Ear abnormalities in patients with oculo-auriculo-vertebral spectrum (Goldenhar syndrome)

UNLABELLED Oculo-auriculo-vertebral spectrum (OAVS) is a rare condition characterized by the involvement of the first branchial arches. PURPOSE To investigate the ear abnormalities of a sample of patients with OAVS. MATERIALS AND METHODS The sample consisted of 12 patients with OAVS seen at the Clinical Genetics Unit, UFCSPA/CHSCPA. The study included only patients who underwent mastoid computed tomography and with normal karyotype. We performed a review of its clinical features, giving emphasis to the ear findings. RESULTS Nine patients were male, the ages ranged from 1 day to 17 years. Ear abnormalities were observed in all patients and involved the external (n = 12), middle (n = 10) and inner ear (n = 3). Microtia was the most frequent finding (n = 12). The most common abnormalities of the middle ear were: opacification (n = 2), displacement (n = 2) and malformation of the ossicular chain. Agenesis of the internal auditory canal (n = 2) was the most frequent alteration of the inner ear. CONCLUSIONS Ear abnormalities are variable in patients with OAVS and often there is no correlation between findings in the external, middle and inner ear. The evaluation of these structures is important in the management of individuals with OAVS.

Avaliação de recém-nascidos prematuros durante a primeira oferta de seio materno em uma uti neonatal

PURPOSE: to evaluate newborn prematures during the first offering of maternal breast in a Neonatal Intensive Care Unit. METHODS: 15 mother-newborn dyads were assessed during the first offering of maternal breast in the Neonatal Intensive Care Unit in Maternity Mario Totta in the Hospital Santa Clara da Irmandade Santa Casa de Misericordia de Porto Alegre. The data collection occurred in three parts with each dyad: 1st) Consulting the medical records of newborns and filling out a form. 2nd) Applying a structured questionnaire to mothers. 3rd) Assessment of the first offer of maternal breast applying the Speech Therapy Evaluation Form feedings. RESULTS: 46.7% of newborn had prematurity as the sole reason for admission, 60% were already in speech therapy before initiating breastfeeding. Mean maternal age was 29.27 + / - 8.058 years and 73.3% had a stable relationship with the father of the newborn. It was found that the higher the corrected gestational age of newborns, the best conditions for making the handle and milking breast, and final classification of breastfeeding. The more days of life the newborn had, the better the handle in the maternal breast. CONCLUSION: prematurity was the main factor against breastfeeding, however, the positive characteristics of the mothers, a like educational level, marital status and previous experience in breastfeeding and the early onset of therapeutic sessions in most premature may have provided good results in the first offering of the maternal breast.

Identificação das dúvidas e dificuldades de gestantes e puérperas em relação ao aleitamento materno

Purposes this research aimed to identify and describe the doubts and difficulties of pregnant and postpartum women about breastfeeding, and compare them on the pregnancy period and postpartum period. Methods it is characterized by a cross-sectional, descriptive, comparative, composed of two groups: pregnant and postpartum women. There was the approval of the Ethics Committee in Research / Santa Casa under no 23355. For collect it drafted a questionnaire with questions about breastfeeding. The variables were analyzed by associations between the answers of the groups using the Chi-square test, Fisher’s exact and Student’s t. Results the postpartum women had higher knowledge scores about breastfeeding compared to pregnant women (p = 0,001). By linking the age with the knowledge, the higher the postpartum women age the higher their percentage of knowledge (r = 0, 283, p = 0,011). By analyzing the primiparous mothers, we found that these were more complaints compared to those who already have one or more children (p = 0,014). Conclusions pregnant women, primiparous women and teenager girls have more doubts and difficulties in relation to breastfeeding. The care team must be prepared for the pregnant woman has the right to treat her breast awareness and breast feeding, reaching the puerperal period safer and encouraged for exclusive breastfeeding.

Avaliação das características orofaciais e da amamentação de recém-nascidos prematuros antes da alta hospitalar

Purpose: to evaluate oral characteristics and breastfeeding in premature newborns before hospital discharge and verify the relationship between the oral sensory motor system and breastfeeding. Methods: this was a cross-sectional, quantitative and descriptive study. Data collection was conducted from June to October 2014 and was carried out in three stages: Data collection from medical records, the clinical assessment of the oral sensory motor system and the evaluation of breastfeeding. A convenience sample was obtained from the intensive care unit of a university hospital in Porto Alegre. The inclusion criteria were: prematurity; receiving speech therapy, involvement in the hospital discharge process; recommendation of exclusive breastfeeding; have a free and informed consent form signed by a legal guardian. Results: the sample consisted of 26 subjects, of which 6 individuals were excluded. Most of the characteristics of the oral sensory motor system were adequate and most of the categories evaluated in breastfeeding were close to the maximum score. It was found that premature infants with an alert state of consciousness showed better mother / newborn positioning during breastfeeding (p = 0.043). It was observed that higher corrected gestational ages were associated with better final scores in evaluation of the oral sensory motor system (rs = 0.512, p = 0.021). Conclusions: the study elucidated some characteristics of the oral sensory motor system and of breastfeeding. Additionally, relationships were found between the oral sensory motor system and breastfeeding in premature infants.

Efficacy of Myofunctional Therapy Associated with Voice Therapy in the Rehabilitation of Neurogenic Oropharyngeal Dysphagia: a pilot study

Introduction  Dysphagia causes changes in the laryngeal and stomatognathic structures; however, the use of vocal exercises is poorly described. Objective  To verify whether the therapy consisting of myofunctional exercises associated with vocal exercises is more effective in rehabilitating deglutition in stroke patients. Methods  This is a pilot study made up of two distinct groups: a control group, which performed only myofunctional exercises, and an experimental group, which performed myofunctional and vocal exercises. The assessment used for oral intake was the functional oral intake scale (FOIS). Results  The FOIS levels reveal that the pre-therapy median of the experimental group was 4, and increased to 7 after therapy, while in the control group the values were 5 and 6 respectively. Thus, the experimental group had a statistically significant difference between the pre- and post-therapy assessments ( p  = 0.039), which indicates that the combination of myofunctional and vocal exercises was more effective in improving the oral intake levels than the myofunctional exercises alone ( p  = 0.059). On the other hand, the control group also improved, albeit at a lower rate compared with the experimental group; hence, there was no statistically significant difference between the groups post-therapy ( p  = 0.126). Conclusion  This pilot study showed indications that using vocal exercises in swallowing rehabilitation in stroke patients was able to yield a greater increase in the oral intake levels. Nevertheless, further controlled blind clinical trials with larger samples are required to confirm such evidence, as this study points to the feasibility of conducting this type of research.

Desempenho da alimentação oral em recém-nascidos prematuros estimulados pela técnica treino de deglutição

Introduction Prematurity can affect proper development of a newborn. Purpose To evaluate performance of oral feeding in preterm infants stimulated by the swallowing training technique. Methods The study was conducted in a neonatal intensive care unit from March to August 2015. The study was developed as a Times Series Quasi Experiment with 14 preterm newborns (PTNB) with corrected gestational age between 30-36 weeks. The readiness to feed orally protocol for preterm infants (pre and post-swallowing training) was applied. Swallowing exercises were conducted once a day and on average for six consecutive days. Results Improvement post-stimulation was observed, comparing the pre-intervention and post-intervention protocol data. Regarding oral feeding skills, 50% of preterm infants were classified as level 4. Oral feeding commenced, on average, 1 day after the end of swallowing training; the gastric tube was removed approximately 7 days after beginning oral feeding. There was a borderline inverse relationship between improvement in the readiness score and transition time from tube to full oral feeding and an inverse relationship between improvement in the readiness score and corrected gestational age. Conclusion The intervention with swallowing training made it possible to improve feeding skills in 50% of the sample population, starting oral feeding and obtaining exclusive oral feeding in a short period of time, with oral readiness at early gestational ages.

Disfagia orofaríngea na Síndrome de Wallenberg - série de casos

Purpose: characterizing the impairment condition of the swallowing function in patients with Wallenberg Syndrome. Methods: case series of seven patients, with diagnosis of this syndrome, referred for phonological assessment. The Gugging Swallowing Screen scale was used to evaluate the level of dysphagia and the Functional Oral Intake Scale was used to evaluate the level of oral ingestion. Results: the mean age was 60.57 years; all subjects presented changes in the function of swallowing of severe degree (71.42%) to moderate degree (28.58%); 85.71% required Alternative Feeding Route, wherein, 71.43% were fed exclusively by nasoenteric tube; all subjects required speech therapy. Conclusion: this study concluded that oropharyngeal dysphagia in Wallenberg Syndrome presents itself as a disorder of serious to moderate degree, being the use of Alternative Feeding Route required in most cases.

Association between neurological injury and the severity of oropharyngeal dysphagia after stroke

Purpose To associate the degree of biomechanical impairment in the swallowing process with the severity (National Institute of Health Stroke Scale - NIHSS) and type of neurological injury in patients post stroke. Methods A cross-sectional, descriptive study conducted with 42 patients (22 females), aged 65.7 years on average diagnosed with stroke. All patients underwent clinical neurological evaluation and application of the NIHSS in the first 48 hours after stroke. The swallowing function was evaluated using the Functional Oral Intake Scale (FOIS) and the Protocol for the Investigation of Oropharyngeal Dysphagia in Adults. The Fishers Exact Probability Test was used to assess the correlation between the degree of swallowing impairment and the severity (NIHSS score) and type of stroke. The study results were statistically analyzed at 5% significance level (p≤0.05). Results 92.9% of the patients presented ischemic stroke; 59.5% presented impairment of the anterior cerebral circulation. Statistically significant correlation was found between the neurological scale (NIHSS) scores and the swallowing impairment scale (p=0.016). Conclusion An association between stroke severity and oropharyngeal dysphagia severity was observed. A high proportion of patients with ischemic stroke with circulation affected in the anterior cerebral region presented severe oropharyngeal dysphagia. No statistically significant correlation was observed between the FOIS scale and stroke severity.

Alterações estomatognáticas e de fala são comuns entre crianças com incontinência pigmentar

Purpose: To identify possible speech-language disorders in children with Incontinentia Pigmenti (IP), seeking to characterize the role of speech therapy in the evaluation and management of this genetic condition. Methods: The sample was composed of seven female children diagnosed with IP. Results: The patients in the sample had a mean age of 6.4 years. Among the main structural features verified in the patients, highlighted the presence of no physiological diastema and hard palate abnormalities, found both in 85.7 % of the sample, in addition to tooth agenesis in 71.4% of cases. As for functional findings, 71.4 % of the sample had abnormal tongue mobility and 57.1%, inappropriate chewing. As for changes in speech, the main findings consisted of phonetic/phonological alterations, verified in 85.7 % of the sample, being the most common phonetic alteration characterized by distortion of alveolar fricative [s], present in 57.1 % of cases. None of the children had abnormal voice and swallowing according to the used protocol. Furthermore, no detectable hearing abnormality was observed according to claim of the family or by observation during the evaluation. Conclusion: In this sample the most frequent speech-language alterations verified among the patients with IP were mainly related to the stomatognathic system structures and speech.
