OBJECTIVE We present an extremely rare case of isolated angiokeratoma of the tongue. METHOD Case report and review of related literature. RESULTS An 18-year-old, male adolescent presented with a fleshy, intermittently bleeding mass in the posterior third and base of the tongue. The lesion was initially suspected to be a lingual thyroid or haemangioma, but histopathological features were consistent with angiokeratoma. Magnetic resonance imaging revealed that the lesion extended up to the vallecula and involved the lamina propria and superficial tongue musculature. No similar lesions were found elsewhere in the body. No metabolic derangements were identified in the patient or his family. The 2.6 × 1.5 × 0.5 cm mass was excised under general anaesthesia. CONCLUSION We present the 1st case of isolated lingual angiokeratoma in a male, the 4th such case overally, the largest ever documented. The lesion was situated in the posterior third and base of the tongue, a position not previously described.

Nonosseous or soft tissue Ewings sarcoma is a rare form of Ewings sarcoma/primitive neuroectodermal tumour that seldom affects the head and neck region. Involvement of the nose and paranasal sinuses is extremely uncommon, with only eight of such patients being reported to date, mostly affecting adolescents and young adults. To our knowledge, this study is the first comprehensive report of primary soft tissue Ewings sarcoma involving the paranasal sinuses in an elderly patient who successfully completed treatment. We herein discuss the pathogenesis, management and factors affecting the prognosis of this rare group of tumours involving the nose and paranasal sinuses, in relation to the available literature.

Even though laryngeal malignancies are the most frequent primary malignancies of the upper aero digestive tract except for oral cavity cancers, laryngeal chondrosarcomas are rare tumors, constituting less than 1% of all laryngeal tumors. We present a rare case of chondrosarcoma arising from the right arytenoid cartilage with sub glottic extension. The mode of presentation and management of the case is presented along with a review of the literature.

A 24-year-old woman presented with a painless swelling in her right pinna which was noticed about four months back and had a recent increase in size. On examination, a 1.5×1 cm, firm, non-tender skin-colored nodule was seen in the helix. A clinical diagnosis of keloid was rendered. Considering chances of recurrence following surgical excision and because the woman was not concerned about cosmesis, we preferred to wait-and-watch. We were skeptical about the provisional diagnosis though, as she gave no history of ear-prick, a custom common among women for wearing earrings, and also because there was a history of transient peri-lesional reddening one month back. She returned two weeks later with the swelling increased in size, erythematous, and covered by crust. The scab was removed and the raw, bleeding, necrotic surface exposed (Fig. 1). Recurrent Acute Exacerbations of Chronic Rhinosinusitis (AECRS) in a setting of atrophic rhinitis are perplexing clinical situations. While either may act as the primary factor, repeated inflammation in a vessel-poor atrophied tissue is unusual. Progressive mucosal degeneration, vascularity and the effect of bacterial colonization are the factors suggested to play an important role for the pathobiogenesis of the co-existence of the two diseases. Through presentation of a case, a brief attempt has been made here to explore the temporal relationship between rhinosinusitis and atrophic rhinitis; in the process, other noteworthy features of the patient have also been highlighted.

Clotrimazole, a broad-spectrum imidazole antimycotic, is a widely-used prescription as well as an over-the-counter drug for treatment of mucosal (oral, vulvovaginal) candidiasis and dermatophytes. An extremely safe and well-tolerated drug, its safety profile has been well-explored in researches and trials. The commonest complaints are almost always non-serious-pruritus, rashes, paraesthesia (stinging/burning sensation) and erythema on topical applications, and nausea, vomiting, unpleasant taste sensation and elevated liver enzymes on oral preparations (troches). However, mucosal ulceration on topical clotrimazole preparation has not been reported in literature before. We present here the first documentation of extensive bilateral erosions and necrosis of tongue involving the mucosa, submucosa and musculature on application of one percent weight/volume clotrimazole mouth paint in a 27-year-old immunocompetent woman who presented with oral candidiasis.
