Axial and Reformatted Four-Chamber Right Ventricle–to–Left Ventricle Diameter Ratios on Pulmonary CT Angiography as Predictors of Death After Acute Pulmonary Embolism

OBJECTIVE The purpose of this article is to retrospectively compare right ventricular-to-left ventricular (RV/LV) diameter ratios measured on the standard axial view versus the reformatted four-chamber view as predictors of mortality after acute pulmonary embolism (PE). MATERIALS AND METHODS Six hundred seventy-four consecutive patients (mean age, 58 years; 372 women) with a diagnosis of acute PE on pulmonary CT angiography were considered. The axial and reformatted four-chamber RV/LV diameter ratios were compared as predictors of 30-day all-cause and PE-related mortality. RESULTS Ninety-seven patients (14%) died within 30 days; 39 deaths were PE related. There was no significant difference in the univariate hazard ratios (HRs) of axial and four-chamber RV/LV diameter ratios greater than 0.9 for both all-cause (HR, 2.13 [95% CI, 1.29-3.51] vs HR, 1.95 [95% CI, 1.22-3.14]; p = 0.74) and PE-related (HR, 19.6 [95% CI, 2.70-143] vs HR, 21.8 [95% CI, 2.99-158]; p = 1.0) mortality. Axial and four-chamber multivariate HRs accounting for potential confounders such as age and cancer were also similar for all-cause (HR, 1.79 [95% CI, 1.07-2.99] vs HR, 1.54 [95% CI, 0.95-2.49]; p = 0.62) and PE-related (HR, 16.3 [95% CI, 2.22-119] vs HR, 17.7 [95% CI, 2.43-130]; p = 1.0) mortality. There was no significant difference in sensitivity, specificity, negative predictive value, or positive predictive value. Axial and four-chamber measurements were well correlated (correlation coefficient, 0.857), and there was no significant difference in overall accuracy for predicting all-cause (area under the curve [AUC], 0.582 vs 0.577; p = 0.72) and PE-related (AUC, 0.743 vs 0.744; p = 1.0) mortality. CONCLUSION The axial RV/LV diameter ratio is no less accurate than the reformatted four-chamber RV/LV diameter ratio for predicting 30-day mortality after PE.

Quantification of Urinary Stone Volume: Attenuation Threshold–based CT Method—A Technical Note

PURPOSE To compare two threshold-based computed tomographic (CT) methods for the quantification of urinary stone volume; to assess their accuracy and precision at varying tube voltages, tube currents, and section thicknesses in a phantom; and to determine interobserver agreement with each of these methods in a pilot clinical study. MATERIALS AND METHODS After institutional review board approval, written informed consent was waived. The study was HIPAA compliant. Thirty-six calcium oxalate stones were scanned in an anthropomorphic phantom. For the fixed threshold method, stones were segmented with 0.6-mm-thick sections by using attenuation thresholds of 130 and 575 HU (equal to half of mean attenuation of all stones). For the variable threshold method, stones were segmented at an attenuation threshold equal to half of the attenuation of each stone and at variable section thicknesses (0.6, 1, and 3 mm), tube currents (150, 100, and 50 mAs [reference]), and tube voltages (100 and 80 kVp). Normalized Bland-Altman analysis was used to assess the bias and precision of the two CT methods compared with that of the fluid displacement method (reference standard). Two independent readers retrospectively measured stone volumes in 17 patients (male-to-female ratio, 1.4; mean age, 55 years), and interobserver agreement was assessed by using Bland-Altman limits of agreement. RESULTS The variable threshold method was more accurate and precise than the fixed threshold method with an attenuation threshold of 130 HU (P < .0001). Thinner sections (0.6 and 1 mm) resulted in more accurate (P < .05) and precise (P < .0001) stone volume measurements than 3-mm-thick sections. With the variable threshold method, no significant difference was seen in the accuracy and precision of stone volume measurements at various tube currents and tube potentials. Interobserver agreement was high with the fixed and variable threshold methods (r > 0.97). CONCLUSION An attenuation threshold-based CT method can be used to quantify urinary stone volume even at low radiation doses. The most accurate and precise method utilizes variable attenuation derived from the attenuation of each stone and thin sections.

Assessment of image quality in soft tissue and bone visualization tasks for a dedicated extremity cone-beam CT system

AbstractObjectiveTo assess visualization tasks using cone-beam CT (CBCT) compared to multi-detector CT (MDCT) for musculoskeletal extremity imaging.MethodsTen cadaveric hands and ten knees were examined using a dedicated CBCT prototype and a clinical multi-detector CT using nominal protocols (80kVp-108mAs for CBCT; 120kVp- 300mAs for MDCT). Soft tissue and bone visualization tasks were assessed by four radiologists using five-point satisfaction (for CBCT and MDCT individually) and five-point preference (side-by-side CBCT versus MDCT image quality comparison) rating tests. Ratings were analyzed using Kruskal–Wallis and Wilcoxon signed-rank tests, and observer agreement was assessed using the Kappa-statistic.ResultsKnee CBCT images were rated “excellent” or “good” (median scores 5 and 4) for “bone” and “soft tissue” visualization tasks. Hand CBCT images were rated “excellent” or “adequate” (median scores 5 and 3) for “bone” and “soft tissue” visualization tasks. Preference tests rated CBCT equivalent or superior to MDCT for bone visualization and favoured the MDCT for soft tissue visualization tasks. Intraobserver agreement for CBCT satisfaction tests was fair to almost perfect (κ ~ 0.26–0.92), and interobserver agreement was fair to moderate (κ ~ 0.27–0.54).ConclusionCBCT provided excellent image quality for bone visualization and adequate image quality for soft tissue visualization tasks.Key Points• CBCT provided adequate image quality for diagnostic tasks in extremity imaging. • CBCT images were “excellent” for “bone” and “good/adequate” for “soft tissue” visualization tasks. • CBCT image quality was equivalent/superior to MDCT for bone visualization tasks.

Compressed Sensing SEMAC: 8-fold Accelerated High Resolution Metal Artifact Reduction MRI of Cobalt-Chromium Knee Arthroplasty Implants.

ObjectiveThe aim of this study was to prospectively test the hypothesis that a compressed sensing–based slice encoding for metal artifact correction (SEMAC) turbo spin echo (TSE) pulse sequence prototype facilitates high-resolution metal artifact reduction magnetic resonance imaging (MRI) of cobalt-chromium knee arthroplasty implants within acquisition times of less than 5 minutes, thereby yielding better image quality than high-bandwidth (BW) TSE of similar length and similar image quality than lengthier SEMAC standard of reference pulse sequences. Materials and MethodsThis prospective study was approved by our institutional review board. Twenty asymptomatic subjects (12 men, 8 women; mean age, 56 years; age range, 44–82 years) with total knee arthroplasty implants underwent MRI of the knee using a commercially available, clinical 1.5 T MRI system. Two compressed sensing–accelerated SEMAC prototype pulse sequences with 8-fold undersampling and acquisition times of approximately 5 minutes each were compared with commercially available high-BW and SEMAC pulse sequences with acquisition times of approximately 5 minutes and 11 minutes, respectively. For each pulse sequence type, sagittal intermediate-weighted (TR, 3750–4120 milliseconds; TE, 26–28 milliseconds; voxel size, 0.5 × 0.5 × 3 mm3) and short tau inversion recovery (TR, 4010 milliseconds; TE, 5.2–7.5 milliseconds; voxel size, 0.8 × 0.8 × 4 mm3) were acquired. Outcome variables included image quality, display of the bone-implant interfaces and pertinent knee structures, artifact size, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR). Statistical analysis included Friedman, repeated measures analysis of variances, and Cohen weighted k tests. Bonferroni-corrected P values of 0.005 and less were considered statistically significant. ResultsImage quality, bone-implant interfaces, anatomic structures, artifact size, SNR, and CNR parameters were statistically similar between the compressed sensing–accelerated SEMAC prototype and SEMAC commercial pulse sequences. There was mild blur on images of both SEMAC sequences when compared with high-BW images (P < 0.001), which however did not impair the assessment of knee structures. Metal artifact reduction and visibility of central knee structures and bone-implant interfaces were good to very good and significantly better on both types of SEMAC than on high-BW images (P < 0.004). All 3 pulse sequences showed peripheral structures similarly well. The implant artifact size was 46% to 51% larger on high-BW images when compared with both types of SEMAC images (P < 0.0001). Signal-to-noise ratios and CNRs of fat tissue, tendon tissue, muscle tissue, and fluid were statistically similar on intermediate-weighted MR images of all 3 pulse sequence types. On short tau inversion recovery images, the SNRs of tendon tissue and the CNRs of fat and fluid, fluid and muscle, as well as fluid and tendon were significantly higher on SEMAC and compressed sensing SEMAC images (P < 0.005, respectively). ConclusionsWe accept the hypothesis that prospective compressed sensing acceleration of SEMAC is feasible for high-quality metal artifact reduction MRI of cobalt-chromium knee arthroplasty implants in less than 5 minutes and yields better quality than high-BW TSE and similarly high quality than lengthier SEMAC pulse sequences.

The Relationship Between Tibial Tuberosity-Trochlear Groove Distance and Abnormal Patellar Tracking in Patients With Unilateral Patellar Instability.

PURPOSE To evaluate the role of tibial tuberosity-trochlear groove (TT-TG) distance in patellofemoral kinematics by retrospectively reviewing the dynamic computed tomography scans of patients with unilateral patellofemoral instability and comparing unstable and contralateral asymptomatic knees. METHODS We reviewed all dynamic computed tomography scans obtained at one tertiary care hospital from 2008 through 2013 and identified 25 patients with a history of recurrent unilateral patellofemoral instability. During the scans, subjects performed active knee extension against gravity. Both knees were imaged simultaneously. Lateral patellar tilt (LPT) and bisect offset (BO) were measured to assess tracking. TT-TG distance was measured to assess alignment. Measurements were made in full extension, maximum flexion, and approximately 10° increments in between. The significance level was set at P < .05. RESULTS LPT, BO, and TT-TG distance were highest in extension and decreased with flexion. Measurements were higher in symptomatic than in asymptomatic knees, with significant differences identified for LPT, BO, and TT-TG distance at 5° and 15° and for TT-TG distance at 25° and 35° (P < .05). TT-TG distance was associated with LPT and BO, with r(2) values in symptomatic knees of 0.55 for TT-TG distance and LPT and of 0.45 for TT-TG distance and BO. CONCLUSIONS In patients with unilateral patellar instability, LPT, BO, and TT-TG distance are higher on the unstable side. An association exists between TT-TG distance and the tracking parameters studied, suggesting that TT-TG distance relates to patellar tracking, and a laterally positioned tibial tuberosity may predispose to instability episodes. Conventional and novel imaging modalities in osteoarthritis: current state of the evidence.

Purpose of reviewImaging modalities are currently an inseparable part of osteoarthritis diagnosis. In this review, we describe the current state of evidence regarding conventional and novel imaging modalities in evaluation of osteoarthritis. Modalities including radiography (qualitative and semi-quantitative assessments), ultrasonography, computed tomography [CT; conventional multidetector CT (MDCT), cone-beam CT (CBCT) and four-dimensional CT (4DCT)], MRI (MRI; semi-quantitative, quantitative and compositional) and PET and their applications are reviewed. Recent findingsRadiography is the modality of choice for initial assessment of osteoarthritis. However, due to its low sensitivity and specificity, numerous recent investigations have proposed MRI as a powerful addition to detect and grade osteoarthritis features, which are not apparent in radiography. Semi-quantitative MRI measurements are feasible to perform in routine clinical practice. Quantitative and compositional MRI measurements have extended the amount of information an MRI examination can provide regarding the three-dimensional shape and tissue composition of articular cartilage. 4DCT and CBCT are introduced as imaging examinations that may reveal biomechanical cartilage abnormalities in osteoarthritis joint by dynamic and weight-bearing evaluations, respectively. Recent PET studies may unveil the underlying metabolic activities that can be associated with osteoarthritis. SummaryIn addition to the established role of radiographs, MRI is the advanced modality of choice for detection and quantification of various osteoarthritis features. 4DCT and CBCT may have specified applications when diagnosis of underlying motion abnormality or dynamic changes in weight-bearing situation is suspected. Future studies should elucidate the specific clinical applications of ultrasonography and PET.

Extremity cone-beam CT for evaluation of medial tibiofemoral osteoarthritis: Initial experience in imaging of the weight-bearing and non-weight-bearing knee

PURPOSE To investigate differences in joint space width (JSW) and meniscal extrusion (ME) between non-weight bearing (NWB) and weight bearing (WB) examinations of knee joints with medial compartment osteoarthritis (OA) using a cone-beam CT (CBCT) extremity imaging system. MATERIALS AND METHODS In this IRB approved prospective study, informed consent was obtained for 17 patients symptomatic for OA (11 F,6 M; 31-78 years, mean 56 years) and 18 asymptomatic controls (0 F,18 M; 29-48 years, mean 38.5 years) enrolled for CBCT exams in NWB and WB positions. Three independent observers measured medial tibiofemoral JSW and ME. Measurements were compared between NWB and WB images using paired Wilcoxon signed-rank sum test. RESULTS OA subjects exhibited a statistically significant reduction in JSW between NWB and WB scans (average JSW(NWB)(OA)=2.1 mm and JSW(WB)(OA)=1.5 mm, p=0.016) and increase in ME (average ME(NWB)(OA)=6.9 mm and ME(WB)(OA)=8.2 mm, p=0.018)). For non-OA subjects, the change in JSW and ME between NWB and WB exams was reduced (average JSW(NWB)(nonOA)=3.7 mm and JSW(WB)(nonOA)=3.4 mm; average ME(NWB)(nonOA)=2.6 mm and ME(WB)(nonOA)=2.7 mm) and was not statistically significant. Inter-observer agreement was evaluated using Bland-Altman limits of agreement, with good agreement for all measurements (correlation coefficient 0.89-0.98). CONCLUSION The ability to conduct NWB and WB exams in CBCT with a dose profile that is favorable in comparison to multidetector CT (MDCT) and with image quality sufficient for morphological analysis of joint space narrowing and meniscal extrusion could provide a valuable tool for OA diagnosis and treatment assessment.

Imaging characteristics of contralateral asymptomatic patellofemoral joints in patients with unilateral instability.

PURPOSE To test the hypothesis that in patients with unilateral patellofemoral instability ( PI patellofemoral instability ), the contralateral asymptomatic joints have abnormal morphology and imaging features of osteoarthritis ( OA osteoarthritis ) at four-dimensional ( 4D four-dimensional ) computed tomography (CT). MATERIALS AND METHODS The institutional review board approved this HIPAA-compliant retrospective study. Informed consent was waived. Radiology records were reviewed to identify 25 patients (male-female ratio, 0.56; median age, 20 years; range, 13-43 years) with unilateral PI patellofemoral instability undergoing 4D four-dimensional CT and 25 age- and sex-matched control patients undergoing multidetector CT. Trochlear groove depth ( TGD trochlear groove depth ), tibial tuberosity-trochlear groove ( TT-TG tibial tuberosity-trochlear groove ) distance, and patellar height ratio ( PHR patellar height ratio ) were determined to compare morphology, and bisect offset ( BO bisect offset ) measurements were obtained to compare lateral displacement of the patella between the two groups by using the Wilcoxon rank-sum test. All images were interpreted by trained observers. Tracking patterns of the patellae were determined by obtaining BO bisect offset measurements at various flexion angles with 4D four-dimensional CT. RESULTS In the contralateral asymptomatic joints, TGD trochlear groove depth (median, 3.0 mm; 95% confidence interval [ CI confidence interval ]: 2.5, 4.6; P < .0001), TT-TG tibial tuberosity-trochlear groove (median, 15 mm; 95% CI confidence interval : 12.7, 18; P = .008), PHR patellar height ratio (median, 1.17; 95% CI confidence interval : 1.09, 1.2; P = .002), and patellar lateral displacement ( BO bisect offset , 85%; 95% CI confidence interval : 76.2%, 98.2%; P < .0001) were different from measurements obtained in the control group: TGD trochlear groove depth median, 5.0 mm (95% CI confidence interval : 2.2, 7.6); TT-TG tibial tuberosity-trochlear groove median, 10.9 mm (95% CI confidence interval : 3.4, 20.7); PHR patellar height ratio median, 0.92 (95% CI confidence interval : 0.67, 1.36); and BO bisect offset median, 63% (95% CI confidence interval : 59%, 68.4%). OA osteoarthritis was detected in 40% of asymmetrical contralateral joints (10 of 25). By using 4D four-dimensional CT data, multiple regression analysis demonstrated that TGD trochlear groove depth (P = .026) and BO bisect offset measurements obtained at 30° of knee flexion (P = .047) had an association with the presence of OA osteoarthritis . CONCLUSION Abnormal morphology and imaging features of OA osteoarthritis are relatively common in contralateral asymptomatic joints of young patients with unilateral PI patellofemoral instability .
