cancer patients was “difficult.” Difficulties experienced by the student nurses included patients rejecting their care, a large number of problems cases encountered when providing care to cancer patients, communication problems (38.0%), working with patients and attendants who fear death, and problems arising from family attendants who obstruct care. The majority of students experienced patients fearing death (28.7%) and felt feelings such as pity (71.1%), sadness (50.0%), and fear of cancer (41.9%) during their internships in oncology clinics. Conclusions: Students should be supported by instructors and oncology nurses, and nursing curricula should contain topics on how to best approach cancer patients.
