Obturators are routinely used in the rehabilitation of maxillectomy defects. Ideally, obturators should be easily constructed to produce a comfortable and stable prosthesis. This article describes a technique that provides an easy method for the construction of an obturator prosthesis using a visible light-cured (VLC) resin. As a result, technicians can easily construct obturators in little time; this would, in turn, provide patients with light, comfortable, and tolerable prostheses.

The aim of this study was to evaluate the stress distribution in the bone around dental and zygomatic implants for 4 different implant-supported obturator prostheses designs in a unilaterally maxillary defect using a 3-dimensional finite element stress analysis. A 3-dimensional finite element model of the human unilateral maxillary defect was constructed. Four different implant-supported obturator prostheses were modeled; model 1 with 2 zygomatic implants and 1 dental implant, model 2 with 2 zygomatic implants and 2 dental implants, model 3 with 2 zygomatic implants and 3 dental implants, and model 4 with 1 zygomatic implant and 3 dental implants. Bar attachments were used as superstructure. A 150-N vertical load was applied in 3 different ways, and von Mises stresses in the cortical bone around implants were evaluated. When the models (model 1-3) were compared in terms of number of implants, all of the models showed similar highest stress values under the first loading condition, and these values were less than under model 4 conditions. The highest stress values of models 1-4 under the first loading condition were 8.56, 8.59, 8.32, and 11.55 Mpa, respectively. The same trend was also observed under the other loading conditions. It may be concluded that the use of a zygomatic implant on the nondefective side decreased the highest stress values, and increasing the number of dental implants between the most distal and most mesial implants on the nondefective side did not decrease the highest stress values.

BACKGROUND The objective of this study was to investigate the stress distribution in the bone around zygomatic and dental implants for 3 different implant-retained obturator prostheses designs in a Aramany class IV maxillary defect using 3-dimensional finite element analysis (FEA). MATERIAL AND METHODS A 3-dimensional finite element model of an Aramany class IV defect was created. Three different implant-retained obturator prostheses were modeled: model 1 with 1 zygomatic implant and 1 dental implant, model 2 with 1 zygomatic implant and 2 dental implants, and model 3 with 2 zygomatic implants. Locator attachments were used as a superstructure. A 150-N load was applied 3 different ways. Qualitative analysis was based on the scale of maximum principal stress; values obtained through quantitative analysis are expressed in MPa. RESULTS In all loading conditions, model 3 (when compared models 1 and 2) showed the lowest maximum principal stress value. Model 3 is the most appropirate reconstruction in Aramany class IV maxillary defects. Two zygomatic implants can reduce the stresses in model 3. The distribution of stresses on prostheses were more rational with the help of zygoma implants, which can distribute the stresses on each part of the maxilla. CONCLUSIONS Aramany class IV obturator prosthesis placement of 2 zygomatic implants in each side of the maxilla is more advantageous than placement of dental implants. In the non-defective side, increasing the number of dental implants is not as suitable as zygomatic implants.

The Cleft Lip and Palate (CLP) is variation of a type of clefting congenital anomaly caused by abnormal facial development during pregnancy. The etiology of CLP is unknown. The CLP patient is mainly characterized by the presence of an oronasal communication, malformation or congenital agenesis of the teeth close to the cleft and deficient sagittal and transverse growth of the maxilla. CLP is needing complex treatment strategies scheduled in many years of duration. These patients require various treatments involving a multidisciplinary team, which may include a maxillofacial surgeon, an orthodontist, prosthodontist, an ear nose and throat specialist(ENT), speech therapist and all those professionals who can help provide functional and aesthetic improvement. CLP patients may reconstructed with conventional prostheses or (dental-zygomatic) implant retained prostheses. Clinicians still have some difficulties in prosthetic reconstruction. Prosthetic reconstruction of the premaxillary region is important for these patients. In this case report, to restore masticatory function and improve speech and esthetics of the 21 years old cleft lip and palate patient fixed- removable protheses is used and advantages and disadvantages of prosthetic reconstruction are discussed By applying prothetic rehabilitation the function, phonation and esthetic were reformed. As a result of these deficiencies physcosocial problems were abolished. During one year clinical observation it was constated that the patient was living without any problem

Patients who underwent maxillectomy operation are faced with many problems like aesthetic, phonetic, functional, psychological. The treatment of patients is quite difficult and troublesome. Different treatment alternatives are available for maxillectomy patients. There are advantages and disadvantages of each treatments. The primarily aim of prosthetic rehabilitation is to cover the maxillary defect and eliminate the problems. The best treatment option is obturator when surgical techniques are not sufficient. Clinical findings etiology and prognosis of the case must be evaluated completely for exact treatment and suitable technique must be apply for treatment. In this article, rehabilitation with obturator prosthesis was explained for two patients subjected to maxillary resection at the end of tumoral reason
